Objective:To explore the expressions of long non-coding RNA LINC00673 and ISG15 protein in pancreatic cancer and their clinical significances.Methods:The clinical data of 57 patients diagnosed as pancreatic ductal carcinoma (PDAC) at the Affiliated Cancer Hospital of Xiangya Medical College of Central South University from January 2014 to December 2018 were retrospectively analyzed. The relative expressions of LINC00673 in pancreatic cancer tissues and paracancerous normal tissues (within 3 cm from the edge of cancer tissues) were examined by using quantificational reverse transcription-polymerase chain reaction (qRT-PCR). The ISG15 protein expressions in pancreatic cancer tissues and paracancerous normal tissues were examined by using immunohistochemistry. The difference in LINC00673 expression between ISG15 protein positive and negative patients was compared. The correlation between LINC00673 and ISG15 protein expressions in pancreatic cancer was analyzed by Spearman rank correlation analysis. Moreover, the correlations of LINC00673 and ISG15 protein expressions with clinical stage and pathological classification of pancreatic cancer patients were analyzed.Results:The positive expression of ISG15 protein in pancreatic cancer tissues was 40.4% (23/57), which was higher than that in paracancerous normal tissues [15.8% (9/57)] ( χ2 = 7.90, P = 0.004), and the relative expression of LINC00673 in pancreatic cancer tissues was 0.99±0.36, which was lower than that in paracancerous normal tissues (1.26±0.41) ( t = 4.80, P < 0.001). For 23 (40.4%) ISG15-positive patients and 34 (59.7%) ISG15-negative patients, the relative expression of LINC00673 was 0.77±0.46 and 0.45±0.27 ( P < 0.001). Spearman analysis showed that there was a correlation between LINC00673 and ISG15 protein expressions ( ρ = -0.429, P = 0.001). The relative expression of LINC00673 decreased in patients with low differentiated or undifferentiated tumor, vascular invasion and lymph node metastasis (all P < 0.05), but there was no correlation between LINC00673 expression and patients' age, tumor site, preoperative CA199 level, and TNM stage (all P > 0.05); ISG15 protein expression increased in patients with low differentiated or undifferentiated tumor, TNM stage Ⅲ-Ⅳ, vascular invasion and lymph node metastasis (all P < 0.05), but there was no correlation between ISG15 protein expression and patients' gender, age, tumor site, and preoperative CA199 level (all P > 0.05). Conclusions:The expression of LINC00673 in pancreatic cancer is related to vascular invasion, tumor differentiation degree and lymph node metastasis, and the expression of ISG15 in pancreatic cancer is related to vascular invasion, tumor differentiation degree, lymph node metastasis and TNM stage. The combined detection of LINC00673 and ISG15 protein could be a valuable prognostic indicator for pancreatic cancer. The therapies targeting LINC00673 and ISG15 protein signaling pathways are expected to be a potential option for immunotherapy of pancreatic cancer.

