RESUMO

The article presents a case of idiopathic hypertrophic pachymeningitis of a 61-year-old male patient with severe cephalgia and progressive neuropathy of the oculomotor nerves. The diagnosis was confirmed by MRI with gadolinium, which revealed thickening of the dura mater with accumulation of paramagnetic in the convexital parts of the frontal and temporal regions, as well as on the base of the skull and tentorium. During the use of pulse therapy with glucocorticosteroids (GCS) the symptoms regressed, but when the therapy was stopped, there was a relapse of ptosis and oculomotor abnormalities on the other side followed by a slower effect of GCS therapy. The article also presents a brief review of current knowledge about this pathology.

RESUMO

OBJECTIVE: To test a new approach to gait stereotype correction for patients with central hemiparesis with talipes equinovarus. MATERIAL AND METHODS: The study was performed in a group of patients with formed talipes equinovarus and post-stroke hemiparesis. Footwear with orthopaedic elements was offered to the patients. Evaluation of spatial and temporal gait parameters was performed and the risk of falls was assessed. RESULTS: In the process of work, data were obtained confirming the effectiveness of using specialized shoes for equinovarus foot placement. The risk of falling significantly decreased when walking; walking became more symmetrical due to an increase in the anterior extension of the paretic limb. CONCLUSION: Application of this method does not decrease the tone in the paretic limb but optimises the gait stereotype, facilitates the increases of its velocity and decreases the risk of falling.

OBJECTIVE: To estimate the informativity, sensitivity and specificity of the method of assessing gait function in patients with ataxia and post-stroke hemiparesis using the LA-1 «Laser Analyzer of Kinematic Gait Parameters¼. MATERIALS AND METHODS: Main tempo-rhythm and spatial gait parameters were assessed in groups of healthy people of young (n=30) and middle (n=30) age. At the second stage, the gait parameters of healthy people were compared with those of patients with ataxia due to vertebrobasilar stroke (n=30) and with post-stroke hemiparesis (n=30). The following parameters were evaluated: the average length and duration of step, the standard deviation of step length, the standard deviation of step duration, coefficient of variability for step length & duration. RESULTS: The high sensitivity and specificity of the method were established. Standard values were determined for all indicators in groups of healthy people and statistically significant features of the main gait parameters for these syndromes were identified. There were an increase in the step length and duration, standard deviation of step length and duration, step variability coefficients, and a decrease in walking speed. Conclusion. The results are in line with those of earlier studies on the gait change in patients with post-stroke disorders. The LA-1 method can be recommended for clinical use.

RESUMO

The article describes the clinical search for diagnosis and treatment of a patient who developed a severe ischemic stroke at a young age, when specifying the cause of which chronic myeloleucosis was identified. The unavailability of patient compliance and non-compliance with the recommended examination at the outpatient stage led to the development of severe complications. It would seem that the diagnosis of chronic myeloid leukemia, which does not represent excessive complexity, with the development of an acute condition in a young patient, such as an ischemic stroke, results in a complex diagnostic search, complicated by developing comorbidities due to general decompensation of the body.

RESUMO

Stiff-person syndrome (SPS) is a rare chronic neurological disease characterized by progressing muscle rigidity and painful muscle spasms. The signs of SPS are pain and stiffness in spinal, abdominal and cervical muscles, increased muscle tonus in extensor muscles of extremities, constant stiffness of paravertebral and abdominal muscles and muscle spasms. A clinical case of a SPS patient T., aged 23 years, is presented. The peculiarity of this case is additional left-sided peripheral upper extremity monoparesis, which is most likely associated with the development of left-sided compression-ischemic brachial plexopathy resulted from profound muscular tonic syndrome in the neck and shoulder girdles.

RESUMO

The article presents the mode of detection of anti-thrombocyte auto-antibodies using flow cytometry technique. The values of IgG conjugated with thrombocytes are established in healthy persons. The rate of frequency of auto-antibodies to thrombocytes is analyzed in patients with primarily diagnosed immune thrombocytopenia, with relapse of disease and after therapy of immune thrombocytopenia. The carried out studies demonstrate that flow cytometry technique has a high sensitivity to detect thrombocyte-related antibodies and can be applied under examination of patients with thrombocytopenia.

