RESUMO

Objective To explore the mediating effect of health-promoting lifestyle on the relationship between health literacy and stigma among middle-aged and elderly patients with type 2 diabetes mellitus (T2DM). Methods A stratified random sampling method was used to select 415 patients with T2DM from three general hospitals in Weifang to conduct a questionnaire survey using the Disease Stigma Assessment Scale for Type 2 Diabetes Mellitus (DSAS-2), the Health Literacy Scale, and the Type 2 Diabetes Health Promotion Scale (T2DHPS). Results Sickness stigma was negatively correlated with health literacy (r=-0.547, P<0.01) and negatively correlated with health promotion (r=-0.505, P<0.01), while health promotion was positively correlated with health literacy (r=0.398, P<0.01). Health-promoting lifestyle mediated the association between health literacy and stigma (β=0.0383, P<0.01), with the mediating effect accounting for 16.00% of the total effect. Conclusion Health literacy can influence the stigma of patients with type 2 diabetes mellitus through health promotion lifestyle, suggesting that the stigma of patients with diabetes can be improved through health promotion lifestyle intervention.

Lithium-sulfur batteries (LSBs) are some of the most promising energy storage systems to break the ceiling of Li-ion batteries. However, the notorious shuttle effect and slow redox kinetics give rise to low sulfur utilization and discharge capacity, poor rate performance, and fast capacity decay. It is proved that the reasonable design of the electrocatalyst is one of the important ways to improve the electrochemical performance of LSBs. Here, a core-shell structure with gradient adsorption capacity for reactants and sulfur products was designed. The Ni nanoparticles core coated with graphite carbon shell was prepared by one-step pyrolysis of Ni-MOF precursors. The design takes advantage of the principle that the adsorption capacity decreases from the core to the shell, and the Ni core with strong adsorption capacity is easy to attract and capture soluble lithium polysulfide (LiPS) during the discharge/charging process. This trapping mechanism prevents the diffusion of LiPSs to the outer shell and effectively inhibits the shuttle effect. In addition, the Ni nanoparticles within the porous carbon, as the active center, expose most of the inherent active sites to the surface area, thus achieving a rapid transformation of LiPSs, significantly reducing the reaction polarization, and improving the cyclic stability and reaction kinetics of LSB. Therefore, the S/Ni@PC composites exhibited excellent cycle stability (a capacity of 417.4 mA h g-1 for 500 cycles at 1C with a fading rate of 0.11%) and outstanding rate performance (1014.6 mA h g-1 at 2C). This study provides a promising design solution of Ni nanoparticles embedded in porous carbon for high-performance, safe and reliable LSB.

Thiazolidinedione PPARγ agonists such as rosiglitazone and pioglitazone are effective antidiabetic drugs, but side effects have limited their use. It has been posited that their positive antidiabetic effects are mainly mediated by the inhibition of the CDK5-mediated Ser273 phosphorylation of PPARγ, whereas the side effects are linked to classical PPARγ agonism. Thus compounds that inhibit PPARγ Ser273 phosphorylation but lack classical PPARγ agonism have been sought as safer antidiabetic therapies. Herein we report the discovery by virtual screening of 10, which is a potent PPARγ binder and in vitro inhibitor of the CDK5-mediated phosphorylation of PPARγ Ser273 and displays negligible PPARγ agonism in a reporter gene assay. The pharmacokinetic properties of 10 are compatible with oral dosing, enabling preclinical in vivo testing, and a 7 day treatment demonstrated an improvement in insulin sensitivity in the ob/ob diabetic mouse model.