Objective:To investigate the effects of silicon photomutipliers (SiPM) detector and Bayesian penalized likelihood (BPL) reconstruction algorithm on semiquantitative parameters of 18F-FDG PET/CT and diagnostic efficiency for solitary pulmonary nodules/masses compared with traditional photomultiplier tube (PMT) and ordered subsets expectation maximization (OSEM). Methods:From March 2020 to January 2022, 118 patients (76 males, 42 females, age (63.0±10.1) years) newly diagnosed with solitary pulmonary nodules/masses in First Hospital of Shanxi Medical University were prospectively enrolled and underwent 18F-FDG PET/CT imaging with two different PET/CT scanners successively. The images were divided into PMT+ OSEM, SiPM+ OSEM and SiPM+ BPL groups according to PET detector and reconstruction algorithms. The SUV max, SUV mean, metabolic tumor volume (MTV) and total lesion glycolysis (TLG) of pulmonary nodules/masses were measured, then signal-to-noise ratio (SNR) and signal-to-background ratio (SBR) were calculated. One-way analysis of variance and Kruskal-Wallis rank sum test were performed to compare differences of above parameters among groups. ROC curve analysis was used to analyze the optimal threshold of SUV max for the differential diagnosis of pulmonary nodules/masses and AUCs were obtained. Results:There were 83 malignant nodules and 35 benign nodules. The image quality of SiPM+ BPL group (4.23±0.64) was better than that of SiPM+ OSEM group (3.57±0.50) or PMT+ OSEM group (3.58±0.51; F=54.85, P<0.001). There were significant differences in SUV max (7.57(3.86, 15.61) vs 4.95(2.22, 10.48)), SUV mean (4.43(2.28, 9.12) vs 2.84(1.21, 5.71)), MTV (3.54(1.57, 7.67) vs 5.09(2.83, 11.79)), SNR (28.12(12.55, 54.38) vs 20.16(8.29, 41.45)) and SBR (4.03(1.83, 7.75) vs 2.32(0.96, 5.03)) between SiPM+ BPL and SiPM+ OSEM groups ( H values: 16.63-37.05, all P<0.001). The optimal threshold values of SUV max in SiPM+ BPL, SiPM+ OSEM and PMT+ OSEM were 3.31, 2.21, 2.05 with AUCs of 0.686, 0.689, 0.615 for nodules < 2 cm, and were 10.29, 6.49, 4.33 with AUCs of 0.775, 0.782, 0.774 for nodules/masses ≥2 cm. Conclusions:Image quality and parameters of pulmonary nodules/masses are mainly affected by the reconstruction algorithms. BPL can improve SUV max, SUV mean, SBR and SNR, but reduce MTV without significant effect on liver parameters. SiPM+ BPL has a higher diagnostic threshold of SUV max than SiPM+ OSEM and PMT+ OSEM.

Microbes are commonly sensitive to shifts in the physiological and pathological state of their hosts, including mothers and babies. From this perspective, the microbiome may be a good indicator for diseases during pregnancy and has the potential to be used for perinatal health monitoring. This is embodied in the application of microbiome from multi body sites for auxiliary diagnosis, early prediction, prolonged monitoring, and retrospective diagnosis of pregnancy and infant complications, as well as nutrition management and health products developments of mothers and babies. Here we summarized the progress in these areas and explained that the microbiome of different body sites is sensitive to different diseases and their microbial biomarkers may overlap between each other, thus we need to make a diagnosis prudently for those diseases. Based on the microbiome variances and additional anthropometric and physical data, individualized responses of mothers and neonates to meals and probiotics/prebiotics were predictable, which is of importance for precise nutrition and probiotics/prebiotics managements and developments. Although a great deal of encouraging performance was manifested in previous studies, the efficacy could be further improved by combining multi-aspect data such as multi-omics and time series analysis in the future. This review reconceptualizes maternal and infant health from a microbiome perspective, and the knowledge in it may inspire the development of new options for the prevention and treatment of adverse pregnancy outcomes and bring a leap forward in perinatal health care.

Objective@#To analyze the awereness and beliefs on sedentary behavior among college students and associated factors, so as to provide a reference for improving sedentary behavior.@*Methods@#A total of 1 261 students from six universities in Hangzhou City, were selected using a convenience sampling method. Sedentary behavior and its influencing factors were analyzed using the Chi square test and binary Logistic regression, respectively.@*Results@#The pass rate of the knowledge and belief scores were 33.2% and 55.5%, respectively. The average duration of sedentary behavior on weekdays and weedends were (6.73±3.03) and (6.22±3.41)h/d, respectively. The results of the binary Logistic regression analysis showed that the influencing factors of health related sedentary behavior included gender, grade, subject major, sports performance and belief score ( P <0.01). Female students were 2.18 times more likely to exhibit unhealthy sedentary behaviors than male students (95% CI =1.58-3.01); senior college students were less likely to exhibit unhealthy static behaviors than freshmen ( OR =0.38,95% CI =0.23-0.61); and sports majors were less likely to exhibit unhealthy sedentary behaviors than medical majors ( OR =0.10, 95% CI =0.03-0.34). Students with high sports performance were less likely to exhibit unhealthy sedentary behavior ( OR =0.63, 95% CI =0.48-0.83). In terms of belief scores, students who obtained a pass rate were less likely to exhibit unhealthy static behavior ( OR =0.67, 95% CI =0.52-0.87).@*Conclusion@#Sedentary behavior is common among college students, and is influenced by several factors influencing factors. Colleges and universities should pay attention to improving the knowledge and belief level of students’ sedentary behavior, promote healthy exercise habits, reduce the duration of static behavior, and maintain the health level of college students.