RESUMO

The article considers immune phenotyping heterogeneity of chronic lymphatic leukemia detected using basic diagnostic markers ofcell. The results of analysis of immune phenotypes of 108 patients with B-cell lymphatic proliferative diseases made it possible to establish that the atypical is related most rarely to indicators of expression of monotypic immunoglobulines and CD5 and most frequently to CD23, FMC7, CD22 and CS79b. During the present observation, the immune phenotyping count made up "3" or "2"points and the atypical alternative was registered among 10% of all examined patients with chronic lymphatic leukemia. It is demonstrated that patients with chronic lymphatic leukemia and with lower immune phenotyping count are characterized by major intensity of tumor substrate.

AIM: to analyze the efficacy of RFC (rituximab, fludarabine, and cyclophosphan), FCM (fludarabine, cyclophosphan, and mitoxantrone), and FC (fludarabine and cyclophosphan) treatment programs in patients with chronic lymphocytic leukemia (CLL) in an open-labeled comparative controlled investigation. MATERIALS AND METHODS: The paper presents the authors' results of treatment in patients with progressive CLL in 2002 to 2007. The study included 229 patients, of them 78 patients received the RFC program, 72 had the FCM program, and 79 had the FC one. RESULTS: With the use of RFC, a clinically significant therapeutic effect was obtained in 96% of the patients, a complete remission (CR) was in 80% of the primary patients and in 53% of the pretreated patients. When the FCM program was applied, a positive response was noted in 93% of the patients, CR was seen in 75 and 42% of the primary and pretreated patients, respectively. In the treatment of FC, the total effect was 80%, CR was in 41 and 14% of the primary and pretreated patients, respectively. CONCLUSION: Comparative analysis of an objective response to therapy has indicated that the effectiveness of the RFC significantly exceeds that of the FCM and FC programs, without enhancing toxicity, which allows he RFC regimen to be regarded as the program of choice in therapy for CLL.

RESUMO

Results of epidemiologic, virologic, and serologic studies of enterovirus infections in Khabarovsk region from 1975 to 2006 were analyzed. Patterns of epidemic process of these infections were established: periodic change of dominating type of pathogen in the population; onset of the large epidemic peaks of incidence during emergence of circulation of new for the given area serotypes of enteroviruses; possibility of realization of several routes of virus transmission. Role of water factor in the progress of the epidemic process was revealed. Etiology of the large epidemic rise of aseptic meningitis incidence in Khabarovsk region in 2006 was established--the leading pathogens were ECHO viruses serotypes E6 and E30.

RESUMO

Epidemiological and social aspects of the vaccinal prophylaxis of medical personnel against hepatitis B are discussed on the basis of the morbidity analysis in viral hepatitis, the results of dynamic screening for the presence of hepatitis B markers and the state of postvaccination immunity. The reactogenicity and immunogenicity of the vaccine "Euvax B", as well as the socio-economic effect of vaccinal prophylaxis, are evaluated.

RESUMO

Bubo-M, the first Russian associated vaccine, was found to have low reactogenicity and high immunogenic potency. The frequency of postvaccinal reactions in the group of persons immunized with Bubo-M (20%) appeared to be considerably lower than among persons who received the combined injection of adsorbed DT toxoid with reduced antigen content and vaccine against hepatitis B (47.7%). Following the course of vaccination the level of anti-HBs considerably exceeded the protective level. Immune response to the diphtheria and tetanus components of Bubo-M exceeded that observed after immunization with absorbed DT toxoid with reduced antigen content (p < 0.05).

RESUMO

The results of immunological control of infections with different mechanisms of the transmission of their causative agents (diphtheria, measles, viral hepatitis, salmonellosis) for many years are summarized. About 25,000 serum samples taken from humans and more than 10,000 probes from agricultural animals have been studied in the passive hemagglutination test and more than 3,600 serum samples have been studied in the enzyme immunoassay for the presence viral hepatitis markers. Planned immunological control has ensured greater possibilities of the epidemiological diagnosis and prognostication of complications in the epidemic and epizootic situations. Urgent immunological control has made it possible to establish the proportion of non-manifest forms of diseases, to differentiate the foci of infection by transmission routes, to determine the degree of protection from infection and to substantiate the tactics of individual immunization.

RESUMO

The authors have investigated the possibility of using various albumin preparations as calibrators in measurements of human blood serum total protein by the biuret method. Analysis of Precinorm U and Precipath U reference sera has demonstrated that use of various albumin preparations as calibrators may result in significant deviations (as much as 27%) of the resultant values from the due ones.