Objective: To observe the long-term toxicity of chelerythrine on the morphology of lung tissue and the expression level of nuclear factor-kappa B (NF-κB) in lung tissue of the rats, and to investigate the related mechanism which causing the lung tissue damage of rats. Methods: A total of 120 Wistar rats were divided into control group (given normal saline) and low (3. 7 mg middot; kg-1), moderate (5. 6 mg middot; kg-1), high (8. 4 mg middot; kg-1) dosages of chelerythrine groups (n=30). The general condition of rats in various groups was observed; HE staining was used to observe the morphology of lung tissue of the rats in various groups; the serum interleukin-6 (IL-6), interleukin-8 (IL-8) and tumor necrosis factor-α (TNF-α) levels of the rats in various groups were measured by ELISA method; real-time fluorescence quantitative PCR and Western blotting methods were used to detect the mRNA and protein levels of NF-κB and intercellular adhesion molecule-1 (ICAM-1). Results: The accumulative mortality of the rats in high dosage of chelerythrine group was the highest, followed by moderate and low dosages of chelerythrine groups. The body weights and food intakes of the rats in different dosages of chelerythrine groups were significantly lower than that in control group (P<0. 05), while the body weight and food intake of the rats in high dosage of chelerythrine group were lower than those in low and moderate dosages of chelerythrine groups (P<0. 05). Chelerythrine led to pulmonary congestion and bloody ascites of the rats. The HE staining results showed that the injuries of lung tissue in different dosages of chelerythrine groups were aggravated with the increasing of the dosage of chelerythrine. Compared with control group, the levels of IL-6, IL-8, and TNF-α in serum of the rats in different dosages of chelerythrine groups were increased significantly (P< 0.05); compared with low and moderate dosages of chelerythrine groups, the levels of IL-6, IL-8, and TNF-α in serum of the rats in high dosage of chelerythrine group were increased significantly (P<0. 05). Compared with control group, the expression levels of NF-κB and ICAM-1 mRNA and proteins in lung tissue of the the rats in different dosages of chelerythrine groups were increased significantly (P<0. 05) in a dose-dependent manner; compared with low and moderate dosages of chelerythrine groups, the expression levels of NF-κB and ICAM-1 mRNA and proteins in lung tissue of the rats in high dosage of chelerythrine group were increased (P<0. 05). Conclusion: Chelerythrine has a long-term toxic effect on lung tissue in a dose-dependent manner of the rats. Moderate and high dosages of chelerythrine may aggravate the toxic lung injury through activation of NF-κB and production of inflammatory cytokines.

Objective To prepare scutellarin liposomes and to investigate the pharmacokinetics in rats. Methods The liposomes of scutellarin coated with carbopol were prepared with the film dispersion method.Minicolumn centrifugation method was applied to measure the encapsulation efficiency (EE) before and after coating. SD rats were randomly divided into three groups, which were given intragastric administration of scutellarin suspension, scutellarin liposomes, and carbopol-coated scutellarin liposomes, respectively. And then the pharmacokinetic parameters were compared. Results Carbopol-coated scutellarin liposomes after intragastric administration in rats showed pharmacokinetic characteristics of two-compartment model. The main pharmacokinetic parameters of scutellarin suspension,scutellarin liposomes,and carbopol-coated scutellarin liposomes were as follows:AUC0-∞(50.03±13.45) μg?h?mL-1 ,(78.99±20.28) μg?h?mL-1 ,and (107.97±27.26) μg?h?mL-1 , respectively. Conclusion After coated liposomes, the oral bioavailability of scutellarin 1iposomes can be significantly improved,and the maximum drug concentration also increased.

This study aimed at optimizing the extraction process of Bi Xie Fen Qing (BXFQ) drink using ultrasound-microwave cooperation with central composite design-response surface method in comparison with the traditional decoction process.Taking microwave frequency,amount of water,extraction time as the main detection factors,diosgenin,glycyrrhizic acid and the extract yield were tested as the evaluation indexes;and taking an integrated score as response value for response surface and contour,predictive analysis was carried out and the optimum extraction conditions were achieved.It was found that the optimum extraction process of BXFQ drink was identified:the microwave frequency was 434 W,water addition was 18.4 times,extraction time was 9.3 mins and the ultrasonic frequency was fixed at 50 W.Under the optimum process,the diosgenin extraction yield rate was 23.17% (mg· g-1),extraction yield rate of glycyrrhizic acid was 0.64% (g·g-1),and the extraction yield rate of extractum was 34.12% (g·g-1).All the indexes were superior to those of the traditional method.It is concluded that the composite design-response surface method is suitable for the extraction optimization of BXFQ drink with favorable predictability of the mathematical model.The optimized ultrasoundmicrowave cooperation was easy to operate with high extraction efficiency.It is suitable for industrialized production with the provision of a scientific reference for the modern formulation development of BXFQ drink.