Rheumatoid arthritis (RA) is a refractory autoimmune disease that can cause symmetrical polyarticular disease. The key mechanism of its occurrence and development is the dysequilibrium of helper T cell 17 (Th17)/regulatory T cell (Treg) balance. Therefore, reconstructing Th17/Treg balance may be a new strategy for the treatment of RA. Traditional Chinese medicine has significant advantages in the treatment of RA such as integrity, multi-target, multi-link and multi-path. This paper summarizes the basic and clinical studies on the regulation of Th17/Treg balance in the treatment of RA by traditional Chinese medicine in the past five years, and finds that the active components/sites of traditional Chinese medicine such as flavonoids, alkaloids and terpenes have unique advantages in the regulation of Th17/Treg balance. The traditional Chinese medicine compound formula interferes with Th17/Treg balance by exerting the effects of dispelling wind, dehumidifying, removing blood stasis, unblocking collaterals, relieving pain, dispersing cold and strengthening health. The effect of external treatment of traditional Chinese medicine is obvious and can be used as a clinical adjuvant therapy for RA; related mechanisms of action include regulating the production of inflammatory factors, regulating the expression of transcription factors and interfering with the activation of signaling pathways. However, the existing research has the shortcomings of insufficient mechanism research, few clinical research, limited external treatment research of traditional Chinese medicine, and lack of combination therapy research, which need to be improved by follow- up research.

Governments worldwide have rapidly deployed non-pharmaceutical interventions (NPIs) to mitigate the COVID-19 pandemic. However, the effect of these individual NPI measures across space and time has yet to be sufficiently assessed, especially with the increase of policy fatigue and the urge for NPI relaxation in the vaccination era. Using the decay ratio in the suppression of COVID-19 infections, we investigated the changing performance of different NPIs across waves from global and regional levels (in 133 countries) to national and subnational (in the United States of America [USA]) scales before the implementation of mass vaccination. The synergistic effectiveness of all NPIs for reducing COVID-19 infections declined along waves, from 95.4% in the first wave to 56.0% in the third wave recently at the global level and similarly from 83.3% to 58.7% at the USA national level, while it had fluctuating performance across waves on regional and subnational scales. Regardless of geographical scale, gathering restrictions and facial coverings played significant roles in epidemic mitigation before the vaccine rollout. Our findings have important implications for continued tailoring and implementation of NPI strategies, together with vaccination, to mitigate future COVID-19 waves, caused by new variants, and other emerging respiratory infectious diseases.

Objective:To explore the diagnosis, surgical methods and therapeutic effect of primary duodenal malignant tumor.Methods:The clinical data of 116 patients with primary duodenal malignant tumor from January 2010 to December 2018 were retrospectively analyzed.Results:Among 116 patients, adenocarcinoma was in 74 cases, interstitial tumor was in 25 cases, carcinoid was in 9 cases, the others was in 8 cases. Before operation, duodenoscopy was performed in 107 cases, and CT examination was performed in 76 cases. There were 57 cases of pancreaticoduodenectomy, 15 cases of duodenal segmental resection, 13 cases of subtotal gastrectomy and duodenal bulbar resection, 13 cases of duodenal partial resection, and 18 cases of palliative short circuit operation. The total incidence of postoperative complication was 31.9% (37/116), including pancreatic fistula in 8 cases (grade B 5 cases, grade C 3 cases), biliary fistula in 6 cases, abdominal infection in 5 cases, pulmonary infection in 4 cases, intestinal fistula in 3 cases, delayed gastric emptying in 3 cases, and hemorrhage in 8 cases. Four cases (3.4%) died during the perioperative period. Single factor Cox regression analysis result showed that the postoperative survival time was related to the tumor differentiation degree, operation method, tumor infiltration degree and lymphatic metastasis ( P<0.05 or <0.01); multi-factor Cox regression analysis results showed that the operation method, tumor infiltration degree and lymphatic metastasis were the independent risk factors for the postoperative survival time of patients with primary duodenal malignant tumor ( P<0.05). The patients were followed up until June 2021, and 9 cases were lost to follow-up. Kaplan-Meier survival curve analysis result showed that the postoperative overall 1-,3- and 5-year survival rates were 82.11%, 57.56% and 33.11%, respectively. Conclusions:Adenocarcinoma is the main primary malignant tumor of duodenum. Duodenoscopy and CT are the main examination methods. Radical resection is the most effective treatment for primary duodenal malignant tumor, and pancreaticoduodenectomy is the first choice. Surgical method, tumor infiltration degree and lymphatic metastasis are the independent risk factors affecting the prognosis of patients.

