RESUMO

Sixty-five patients with lichen planus were included in a multicenter trial of acitretin. At the end of an 8-week placebo-controlled, double-blind phase, a significantly higher number of patients treated with 30 mg/day acitretin (64%) showed remission or marked improvement compared with placebo (13%). Furthermore, during the subsequent 8-week open phase, 83% of previously placebo-treated patients responded favorably to acitretin therapy. Typical retinoid adverse reactions were present in all patients on active drug. Laboratory studies did not show any clinically significant changes. This study shows that acitretin is an effective and acceptable therapy for severe cases of lichen planus.

RESUMO

In a double-blind randomized study, sixty-two patients suffering from psoriasis or eczema were treated, twice daily for three weeks, either with 0.05% betamethasone 17, 21-dipropionate cream or with 0.025% fluocinolone acetonide cream. The results showed that both topical corticosteroid preparations were effective, well tolerated and cosmetically acceptable. Fifty-seven per cent of the patients treated with betamethasone dipropionate were rated as being 'much better' in the overall assessment of response at the end of the trial period compared to only 25% of those in the fluocinolone acetonide group.

Circulating immune complexes (CIC) were measured by three different methods in serum from 17 patients with systemic lupus erythematosus (SLE), 3 patients with "hydralazine-induced" SLE-like syndromes, 14 patients with discoid lupus (DLE), 8 patients with systemic sclerosis and 5 patients with dermatomyositis. Immune complexes were detected in 13 of the 17 patients with SLE. All patients with lupus nephritis and typical exanthema had circulating immune complexes. The concentration of immune complexes was inversely correlated to serum complements C4 and C3. All 3 patients with "hydralazine-induced" SLE-like syndromes had circulating immune complexes that disappeared after withdrawal of the drug. Immune complexes were detected in 3 of the 14 patients with DLE; all 3 patients with CIC had wide-spread DLE. In systemic sclerosis, CIC were detected in only 1 of the 8 patients. Four of the 5 patients with dermatomyositis demonstrated CIC in serum. No complement consumption was detected in dermatomyositis and the immune complexes may have been secondary to tissue destruction.

RESUMO

A case of transient nephrotic syndrome caused by secondary syphilis is described. A renal biopsy was performed revealing subepithelial hump-like electron-dense deposits and fusion of epithelial foot-processes. Complement C1q-binding-activity and anticomplementarity were demonstrated in the blood, indicating the presence of circulating immune complexes. This strongly suggests that circulating immune complexes are significant in the immunopathogenesis of syphilitic nephropathy.

RESUMO

PIP: A case of perforation of the uterus by a Lippes loop IUD in a 33-year-old woman is described. X-rays had shown the IUD in its characteristic intrauterine position after it had perforated the uterus and migrated extrauterinely. Hysterosalpingography was required to locate the IUD. Characteristic symptoms of these patients are discussed.^ieng
