RESUMO

Purified Shilajit, an Ayurvedic rasayana, was evaluated in healthy volunteers of age between 45 and 55 years for its effect on male androgenic hormone viz. testosterone in a randomised, double-blind, placebo-controlled clinical study at a dose of 250 mg twice a day. Treatment with Shilajit for consecutive 90 days revealed that it has significantly (P < 0.05) increased total testosterone, free testosterone and dehydroepiandrosterone (DHEAS) compared with placebo. Gonadotropic hormones (LH and FSH) levels were well maintained.

RESUMO

Extensive use of quantum chemical calculations has been made to rationally design a molecule whose spin state can be switched reversibly using light of two different wavelengths at room temperature in solution. Spin change is induced by changing the coordination number of a nickel complex. The coordination number in turn is switched using a photochromic ligand that binds in one configuration and dissociates in the other. We demonstrate that successful design relies on a precise geometry fit and delicate electronic tuning. Our designer complex exhibits an extremely high long-term switching stability (more than 20 000 cycles) and a high switching efficiency. The high-spin state is extraordinarily stable with a half-life of 400 days at room temperature. Switching between the dia- and paramagnetic state is achieved with visible light (500 and 430 nm). The compound can also be used as a molecular logic gate with light and pH as input and the magnetic state as non-destructive read-out.

Magnetic bistability, as manifested in the magnetization of ferromagnetic materials or spin crossover in transition metal complexes, has essentially been restricted to either bulk materials or to very low temperatures. We now present a molecular spin switch that is bistable at room temperature in homogeneous solution. Irradiation of a carefully designed nickel complex with blue-green light (500 nanometers) induces coordination of a tethered pyridine ligand and concomitant electronic rearrangement from a diamagnetic to a paramagnetic state in up to 75% of the ensemble. The process is fully reversible on irradiation with violet-blue light (435 nanometers). No fatigue or degradation is observed after several thousand cycles at room temperature under air. Preliminary data show promise for applications in magnetic resonance imaging.

Centella asiatica is reputed for its beneficial effects in various neurological disorders. The present investigation was undertaken to evaluate the role of 70% hydro-ethanolic extract of Centella asiatica (CA) on generalized anxiety disorder (GAD) in man. Hamilton's Brief Psychiatric Rating Scale (BPRS) was used to screen the subjects. Thirty-three participants (18 male and 15 female; average age 33 yrs) were medicated with the CA in a fixed dose regime (500 mg/capsule, twice daily, after meal). They were thoroughly investigated using standard questionnaires based on psychological rating scale at baseline (day 0), mid-term (day 30) and final (day 60). The scale also includes a number of direct queries about current levels of experienced stress. The observations revealed that, CA not only significantly (p<0.01) attenuated anxiety related disorders but it also significantly (p<0.01) reduced stress phenomenon and its correlated depression. CA further significantly (p<0.01) improved the willingness for adjustment and cognition. Results indicated that Centella asiatica may be useful in the treatment of GAD and may be used as a promising anxiolytic agent in near future.

The safety and spermatogenic activity of processed Shilajit (PS) were evaluated in oligospermic patients. Initially, 60 infertile male patients were assessed and those having total sperm counts below 20 million ml(-1) semen were considered oligospermic and enrolled in the study (n = 35). PS capsule (100 mg) was administered twice daily after major meals for 90 days. Total semenogram and serum testosterone, luteinising hormone and follicle-stimulating hormone were estimated before and at the end of the treatment. Malondialdehyde (MDA), a marker for oxidative stress, content of semen and biochemical parameters for safety were also evaluated. Twenty-eight patients who completed the treatment showed significant (P < 0.001) improvement in spermia (+37.6%), total sperm count (+61.4%), motility (12.4-17.4% after different time intervals), normal sperm count (+18.9%) with concomitant decrease in pus and epithelial cell count compared with baseline value. Significant decrease of semen MDA content (-18.7%) was observed. Moreover, serum testosterone (+23.5%; P < 0.001) and FSH (+9.4%; P < 0.05) levels significantly increased. HPLC chromatogram revealed inclusion of PS constituents in semen. Unaltered hepatic and renal profiles of patients indicated that PS was safe at the given dose. The present findings provide further evidence of the spermatogenic nature of Shilajit, as attributed in Ayurvedic medicine, particularly when administered as PS.