Objective:To evaluate the influence of psychological nursing on the functional recovery of patients with dysphagia after stroke using the method of Meta analysis.Methods:A computer search of PubMed, Cochrane Library, Web of Science, VIP, Wanfang Data, China Knowledge Infrastructure Engineering and China Biomedical Literature Database was used to collect the psychological nursing on the function of patients with dysphagia after stroke. The data were collected from June 1956 to August 2020. The literature of randomized controlled studies using Revman 5.3 for Meta analysis.Results:A total of 7 controlled studies on the impact of the use of the core quality of life scale (QLQ-C30) and the treatment efficacy survey on the functional recovery of patients with dysphagia after stroke and the impact of quality of life were included, with a total of 660 samples. The results of Meta analysis showed that the average difference of mental function after psychological nursing intervention was statistically significant [weight mean difference( WMD)=-16.66, 95% CI -18.09--15.22, P<0.001]; in the physical function score, the intervention group performed relatively significant difference compared with the control group ( WMD=-14.10, 95% CI -16.26--11.95, P<0.001); the average difference in social function after psychological nursing intervention was statistically significant ( WMD=-13.52, 95% CI -15.64--11.41, P<0.001); in the quality of life score, the control group performed worse than the intervention group ( WMD=-14.58, 95% CI -15.86--13.31, P<0.001); the treatment effect of the intervention group was better than that of the control group, and the difference was statistically significant [odds ratio ( OR=0.16, 95% CI 0.08-0.32, P<0.001]. Conclusions:Through the intervention of psychological care, the swallowing disorder of stroke patients is relieved and the quality of life is improved.

Modern transportation plays a key role in the long-distance and rapid spread of SARS-CoV-2. However, little is known about the transmission risk of SARS-CoV-2 on confined vehicles, such as airplanes and trains. Based on the itinerary and epidemiological data of COVID-19 cases and close contacts among 9,265 airline passengers on 291 airplanes and 29,335 passengers on 830 high-speed trains in China from December 20, 2019 to March 17, 2020, we estimated that the upper bound of overall attack rate of COVID-19 among passengers was 0.60% (95% confidence interval: 0.43%-0.84%) for airplanes and 0.35% (0.28%-0.44%) for trains departing from Wuhan before its lockdown, respectively. The reproduction number during travel ranged from 0.12 to 0.19 on airplanes and from 0.07 to 0.12 on trains, with the risk varying by seat distance from the index case and joint travel time, but the difference in risk was not significant between the types of aircraft and train. Overall, the risk of SARS-CoV-2 transmission on planes and high-speed trains with high efficiency air filtration devices was relatively low. Our findings improve understanding of COVID-19 spread during travel and may inform response efforts, such as lifting travel restrictions, and resuming transportation in the pandemic.

