RESUMO

We conducted a meta-analysis to assess the effect of endoscopic submucosal dissection compared with gastrectomy on the wound infection in early stomach cancer subjects. A systematic literature search up to November 2022 was performed and 2765 related studies were evaluated. The chosen studies comprised 7842 early stomach cancer subjects participated in the selected studies' baseline trials; 3308 of them used the endoscopic submucosal dissection, while 4534 used gastrectomy. Odds ratio (OR) with 95% confidence intervals (CIs) were calculated to assess the wound infection in endoscopic submucosal dissection versus gastrectomy for early stomach cancer by the dichotomous methods with a random or fixed effect model. The use of endoscopic submucosal dissection resulted in significantly lower wound infection (OR, 0.45; 95% CI, 0.34-0.60, P < .001) with no heterogeneity (I2  = 8%) compared with the gastrectomy for early stomach cancer. The use of endoscopic submucosal dissection resulted in significantly lower wound infection compared with the gastrectomy for early stomach cancer. The small sample size of some studies in the comparison calls for care when analysing the results.

Dystrophic epidermolysis bullosa (DEB) is a series of severe genetic conditions affecting skin and nails caused by mutations in the COL7A1 gene. DEB has a strong phenotypic variability. In the present study, we recruited a case with a boy exhibiting typical DEB indication, and performed a clinical, genetic, and experimental investigation, followed by a prenatal diagnosis on their current pregnancy. Whole exome sequencing identified a novel compound heterozygous variation in COL7A1, consisting of two variants, namely c.191T>C (p.Leu64Pro) and c.5124G>A (p.Leu1708=) in the proband. In vitro study by minigene system indicated that c.5124G>A would result in an increased ratio of a transcript with exon-skipping, which supported its pathogenicity. Further prenatal detection confirmed the genotype-phenotye co-separation in this family. In conclusion, the findings in our study expanded the mutation spectrum of DEB, and emphasized the importance of paying attention to specific synonymous variants in the filtering process.

The preparation of hydrogels for wound healing properties with high antibacterial activities and good biosafety concurrently can be relatively challenging. For addressing these issues, we report on the synthesis and characterisation of a nanocomposite hydrogel dressing by introducing the silver nanoparticles in hydroxypropyl methylcellulose-hydroxyapatite scaffold hydrogel (HMC-HA/AgNPs). The different concentrations of AgNPs in HMC-HA/AgNPs hydrogels were confirmed by swelling ratio, degradation, and gelatin time. The synthesised HMC-HA/AgNPs hydrogels were further characterised using the UV-visible, scanning electron microscopy, transmission electron microscopy, Fourier transform infrared spectrum, and X-ray diffraction. The results showed that the novel HMC-HA/AgNPs hydrogel exhibited a porous 3D network and high mechanical properties because of the inter-molecular and intra-molecular interactions. The AgNPs give the HMC-HA hydrogels excellent antibacterial activities against both Staphylococcus aureus and Escherichia coli, without any chemical reductant and cross-linking agent required endows the hydrogel high biocompatibility. More importantly, HMC-HA/AgNPs effectively repaired wound defects in mice models, and wound healing reached 94.5 ± 1.4% within 16 days. The HMC-HA hydrogel with AgNPs showed excellent antimicrobial activity and burn wound healing. Therefore, these HMC-HA/AgNPs hydrogels have great potential as an injectable hydrogel for wound healing activity in children with burn injuries.

Small supernumerary marker chromosomes (sSMCs) are a group of rare chromosomal anomalies, which pose challenges in the clinical practice of prenatal diagnosis and genetic counseling. This study enrolled an extended family with an underage male patient displaying infantile seizures, intellectual disability, and retarded speech and psychomotor function. A series of multiplatform genetic detections was conducted to explore the diagnostic variation. Whole exome sequencing (WES) and chromosomal microarray analysis (CMA) indicated a mosaic sSMC derived from the pericentromeric region of chromosome 8 in the patient, which was confirmed using cytogenetic methods. The proband and his mother, who carried this mosaic variant, exhibited strong phenotypic variability. We also ruled out the pathogenicity of a KDM5C variant by extended validation. Our results emphasized the capacity of WES to detect mosaic SMCs and the importance of mosaic ratios in the appearance and severity of symptomatic phenotypes.

OBJECTIVE: To study the expressions of NLRP3 inflammasome and its downstream molecules in the gastric tissues and sera of Helicobacter pylori (H.pylori)-infected C57BL/6 mice, and to preliminary explore the role of NLRP3 inflammasome signalling pathway in the pathogenesis of H.pylori infection. METHODS: C57BL/6 mice were randomly divided into H.pylori infection group and PBS control group, and mice were sacrificed at different time points after H.pylori infection. The mRNA expressions of NLRP3 inflammasome and downstream signalling pathway related molecules in mouse gastric tissues were detected by RT-PCR, and the protein level of caspase-1 was analyzed by Western blotting. The contents of IL-1ß, IL-18 and IL-33 in the sera were measured with ELISA. RESULTS: Chronic inflammatory response was observed in the gastric mucosal tissues of H.pylori-infected mice and it gradually aggravated. Compared with the control mice, the mRNA expressions of NLRP3 inflammasome signalling pathway related molecules and the protein level of caspase-1 increased markedly in the gastric tissues of H.pylori-infected mice. Moreover, the contents of IL-1ß, IL-18 and IL-33 in the sera of H.pylori-infected mice were also significantly elevated. CONCLUSION: NLRP3 inflammasome signalling pathway could be activated by H.pylori infection.
