RESUMO

Objective To explore the influence of family-integrated transition care on the daily living ability of discharged patients with stroke. Methods Seventy-eight patients with stroke who were admitted to Renmin Hospital of Wuhan University from May 2016 to October 2017 were selected by convenience sampling and were divided into a control group and a family-integrated transition care group (hereinafter referred to as transition care group). The patients in the control group received routine neurological health education, while those in the transition care group received a family-integrated transition care intervention in addition to routine neurological health education. The family-integrated transition care included team building, skills training for family members,family-integrated guidance for discharged patients, and regular visits. The scores of the modified Barthel index were compared between the two groups of patients at discharge, three months after intervention, and six months after intervention. Results Among the 71 patients that were finally included,35 cases were included in the control group,of which 17 cases were men (49%),18 cases were women (51%),and their mean age was (70.1±3.7) years;the transition care group comprised 36 cases,of which 18 cases were men (50%),18 cases were women (50%),and their mean age was (69.8±4.5) years. The baseline scores of the control group and transition care group on the day of discharge were (49.1 ± 7.5) and (49.7 ± 7.9),respectively,with no significant difference (P>0.05).In terms of time effects, the scores of the patients in the two groups had statistically significantly improved at six months after discharge (P<0.05).In the group comparison, the scores of the patients in the transition care group after the intervention were significantly higher compared to the scores of those in the control group (P<0.05).In terms of time and inter-group effects, there was an interaction (P<0.05),and therefore, the influence of time effects was excluded and the same timepoint was compared between the two groups. The scores at three months (63.9±8.8) and six months (76.9± 10.1) in the transition care group were higher than those in the control group (58.1 ±8.1 and 66.0 ±9.3,respectively).The difference was statistically significant (P < 0.05).Conclusion Family-integrated transition care can effectively improve daily living ability and isworthy of promoting.

RESUMO

Objective: To explore the clinical significance of 7-joint ultrasound score (US7) method in evaluating the progression of RA. Methods Totally 198 RA patients were divided into 3 groups according to disease activity score of 28 joints (DAS28), i.e. light active period group (n=55), medium active period group (n=83) and heavy active period group (n=60). Gray-scale ultrasound (GSUS) and power Doppler ultrasound (PDUS) were performed on the severe lateral wrist joints, the second/third metacarpophalangeal joints, proximal interphalangeal joints and the second/fifth metatarsophalangeal joints. US7 scoring was performed. The differences among 3 groups were compared, and their correlation with the severity of the disease were analyzed. Logistic regression analysis was performed to find the predictive factor of RA. Results: With the severity of disease, DAS28, GSUS synovitis, PDUS synovitis, GSUS tendon tenosynovitis, PDUS tendon tenosynovitis, US7 total score showed significant increase (all P<0.001). There were positive correlation of GSUS synovitis, PDUS synovitis, GSUS tendon tenosynovitis, PDUS tendon tendonitis, US7 total score to the severity of RA (all P<0.001). PDUS synovitis in US7 scoring system was an independent predictor of light-medium activity. PDUS synovitis and GSUS tendon tenosynovitis were independent predictors of medium-heavy activity (all P<0.001). Conclusion: There is correlation between the progression of US7 and RA. PDUS synovitis is an efficient indicator for identifying the severity of RA.

RESUMO

With population aging and socio?economic development, the sedentary time and behavior of the elderly are increasing, resulting in an increase in health risks as well. This review aimed to investigate the effects of sedentary behavior on the health of the elderly, factors contributing to sedentary behavior in the elderly, and sedentary behavior interventions for the elderly, as well as provide reference for sedentary behavior investigation and intervention study, to eventually achieve health promotion for the sedentary elderly.

RESUMO

Compassion fatigue is a common mental health problem experienced by rescuers. It is a phenomenon of psychological discomfort caused by the lack or excess of empathy in the implementation of relief. The main manifestation is post-traumatic stress disorder, the reduction of interest and ability of empathy, and the increase of job burnout. Oncology nurses have a higher incidence of compassion fatigue due to greater pressure caused by the complex work environment. The current interventions on the compassion fatigue of oncology nurses abroad, such as Self-Care, Compassion Fatigue Resiliency Program, Mindfulness Training,Educational Intervention, arts therapies, and others, can help improve nurse's quality of service and physical and mental health. However, there is a relative lack of domestic research, and nurse researchers should carry out such studies more actively to provide a reference for the improvement of nurses' mental health situation.

RESUMO

Objective To evaluate the value of the 7-joint ultrasound score (US7) in treatment of rheumatoid arthritis (RA) with Chinese and western medicine. Methods A total of 160 RA patients were divided into 2 groups based on different methods of treatment, including Yi Shen Qing Luo (YSQL) group and western medicine group. The patients were examined by grey scale ultrasound (GSUS) and power Doppler ultrasound (PDUS) with US7 at baseline and after 3 and 6 months later. There were 7 related joints and 5 single factors for scoring in US7 system. The joints included the wrist joints, the second/third metacarpophalangeal joints (MCP Ⅱ/Ⅲ),the second/third proximal interphalangeal joints (PIP Ⅱ / Ⅲ) and the second/fifth metatarsophalangeal joints (MTP Ⅱ/Ⅴ).And the factors included synovitis of GSUS,synovitis of PDUS,myotenositis/tenosynovitis of GSUS,myotenositis/tenosynovitis of PDUS and bone erosion (ES).Meanwhile, the laboratory index included C-reactive protein (C-RP) and erythrocyte sedimentation rate (ESR) were examined. And the clinical indexes as disease activity score in 28 joints (DAS28) were evaluated. Results The statistical differences of synovitis of GSUS,synovitis of PDUS,myotenositis/tenosynovitis of GSUS and myotenositis/tenosynovitis of PDUS scores in US7 system were found in both 2 groups at baseline,3 months and 6 months after treatment (all P<0.01).There was no statistical difference of ES before and after treatment in all cases (P>0.05).The factors of US7 were positively correlated with DAS28,C-RP and ESR in different extent.Condusion US7 is a viable tool for examining patients with RA.

RESUMO

Objective:To investigate the relationship between cervical lesion development and histone acetylation that regulates RAR-β2 expression. Methods:Immunohistochemistry and Western blot analysis were performed to detect AcH3, RAR-β2, and involu-crin expression in normal cervical tissues as well as in tissues with cervical intraepithelial neoplasia (CIN)Ⅱ-Ⅲand squamous cell cer-vical carcinoma. The relationship among histone acetylation level, RAR-β2 expression, and cervical lesion severity were analyzed. Re-sults:AcH3, RAR-β2, and involucrin expression were reduced or absent with the progression of cervical lesions;significant differences were noted between the groups (P<0.05). Histone acetylation level and RAR-β2 expression were positively correlated (r=0.797, P<0.05). AcH3 and RAR-β2 expression, which were both associated with the cervical lesions, were negatively correlated [r=-0.547(AcH3), r=-0.585(RAR-β2), P<0.05]. Conclusion:Histone acetylation modification is associated with the regulation of RAR-β2 expression. This pro-cess is also likely to participate in the carcinogenesis of cervical carcinoma.