Objective:To observe the clinical effect of Bifidobacterium combined with compound pepsin powder in the treatment of secondary diarrhea following bronchopneumonia in infants, so as to explore the best clinical treatment of the disease and promote the early recovery of children.Methods:From January 2018 to December 2018, 100 infants with secondary diarrhea following bronchopneumonia who admitted to Department of Pediatrics of the Fourth People's Hospital of Wenling were divided into group A( n=50) and group B( n=50) according to random number table method.Bifidobacterium treatment and Bifidobacterium combined with compound pepsin powder were given, respectively.The clinical efficacy, disappearance time of diarrhea and hospitalization time, levels of inflammatory factors[interleukin-6 (IL-6), tumor necrosis factorα (TNF- α)] and serological indicators[gastrin(GAS), plasma motilin(MOT), somatostatin(SS), diamine oxidase(DAO)] were compared between the two groups. Results:The total effective rate of group B(96.0%) was higher than that of group A(82.0%)(χ 2=4.184, P<0.05). The disappearance time of diarrhea and hospitalization time in group B[(2.5±1.0)d, (3.6±0.6)d] were shorter than those in group A[(4.1±1.3)d, (5.7±0.8)d]( t=3.762, 2.541, all P<0.05). The levels of IL-6 and TNF-in the group B[(10.5±2.1)ng/L, (12.6±3.5)ng/L] were lower than those in group A[(16.6±2.5)ng/L, (21.2±4.4)ng/L]( t=5.129, 6.118, all P<0.05). After treatment, the levels of GAS, MOT, SS, DAO in group B[(86.8±8.4)ng/L, (386.1±32.2)ng/L, (51.5±7.2)ng/L, (1.0±0.3)U/mL] were better than those in group A[(115.4±10.1)ng/L, (502.8±41.6)ng/L, (37.3±5.8)ng/L, (1.4±0.5)U/mL]( t=6.310, 21.576, 10.964, 5.683, all P<0.05). Conclusion:Bifidobacterium combined with compound pepsinase powder is effective in the treatment of infantile secondary diarrhea following bronchopneumonia.It can significantly shorten the disappearance time of diarrhea and hospitalization time, reduce the levels of inflammatory factors and improve serological indicators.It is worthy of implementation.

BACKGROUND: Studies have shown that rhubarb-free anthraquinone can protect the stability of the intestinal mucosa cell structure and function. However, the combination of rhubarb-free anthraquinone with adipose-derived stem cells to treat acute pancreatitis in rats has not yet been reported. OBJECTIVE: To investigate the effects of rhubarb-free anthraquinone on the treatment of acute pancreatitis by fat stem cell transplantation in rats. METHODS: Cryopreserved mouse adipose-derived stem cells (provided by the Chinese Academy of Medical Sciences) were rapidly recovered and cultured in vitro. The survival cells were labeled using PKH-26, and then the cell survival rate of adipose-derived stem cells was determined by MTT colorimetry. Sprague-Dawley rats provided by Beijing Vital River Laboratory Animal Technology Co., Ltd. were equally randomized into four groups. Animal models of acute pancreatitis were made in all rats through intestinal wall puncture for retrograde administration of 5% sodium taurocholate to the pancreaticobiliary tube, and then the model rats were given tail vein injection of L-DMEM complete medium (0.5 mL) in the model group, intragastric administration of 200 mg/kg rhubarb-free anthraquinone in the rhubarb-free anthraquinone group, tail vein injection of adipose-derived stem cells (1×107 cells/L, 0.5 mL) in the cell transplantation group, and intragastric administration of 200 mg/kg rhubarb-free anthraquinone plus tail vein injection of 1×107 adipose-derived stem cells (0.5 mL) in the combined treatment group. All interventions in each group were performed once a day, for 3 continuous days. RESULTS AND CONCLUSION: (1) The levels of serum amylase and interleukin-6 were significantly reduced in the rhubarb-free anthraquinone group and cell transplantation group compared with the model group (P < 0.05) , while the level of trypsinogen activation peptide significantly increased (P < 0.05). In the combined treatment group, the levels of three indicators mentioned above were significantly decreased compared with the rhubarb-free anthraquinone group and cell transplantation group (P < 0.05). (2) The results of hematoxylin-eosin staining showed that the pathological changes in the pancreatic tissue of rats were significantly reduced in the rhubarb-free anthraquinone group and cell transplantation group, such as fatty degeneration, hemorrhage, cell necrosis and inflammatory cell infiltration. The remission of pathological changes was more obvious in the combined treatment group. (3) The number of positive cells labeled by PKH-26 was the highest in the combined treatment group, followed by the cell transplantation group, while there were no PKH-26-positive cells in the rhubarb-free anthraquinone group and model group (P < 0.05). (4) Compared with the model group, the number of apoptotic cells in the pancreatic tissue was significantly reduced in the rhubarb-free anthraquinone group and cell transplantation group, and lowest in the combined treatment group (P < 0.01). (5) The expression of transforming growth factor-β1 and Smad2/3 at gene and protein levels was highest in the model group, followed by the rhubarb-free anthraquinone group and cell transplantation group (P < 0.05) , and lowest in the combined treatment group (P < 0.01). To conclude, the combined intervention of rhubarb-free anthraquinone and adipose-derived stem cell transplantation could effectively improve the blood biochemical index level in the rats with acute pancreatitis, significantly relieve the degree of inflammatory response, pancreatic tissue morphology, and apoptosis of pancreatic cells, which may be related to the reduction of the transforming growth factor-β/Smad signal pathway.