Objective@#This study aimed to measure the morphological parameters of the internal acoustic meatus(IAM) and its adjacent structures using temporal-bone thin-section CT(computed tomography).@*Methods@#CT images were obtained from 50 Chinese adult patients (25 males and 25 females, 100 sides) which had no visible lesion in the petrous part of the temporal bone and inner ear, the morphological parameters of all inner ear parts were sectionally measured on the specified plane using SPSS 22.0 software for statistical analysis.@*Results@#The integral morphological characteristics of the IAM were observed. These results revealed that anterior-posterior diameter of the internal acoustic poer(IAP)(CD) was (6.93±1.85)mm, the superior-inferior diameter of the IAP(EF) was (4.40±0.86)mm, the length of the IAM(AB) was (9.30±1.60)mm, the superior-inferior diameter of the IAM(the intersection of inner 1/3 section and middle 1/3 section) was (4.13±0.83)mm, the superior-inferior diameter of the IAM(the intersection of middle 1/3 section and outer 1/3 section) was (4.61±1.02)mm, the anterior-posterior diameter of the IAM(the intersection of inner 1/3 section and middle 1/3 section) was (6.62±1.92)mm, the anterior-posterior diameter of the IAM(the intersection of middle 1/3 section and outer 1/3 section) was (6.28±1.65)mm, the depth of transverse crest (superior wall) was (3.10±0.75)mm, the depth of transverse crest (interior wall)the was (1.46±0.59)mm, the distance from transverse crest vertex A to the superior wall of the IAM was (2.05±0.42)mm, the distance from transverse crest vertex A to the interior wall of the IAM was (2.93±0.41)mm, the thickness of the superior bone wall of the IAM (the intersection of inner 1/3 section and middle 1/3 section) was (4.45±1.34)mm, the thickness of the superior bone wall of the IAM (the intersection of middle 1/3 section and outer 1/3 section) was (4.32±1.12)mm, the thickness of the superior bone wall of the IAM (the intersection of outer 1/3 section and transverse crest vertex) was (4.37±1.28)mm, and the appearance ratio of the cells in the whole IAM superior wall was 32%.The whole IAM assumed the shape of short cylinder, inclining about 1 cm outward, with the upper-lower diameter and anterior-posterior diameter about 5 mm.@*Conclusion@#It is necessary for carrying out preoperative the temporal-bone thin-section CT to obtain the morphological parameters of the IAM, determine its basic morphology, and provide references to avoid damaging the other important structures during IAM surgeries.

Objective To compare the long-term efficacy of the traditional open surgery (OSP) with the thoracic endovascular aneurysm repair (TEVAR) in the treatment of patients with the complicated Stanford B aortic dissection. Methods The retrospective analysis of the clinical data was conducted in 118 patients (OSP 45 vs. TEVAR 73) with the complicated type B aortic dissection from January 2004 to January 2015. The long-term survival rate was compared between the two groups and the influencing factors of the postoperative survival was analyzed by using the Cox-regression analysis. Results Significant differences in age, sex and operation time were observed between the two groups (P 0.05). The refractory hypertension and preoperative aortic overall diameter > 55 mm were the risk factors for the long-term survival (P = 0.021, OR = 11.1, 95%CI:1.428 ~ 86.372; P = 0.001, OR = 4.5, 95%CI: 1.842 ~ 11.346). Conclusions Compared with OSP, no obvious advantage of TEVAR was shown in the ten-year overall cumulative survival. In view of the influence factors of refractory hypertension and the aortic diameter for the long-term survival , we should pay attention to the blood pressure control and the performance of endovascular repair technology whether pre- or post-operation.