Ocimumn sanctum, an Indian medicinal plant, has been on trial for its role in generalized anxiety disorder (GAD) in hospital based clinical set-up. Hamilton's brief psychiatric rating scale (BPRS) and thorough clinical investigations were used to screen the subjects. Thirty-five subjects (21 male and 14 female; average age 38.4 years) were medicated with the plant extract in a fixed dose regime (500 mg/capsule, twice daily, p.o. after meal). They were thoroughly investigated clinically and using standard questionnaires based on different psychological rating scale at baseline (day 0), mid-term (day 30) and final (day 60). The observations exhibited that, O. sanctum significantly (p<0.001) attenuated generalized anxiety disorders and also attenuated its correlated stress and depression. It further significantly (p<0.001) improved the willingness to adjustment and attention in human. Therefore, it may be concluded that O. sanctum may be useful in the treatment of GAD in human and may be a promising anxiolytic agent in near future.

Tuberculosis of Tonsil and Oropharynx are almost a forgotten condition now- a- days. Persistent sore throat, ulcer over tonsils and unilateral tonsillar enlargement should arose suspicion of tubercular infection. Confirmation oj which depends on histopathology favouring tuberculosis and /or AFB positivity on tonsillar tissue. Condition is curable with proper Anti- TB treatment with or without tonsillectomy, A case is presented considering its rarity.

Postictal values of plasma prolactin levels were measured in 15 children with generalized seizures, 8 children with pseudoseizures and 6 control subjects. In patients with generalized seizure, the mean plasma prolactin level was 28.6 +/- 2.3 ng/ml, whereas in patients with pseudoseizures and in controls, its mean values were 10.4 +/- 3.8 ng/ml and 9.8 +/- 2.6 ng/ml, respectively. Thus, prolactin levels were significantly (p < 0.001) elevated following generalized seizures but were almost normal following pseudoseizures. Plasma prolactin levels may, therefore, be helpful in differentiating between generalized seizures and pseudoseizures.

Postictal serum prolactin and cortisol levels were estimated in 73 children having either epilepsy, febrile seizures, breath-holding spells, or fever without other manifestation and in 20 normal controls. Mean serum prolactin levels (28.6 +/- 2.3 ng/ml) were significantly higher (p < 0.001) in the epileptic group than in the group with febrile seizures (12.7 +/- 2.8 ng/ml), non-specific febrile illness (12.2 +/- 2.4 ng/ml), breath-holding spells (8.8 +/- 1.1 ng/ml) and normal controls (9.8 +/- 2.6 ng/ml). Mean serum cortisol levels were non-specifically elevated in children with epilepsy (32.8 +/- 2.2 ug/dl), febrile convulsion (34.2 +/- 4.1 ug/dl) and non-specific febril illness (30.6 +/- 2.4 ug/dl). Our observations suggest that elevated prolactin levels associated with afebrile epileptic seizures may help in differentiating epilepsy from febrile seizures and breath-holding spells. Cortisol levels appear to be non-specifically elevated in all stressful conditions.

A pair-matched controlled study was done with 30 children (15 males, 15 females) of mild to moderate PEM in test and equal number in control groups, aged 8-24 months old. At the beginning, in both test and control groups parents were advised to provide sufficient amount of home cooked foods so that the child received on an average 100-150 Kcal/kg/day of energy and 2-3 g/kg/day of protein. Vitamin supplements were provided to all children in both groups. In addition, the test group received a supplementation of 5.625 mg of elemental zinc daily orally while controls received a placebo. Evaluation at the end of 3 months showed that children in test group had a weight gain of 3.742 +/- 0.488 kg against 2.035 +/- 0.383 kg of the control group. Similarly, weight velocity was 5.752 +/- 0.818 g/kg/day in test group against 3.153 +/- 0.617 g/kg/day of the control group. These differences in weight gain and weight velocity were highly significant (p < 0.001). Serum zinc levels, initially and at the end of study were 87.5 +/- 9.6 micrograms/dl and 121.0 +/- 10.1 micrograms/dl, respectively in test group in comparison to 91.2 +/- 9.8 micrograms/dl and 91.0 +/- 10.1 micrograms/dl in controls. This difference was also highly significant (p < 0.001). The daily total calorie intake remained comparable in both groups. Our observations suggest that zinc supplementation during nutritional rehabilitation of mild to moderate PEM hastens the recovery.