(3) The number of positive cells labeled by PKH-26 was the highest in the combined treatment group, followed by the cell transplantation group, while there were no PKH-26-positive cells in the rhubarb-free anthraquinone group and model group (P < 0.05). (4) Compared with the model group, the number of apoptotic cells in the pancreatic tissue was significantly reduced in the rhubarb-free anthraquinone group and cell transplantation group, and lowest in the combined treatment group (P < 0.01). (5) The expression of transforming growth factor-β1 and Smad2/3 at gene and protein levels was highest in the model group, followed by the rhubarb-free anthraquinone group and cell transplantation group (P < 0.05), and lowest in the combined treatment group (P < 0.01). To conclude, the combined intervention of rhubarb-free anthraquinone and adipose-derived stem cell transplantation could effectively improve the blood biochemical index level in the rats with acute pancreatitis, significantly relieve the degree of inflammatory response, pancreatic tissue morphology, and apoptosis of pancreatic cells, which may be related to the reduction of the transforming growth factor-β/Smad signal pathway.

The clinical and urodynamic data of 37 patients with benign prostate hyperplasia (BPH) and 30 diabetic patients complicated with BPH (BPH+DM) admitted between Jan 2014 and July 2017 were analyzed retrospectively. The International Prostate Symptom Score (IPSS), maximal flow rate (Qmax), post-voiding residual urine volume (PVR), maximum cystometric capacity (MCC), first desire to void (FDV), pressure of detrusor maximum (Pdet, max), bladder outlet obstruction index (BOOI), bladder contraction index (BCI) were compared between BPH group and BPH+DM group. According to BOOI-BCI linear regression, 22 cases (group A) and 15 cases (group B) of BPH patients were above and below the linear curve; while there were 14 cases (group C) and 16 cases(group D)of BPH+DM patients above and below the curve, respectively. The mean±SD FDV, MCC, Pdet, max, PVR, BOOI, BCI were (172.7±93.0)ml vs. (300.5±118.4)ml (P<0.05), (311.9±147.1)ml vs. (509.3±98.6)ml (P<0.05), (84.7±51.5)cmH2O(1 cm H2O=0.098 kPa) vs. (49.7± 32.9)cmH2O vs (P<0.05), 10.0 ml(0—200 ml) vs. 41.5 ml(0—450 ml), 69.7 ± 53.7 vs. 35.9 ± 32.3 (P<0.05), 122.3±50.2 vs 84.2±43.3 (P<0.05) in BPH and BPH+DM groups, respectively. In BPH group and BPH+DM group, the regression coefficients of BOOI-BCI were 0.889 and 0.724, respectively. In group A and group B, the difference value of IPSS and Qmax pre and post operation were 7.6±3.5 and 7.3±4.1 (P>0.05), (2.6±1.1)ml/s and (3.7±1.3) ml/s (P<0.05), respectively. In group C and group D, the difference value of IPSS and Qmax pre and post operation were 5.3 ± 2.4 and 6.0 ± 3.3 (P>0.05), (2.4 ± 1.0)ml/s and (3.8 ± 1.4)ml/s (P<0.05), respectively. The study indicates that the therapeutic effect is better for the patients blow BOOI-BCI regression linear curve compared to the patients above the linear curve.

Objectives:To investigate the prevalence of depressive emotion and related factors among rural elderly living in urban-rural fringe zones and traditional villages.Methods:Totally 1135 rural elderly were surveyed in Fuding,Fujian Province.The 10-item CES-D Scale was adopted to assess depressive emotion (score 10 or higher as having depression problem),and a self-made questionnaire was used to investigate other relevant demographical information.Results:Rural elderly lived in traditional villages had significantly higher ratio of depression than the elderly lived in urban-rural fringe zones (49.6% vs.30.3%,P <0.001).Logistic regression analysis showed that the ratio of depression in elderly of traditional villages was 1.40 times that of urban-rural fringe zone after control ling other factors.Other important factors (P < 0.001) related to depression were poorer self-rated health (OR =7.52),financial strain (OR =4.41),negative life event (OR =2.91),and living alone (OR =2.72) in elderly of urban-rural fringe zone,but only financial strain (OR =8.52) in elderly of traditional villages.Conclusion:Depression is more prevalently identified among rural elderly living in traditional villages,and urbanization could effectively reduce depression in rural elderly.Public policies should improve the financial and medical security,social support and so on to promote the emotional well-being among rural elderly during urbanization.