Objective To evaluate the application value of arcuate fasciculus diffusion tensor imaging (DTI) combined with neuronavigation in epilepsy surgery. Methods A retrospective analysis of 24 patients with refractory epilepsy, admitted to our hospital from March 2011 to March 2014, was performed; the epileptic foci of these patients were adjacent to the language functional areas conformed by imaging before operation. Surgical resection was performed with the help of arcuate fasciculus of DTI, diffusion tensor fiber tracking (DT-FT) combined with neuronavigation. The surgical efficacy and language function of the patients were evaluated by Engel classification and Verbal Behavior Assessment Scale (VerBAS) at postoperative follow-up. Results During the follow-up from 12 to 47 months, according to Engel classification, in 7 patients with foci adjacent to Broca area, Engel grade I was achieved in 3, Engel grade II in 3 and Engel grade Ⅲ in one; and in 17 patients with foci adjacent to Wernicke area, seizure control was as follows: Engel grade I was noted in 13, Engel grade II in 2 and Engel grade III in 2. No perpetual language dysfunction after operation was noted in all the patients. Conclusion Arcuate fasciculus DTI combined with intraoperative neuronavigation has very high application value in the epilepsy surgery.

Objective To investigate the risk factors of postoperative hemorrhage after pancreatoduodenectomy (PD).Methods The retrospective case-control study was adopted.The clinical data of 857 patients with pancreatic diseases who were admitted to the First Affiliated Hospital of Hunan Normal University from January 2007 to December 2014 were collected.All the 857 patients underwent PD and digestive tract reconstruction using the Child method.The number of patients with postoperative hemorrhage,classification,bleeding sites,source and time of bleeding and method and effect of treatment after PD were observed.The correlations among the gender,age,concomitant diseases (diabetes and hypertension),malignancy degree of tumor,the preoperative levels of serum alanine transaminase (ALT),total bilirubin (TBil),albumin (Alb) and prothrombin time (PT),international normalized ratio (INR),operation time,volume of intraoperative blood loss,method of pancreatic and jejunal anastomosis and postoperative hemorrhage after PD were analyzed.The follow-up of outpatient examination and telephone interview was performed to observe postoperative recovery of patients for 2 months till February 2015.Univariate analysis and multivariate analysis were done using the chisquare test and Logistic regression model,respectively.Results Of 72 patients with postoperative hemorrhage,grade A,B and C hemorrhage were detected in 3,41 and 28 patients,respectively,and 41,29 and 2 patients had respectively enteral hemorrhage,parenteral hemorrhage and enteral and parenteral hemorrhage.After PD,38 patients had hemorrhage located at the gastrointestinal tract,9 at the common hepatic artery,proper hepatic artery and gastroduodenal artery (5 due to pseudoaneurysm),5 at the pancreatic section,3 at the jejunal mesenteric vessels,2 at the middle colic arterial branches,1 at the superior mesenteric artery,1 at the superior mesenteric vein and 13 at the ambiguous bleeding sites.The early and late stage hemorrhages (within postoperative hour 24 and after postoperative hour 24) were detected in 20 and 52 patients,respectively.Of 44 patients with grade A and B of hemorrhages,17 underwent conservative treatment,16 underwent reoperation,8 underwent hemostatic therapy under gastroscopy,3 underwent interventional treatment.All the 44 patients had good hemostasis effect.Of 28 patients with grade C of hemorrhage,interventional treatment,reoperation,hemostatic therapy under gastroscopy,conservative treatment,interventional treatment + reoperation and gastroscopy + interventional treatment were applied to 10,7,4,3,3 and 1 patients,respectively.Ten of 28 patients died and 18 had successful hemostasis.The gender and preoperative levels of ALT and TBil were related factors affecting postoperative hemorrhage after PD in the univariate analysis (x2 =4.516,7.585,7.209,P < 0.05).Male,preoperative ALT ≥ 172 U/L and preoperative TBil ≥ 159 μmol/L were the independent risk factors affecting postoperative hemorrhage after PD in the multivariate analysis (HR =2.033,1.860,1.872,95% confidence interval:1.237-3.341,1.135-3.047,1.060-3.307,P < 0.05).Fifty of 62 patients were followed up for a median time of 2 months with a follow-up rate of 80.6% (50/62),and no rehemorrhage was occurred.Conclusion Male,preoperative ALT≥172 U/L and preoperative TBil≥≥ 159 μmol/L are the independent risk factors affecting postoperative hemorrhage after PD.

