RESUMO

Gastrointestinal haemorrhage is a common cause of emergency admission of patients to surgical hospitals. Within the structure of nosological entities, not unreasonably referred to the rarest causes of gastrointestinal bleeding is the formation of an aortointestinal fistula whose early diagnosis is of paramount importance. The clinical picture may be different but it is mostly represented by gastrointestinal haemorrhage. The incidence of gastrointestinal fistulas following a surgical intervention ranges from 0.6 to 2.3%. Unless timely diagnosed and with incorrect therapeutic decision-making, the mortality rate amounts to 90%. In this article we present a clinical case report regarding successful treatment of a patient presenting with a secondary aortoduodenal fistula occurring 5 years after previously performed aortofemoral bypass grafting and complicated by relapsing intestinal bleeding and acute ischaemia of the right lower extremity.

RESUMO

It was performed a comparative analysis of treatment of 62 patients who were operated for morbid obesity. Patients were divided into 3 groups depending on kind of surgery: jejunal-iliac intestinal bypass, restrictive operations and combined operations. The complications in early postoperative period were diagnosed in 16.6% of patients after jejunal-iliac intestinal bypass and in 10% of patients after combined operations. Regardless of the kind of surgery there was rapid reduction of the body mass in the first year of observation with gradual increase of weight by an average of 5% in the subsequent period. The highest rate of weight loss by the second year was revealed after combined operations (up to 73.4%). Thus it was 1.1 times more than after jejunal-iliac intestinal bypass and 1.3 times more than after restrictive operations.

Bashkortostan National Council of Ministers adopted a resolution (No 136, April 12, 1995) "On formation of interdepartmental Committee for Industrial Hygiene in Bashkortostan Republic". The Committee puts into practice principles of the Governmental policy in industrial hygiene according to a law "On industrial hygiene in Bashkortostan Republic"; deals with problems and makes proposals in industrial hygiene; coordinates the specific activities of the Ministries, Departments and public organization of Bashkortostan Republic; cooperates with the competent professionals, organizations and researchers for progress in industrial hygiene.

RESUMO

The investigation has been conducted to study the influence of middling and low-frequency ultrasound on the culture of standard microorganisms in vitro and in vivo and on the wound process in 26 patients. It has been shown that the middling ultrasound has a priority bactericidal effect upon the microorganisms and the wound infection. It also makes the process of depuration more active.

RESUMO

Experiments were conducted to study the acid-base equilibrium of blood in pancreatitis and in combination with laser irradiation of venous blood by the intravascular method through a light guide. Irradiation of blood was attended by inhibition of metabolic acidosis development, which was evidently linked with improved blood oxygen transport function in response to exposure to the laser beam.

RESUMO

Experimental investigations and experience with treatment of 74 patients with pancreonecrosis have shown the highest effectiveness of associated usage of hemosorption and external elimination of lymph from the thoracic duct. Associated detoxication for pancreonecrosis facilitates to rapidly decrease the level of the intoxication which is demonstrated by positive dynamics of biochemical and toxicological parameters of blood and lymph.

Basic systemic hemodynamic parameters were compared during intravenous anesthesia with calypsol (ketamine) and diazepam, promedol and diazepam, fentanyl and diazepam, fentanyl and sombrevin in 120 patients operated on the lungs with conventional and high-frequency jet ventilation. The studies have shown that in conventional controlled lung ventilation there were no distinctions in hemodynamic parameters depending on the type of the anesthesia. In high-frequency jet ventilation, specific hemodynamic effects of calypsol were observed. Unlike other anesthetics, calypsol administration is associated with elevated central venous pressure, increased cardiac output and enhanced left ventricular work, which can be accounted for by higher venous return.

RESUMO

Data are given on the specific features of the diagnosis and treatment of more than 2000 pancreatitis patients. In 388 of them the clinical signs of affection of the spine were verified by X-ray: intervertebral osteochondrosis and osteoarthrosis of the costovertebral articulations. The appropriate correction of changes in the spine promotes more rapid improvement of the state of the pancreas.

Inpatient examinations included 17 patients with necrosis of the pancreas (4 patients had hemorrhagic and 14 fat necrosis): synthetic lymphocyte activity was studied by the luminescent method and the erythrocyte content of adenyl nucleotides by thin-layer chromatography in the toxic phase of pancreatitis. Synthetic lymphocyte activity was found to be decreased and the ATP pool of erythrocytes exhausted in the toxic phase of pancreatitis. Intravascular laser irradiation of the blood caused recovery of lymphocyte activity and increase in the ATP content in erythrocytes.

RESUMO

Experimental (118 experiments) and clinical (30 patients) investigations have shown a positive effect of endovascular irradiation of blood with the helium-neon and ultraviolet lasers on recovery of homeostasis in pancreatitis and peritonitis. The procedure stimulates the erythrocyte bioenergetics, reduces the degree of haemic hypoxia and increases nonspecific immunological reactivity.