Objective To evaluate the safety and feasibility of reconstruction technique of atrial septum puncture trajectory with the help of three - dimensional mapping system in performing radiofrequency catheter ablation for atrial fibrillation. Methods Sixty- eight consecutive patients with atrial fibrillation received two times of atrial septum puncture under fluoroscopic guidance to perform radiofrequency catheter ablation. Carto 3, a three - dimensional mapping system, was employed to construct the real time left atrium and pulmonary vein anatomy by using a rapid anatomical mapping (FAM) model. Then, FAM model was used to construct the trajectory, along which the ablation catheter passed from left atrium through the long sheath to the right atrium and finally into the inferior vena cava. The safety and the feasibility of this catheter trajectory, which could allow the catheter repeatedly enter the left atrium, were evaluated. Results By using 3D-reconstruction technique of atrial septum puncture trajectory, the ablation catheter could repeatedly enter the left atrium at right anterior oblique position as well as at left anterior oblique position under zero X-ray fluoroscopy. The average time spent for the procedure was (12. 18±2. 28) seconds. No any complication occurred. Conclusion The reconstruction technique of atrial septum puncture trajectory with the help of three-dimensional mapping system is simple and feasible, the ablation catheter can repeatedly enter the left atrium, the X-ray exposure time spent for catheter ablation of atrial fibrillation can be greatly reduced. RESUMO

Objective To discuss the clinical effect of pulmicort respules inhalation in children with acute infectious laryngitis.Methods We selected acute infectious laryngitis in our hospital from November 2015 to November 2016,106 children were divided into two groups,the control group and the observation group of 53 cases,of which the control group of children with conventional treatment,the observation group to take Pulmicort respules inhalation treatment,the two comparison(P<0.05).The total effective rate of the observation group and the control group were 100%and 90.56%respectively(P<0.05).Results The treatment effect of the observation group and the control group were observed and compared(P<0.05),and the improvement of the observation group after treatment and the time of symptom extinction were significantly better than that of the control group(P<0.05); The clinical symptoms of both groups before and after treatment were improved(P<0.05).Conclusion The treatment of children with acute infectious laryngitis by atomizing inhalation of pulmicort respules can obtain obvious therapeutic effect and reduce the adverse reaction caused by improper use of drugs,(fever,cough,wheezing,etc.)have a good improvement in clinical application worthy of recognition.

Objective:To discuss the diagnostic approaches and treatment choices of low-flow priapism.Methods:35 cases of patients suffered from the low-flow priapism in our hospital from September 2010 to October 2016 were selected and diagnosed with the low-flow (ischemic) priapism by combining cavernous blood gas analysisand and color duplex ultrasonography.The priapism lasted 12 to 240 h with a mean of 72 h.31 patients of them had ever been induced by Polysaccharide Sulfate.One appeared priapism after sexual life.One appeared priapism after micturition.2 of them were not known what drug they had taken.Results:The symptoms disappeared in 5 cases as a result of using cold compress,sedation method and intracavemous lavage in hospital.But 30 cases were still priapism.Their penis were in a flaccid state after they were performed with the operation of glandular cavemosum shunting.During the 6-24 months of follow-up,31 patients developed erectile dysfunction.Among them,13 cases were light to mid erectile dysfunction,10 cases were mildly and 8 cases were the worst.Conclusion:(1) Using cavernous blood gas analysis and color duplex ultrasonography is important way to diagnose priapism.(2) Cold compress,sedation method and intracavemous lavage are the first treatments for the the low-flow priapism.If they are not the effectual cure,the operation of glandular cavemosum shunting should be performed in time.