Objective To detect the expression of stem-cell related factors Nanog and CD44 in spheroid body-form?ing cells of gastric cancer cell line MKN-45. Methods The gastric cancer cell line MKN-45 was used to culture spheroid bodies in non-adherent condition in a serum-free medium supplemented with epidermal growth factor (EGF) and basic fibro?blast growth factor (bFGF). Using Western blot analysis, immunofluorescence staining and quantitative real time polymerase chain reaction(qRT-PCR), the expression levels of stem cell-related genes Nanog and CD44 were studied. Results In this study, we observed that MKN45 cells formed spheroid bodies in non-adherent condition in a serum-free medium, and the levels of Nanog and CD44 mRNA expression in spheroid body-forming cells were 2.34 ± 0.22 and 1.18 ± 0.04,respectively, which were higher than those in parental cells (1.00±0.00 and 1.00±0.05). The levels of Nanog and CD44 protein expression in spheroid body-forming cells were 0.18±0.02 and 0.24±0.04, respectively, which were significantly higher than those in pa?rental cells (0.07±0.02 and 0.18±0.01, P<0.05). Nanog protein was positively stained within the perinuclear and cytoplasm of the spheroid body-forming cells, and CD44 was positively stained mainly in the membrane. Dual staining of Nanog/CD44 indicated that the embryonal protein Nanog was co-localized with CD44 in the spheroid body-forming cells. Conclusion Spheroid body-forming cells developed from human gastric cancer cell line MKN-45 in serum-free medium supplemented with EGF and bFGF show characteristics of cancer stem cell (CSC). The cells co-expressed of CD44 and Nanog maybe a phe?notype of gastric CSCs.

Objective To evaluate the clinical effect of functional neuronavigation combined with intra-operative magnetic resonance imaging (iMRI) in resection of symptomatic occipital lobe epilepsy.Methods Sixteen patients with symptomatic occipital lobe epilepsy,admitted to our hospital from January 2011 to April 2012,were involved in present study.All patients underwent surgical operation assisted by functional neuronavigation.Before the operation,MRI scanning,imaging fusing,scope of lesion excision describing,and operative approach designing were performed successively.Three-D reconstructed navigation plan was integrated into neuronavigation system and used by combining with functional neuronavigation under microscope during the surgical procedure.And,the iMRI was performed in five of them; the operative plan of the residuary lesion was redesigned accordingly.After the operation,the regular clinical and iconography follow-up were in progress.Results The functional neuronavigation was successfully completed.The lesions,posterior central gyrus and pyramidal tracts were projected into operation microscope.The lesions were precisely excised,and the posterior central gyrus and pyramidal tracts were protected effectively.The last iMRI indicated that the range of resection was coincided with the pro-operative plan.During the follow-up of 12-24 months,11 patients were found to be completely seizure-free or with only aura (Engel I),and 5 patients had only rare seizure (Engel Ⅱ,fewer than three seizures per year).Nine patients appeared visual field defect without hemiparalysis or hemianesthesia.Conclusion Functional neuronavigation combined with iMRI is a safe and reliable technique for perfect resection of symptomatic occipital lobe epilepsy and reduction of normal brain functions by precisely locating lesions and important functional structures.

This article was aimed to study the protective effects and mechanism of resveratrol (Res) on endothelial injury induced by atherosclerosis ( AS ) . Lysophosphatidylcholine ( LPC ) was applied to induce the model of injured endothelial cells. Flow cytometer was used to detect the rate of cell apoptosis. TUNEL staining was used to detect the cell apoptosis. MTT colorimetric test was used to detect the cell viability. Automatic biochemistry analyzer was used to measure the LDH level . ELISA was used to detect TNF-α level . RT-PCR was used to determine the mRNA levels of calcitonin gene-related peptide (CGRP). The results showed that Res with the concentration of 20 μmol/L can effectively slow down the degree of endothelial cell injury induced by LPC which caused the cell disability, high LDH level and high TNF-α level (P < 0.01). Res showed effects on mRNA expression of CGRP. It was concluded that Res can protect against endothelial injury induced by AS, the mechanism of which may be associated with accelerating CGRP synthesis and release by activating VR1.

<p><b>OBJECTIVE</b>To explore the method for the isolation, cultivation, and purification of adipose-derived stem cells (ADSCs) and examine the oncogenesis and homing of ADSCs to the liver in vivo.</p><p><b>METHODS</b>ADSCs were isolated from female mice by digestion with 0.075% collagenase I and the morphology of the isolated cells was observed with examination of the cell surface markers and cell cycle. BALB/c mice were injected with 1×10(6) ADSCs on the back to evaluate the oncogenesis of ADSCs or with 1×10(6) ADSCs stained with 5, 6-carboxyfluorescein diacetate-succinimidyl ester (CFSE) via the tail vein to examine the cell homing to the liver.</p><p><b>RESULTS</b>The isolated ADSCs highly expressed CD29 and CD44 and were negative for CD34, CD45, CD11b and CD14. Cell cycle distribution analysis showed cell percentages in G0/G1, S, and G2/M phases of 80.1%, 7.9%, and 12%, respectively. The ADSCs had a low immunogenicity and did not express CD40, CD80, CD86, MHCI, MHCII or PDL-1. After stimulation with IFN-γ, the expression of CD40, CD80 and PDL-1 were up-regulated slightly in the cells. Dorsal injection of the ADSCs did not result in any tumor formation within 1 month, and ADSCs injected via the tail vein showed cell homing to the liver.</p><p><b>CONCLUSION</b>Murine ADSCs can be isolated and expanded effectively by collagenase digestion and adherent culture. The isolated ADSCs can successfully reside in the liver after implantation, and thus may serve as a promising candidate cell in stem cell therapy of liver diseases.</p>

<p><b>OBJECTIVE</b>To investigate the effect of tumor necrosis factor-α (TNF-α) on nutritional status and proteolysis of respiratory muscle in a rat model of chronic obstructive pulmonary disease (COPD).</p><p><b>METHODS</b>Ninety healthy male adult Wistar rats were randomly divided into model group (A) and normal control group (B). COPD malnutrition rat models were established by cigarettes smoke and nutrient limitation and divided into normal nutrition COPD group (A(1)), malnutrition COPD group (A(2)), and malnutrition COPD intervention group (A(3)). In group A(3), the rats received intravenous injection of TNF-α mAb (0.1 mg/kg). TNF-α levels in the serum and respiratory muscle homogenates were measured using enzyme-linked immunosorbent assay (ELISA), and plasma levels of glucose, albumin, and triglyceride were measured with an automatic biochemistry analyzer. High-performance liquid chromatography was used to measure the contents of 3-methylhistidine and tyrosine in the respiratory muscle homogenates.</p><p><b>RESULTS</b>The serum TNF-α level and plasma levels of glucose and triglyceride were significantly higher but the plasma albumin level was significantly lower in group A(2) than in groups B, A(1), and A(3) (P<0.01). The contents of 3-MH and Tyr in the respiratory muscle homogenates were significantly higher in group A(2) than in the other 3 groups (P<0.01, P<0.01). TNF-α in the respiratory muscle showed a strong positive correlation to 3-MH and Tyr.</p><p><b>CONCLUSION</b>TNF-α is one of the causes of increased proteolysis of the respiratory muscle.</p>