Objective To explore the clinical curative effects of hip replacement in the treatment of femoral neck fracture and femoral head necrosis in elderly patients and the existing problems.Methods A total of seventy-five aged patients with femoral neck fracture or femoral head necrosis who were treated by hip replacement were selected as study objects.They were divided into fracture group and femoral head necrosis group according to disease type,all the patients were treated with hip replacement.The surgical time,intraoperative blood transfusion volume,length of hospital stay,in-bed time,perioperative complications and postoperative complications were observed and recorded.The Harris score was used to evaluate hip function of preoperation and postoperation.Results All patients were followed up for 6 ~ 24 months,no incision infection and severe perioperative complications.The volume of intraoperative blood loss,operation time,hospitalization time,in-bed time of the fracture group were (334.9 ± 90.4) mL,(91.7 ± 14.3) min,(19.5 ± 7.5) d,(5.9 ± 1.6) d,respectvely,which of the femoral head necrosis group were (362.9 ± 71.5) mL,(93.5 ± 7.8) min,(20.5 ± 4.2) d,(6.3 ± 1.4) d,respectvely,the differences between the two groups were not statistically significant (all P > 0.05).At the end of the follow-up,the Harris scores of the two groups were higher than before operation,and the score of the fracture group (89.0 ± 7.4) points was significantly higher than (84.0 ± 9.2) points of the femoral head necrosis group (t =19.639,P < 0.05).In the fracture group,1 year after surgery,1 patient appeared delayed fracture healing,2 cases with postoperative pain when walking the affected area.In the femoral head necrosis group,1 case with femoral nerve injury,1 case of death due to its original diseases,1 case of postoperative sore pain when walking,the two groups had no prosthesis loosening or dislocation phenomenon.Conclusion Hip replacement in the treatment of femoral neck fracture and femoral head necrosis in elderly patients can be used as a clinical treatment option with short in-bed time,less intraoperative blood loss,short operation time,less perioperative complication and good treatment effect,and it is worth to be promoted.

OBJECTIVE:To establish a method for the decoction of isoflavone content and its acid hydrolysis conversion rate in the flower of Pueraria lobata. METHODS:Using the flowers of Pueraria lobata as raw material,the isoflavone with main com-ponent of tectoridin in the flower of P. lobata was prepared with ethanol,ethyl acetate extracted,ethanol recrystallized and puri-fied,and it was converted to tectorigenin with hydrolysis in hydrochloric acid. By screening the solvent and wavelength,UV spec-trophotometry was established to determine tectovidin and tectorigenin,and calculate the isoflavone content and acid hydrolysis con-version rate of tectoridin(expressed by the relative percentage of tectorigenin). It was compared with HPLC detection results,the accuracy of UV method was evaluated. RESULTS:The solvent was 70%ethanol solution containing 1%triethylamine,and the iso-flavone content was detected at wavelength of 339,274 nm. The linear range of tectoridin was 8.80-29.33 nmol/mL(r=0.9999). RSDs of precision(n=6),stability(n=5)and reproducibility(n=6)tests were lower than 1.94%;average recovery was 99.7%(RSD=1.77%,n=9). There were no statistical significances in the contents of total flavonoids (UV:17.64-25.55 nmol/mL vs. HPLC:17.39-24.40 nmol/mL) and the relative percentage of tectorigenin (UV:57.65%-87.59% vs. HPLC:55.62%-91.14%). CONCLUSIONS:The established method is accurate,reliable,and can be used for the rapid determination of acid hydrolysis con-version rate of tectoridin.

OBJECTIVE@#To investigate the significance of tumor necrosis factor-alpha (TNF-α) and plasma endothelium (ET) in pathophysiologic process of patients with obstructive sleep apnea hypopnea syndrome(OSAHS) with type 2 diabetes mellitus (T2DM).@*METHOD@#All observed subjects were divided into 4 groups. A number of 80 patients with OSAHS, 65 cases of OSAHS with T2DM patients, 20 patients with T2DM, and 32 cases of healthy control group were observed in this study. The serum levels of TNF-α and ET were detected by double antibody sandwich ELISA, the content of TNF-α and ET were compared between OSAHS group and OSAHS + T2DM group. It were also compared before and after treatment of CPAP or surgery.@*RESULT@#TNF-α level is higher in OSAHS+T2DM group than that in the OSAHS group, T2DM group and the control group (P 0.05).@*CONCLUSION@#TNF-α may be involved in the development of OSAHS and T2DM, while ET may have little effect on the occurrence and development of OSAHS and T2DM.