RESUMO

The clinical and genetic characteristics in a patient with 46,XY complete gonadal dysgenesis was investigated. Clinical features and laboratory data were collected from the patient and the family. The exon of SRY gene was amplified by PCR and sequenced. The patient presented with primary amenorrhea, nonambiguous female external genitalia, slight breast development, and no axillary hair or pubic hair. The female internal reproductive organs consisted of uterus and streaks of ovarian tissue. Howerver, the chromosome karyotype was 46,XY. A missense mutation of A66T in SRY gene was identified, which was not previously reported. The novel SRY mutation caused the sex reversal in this 46,XY female patient.

RESUMO

This study is to investigate the transportation of scutellarin in cell and live models and study on mechanism of absorption and transport of scutellarin in mouse liver. The concentration of scutellarin in plasma and liver from control and pretreated groups was determined by high performance liquid chromatography. The uptake of scutellarin was examined in control hepatocytes group, induced hepatocytes group and induced hepatocytes plus pravastatin group. Pravastatin can affect the pharmacokinetics of scutellarin in mouse: CL is decreased while AUC is increased. The scutellarin absorption of hepatocyte induced group was higher than that of control group, but was decreased in the group with pravastatin added. The research showed that there was potential drug interaction between pravastatin and scutellarin. The drugs may compete for oatp2 mediated transport pathway consisted in the uptake of scutellarin in liver.

Objective To investigate the clinical and genetic characteristics in a male patient with 21hydroxylase deficiency combined with adrenal and testicular tumors.Methods Clinical features and laboratory data were collected from the patient.Testicular biopsy was performed.The CYP21 gene was sequenced for mutations.Results The patient presented left adrenal and testicular enlargements.The laboratory examinations showed that plasma ACTH,androstenedione,testosterone,progesterone,and 17-hydroxyprogesterone were markedly elevated.CT scan revealed that the right adrenal gland being resected and the left adrenal with nodular enlargement.Furthermore,testicular biopsy showed a prominent peritubular fibrosis with increased number of peritubular fibroblasts,tubular hyalinisation,and calcification.Sequencing analysis showed a A>G homozygous mutation at intron 2.Conclusion Patients with untreated 21-hydroxylage deficiency may.have adrenal adenomas and(or)testicular adrenal rest tumor simultaneously.

BACKGROUND: Traditional treatment includes conservative therapy and surgical treatment on bronchial pleural fistula. With the development of endoscopic therapy, a new safe and effective method is concerned. OBJECTIVE: To summarize the therapeutic effect of treating bronchopleural fistula using bronchoscopy-guided injection of fibrin sealant combined with gelatin sponge filled and microwave following after lung resection. METHODS: Totally 8 cases of bronchial fistula patients were received injection of fibrin sealant after microwave and gelatin sponge filled by bronchoscopy, including 6 males and 2 females, mean aged 53.8 years (39-73 year-old). Cases 1 and 2 received left upper pulmonary Iobectomy, cases 3 received left lower pulmonary Iobectorny, case 4 received right lower pulmonary Iobectomy, cases 5 and 6 received right upper pulmonary Iobectomy, and the cases 7 and 8 received pneumonectomy. bronchopleuralfistula occurred after postoperative Chemotherapy in cases 4 and 8. RESULTS AND CONCLUSION: The case 8 was failed to treatment due to the large orificium fistulae (> 5 mm) and disappointed results after 6 times plugging. The other 7 patients were cured successfully. The injection time was correlated to the size of orificium fistulae, and the plugging was successful for 4 patients with less than 3 mm orificium fistulae. However, a third or fifth plugging was performed for 3 cases with 3-5 rnm orificium fistulae. No surgery-related complications occurred in all patients. Bronchoscopy guided injection of fibrin sealant combined with gelatin sponge and microwave is a safe and effective method for bronchial fistula.
