RESUMO

Objective To investigate the relationship of obesity and vascular endothelial growth factor (VEGF)and the clinicopathological characteristics and prognosis of breast cancer. Methods From Jan.2013 to Dec.2015,147 breast cancer patients were treated at the Department of Breast Surgery in Jiangsu University Affiliated People's Hospital. The clinical factors such as BMI,clinicopathological data and survival prognosis were collected. The chi-square test was used to analyze the association between factors and obesity.Log-rank test and COX regression analysis were performed to make univariate and multivariate analysis. The Oncomine and Kaplan Meier plotter were employed to assess the expression of VEGF in breast cancer and its association with survival prognosis of breast cancer patients. Results The median follow-up time was 40 (ranging from 12 to 58) months.Obesity was associated with tumor size(x2=11.009,P=0.001),and there was no significant correlation between axillary lymph node status, hormone receptor and Her-2 expression. The results of single factor analysis suggested that obesity and axillary lymph node status significantly affected the breast cancer patients OS (P=0.035,P=0.038)and RFS (P=0.035,P=0.036)).Cox multivariate regression analysis results suggested that obesity could be considered as an independent prognostic factor of breast cancer:P=0.051,HR=3.849 (95% CI:0.995-14.895);Oncomine database analysis found that VEGF was relatively high in breast cancer tissue compared to that in adjacent tissues.Kaplan Meier plotter analysis results showed better overall survival (OS)and recurrence free survival (RFS)in patients with low expression of VEGF in breast cancer tissues. Conclusions Obesity is an adverse prognostic factor in breast cancer patients, and may be associated with elevated VEGF.Weight control may improve the prognosis of breast cancer patients.

RESUMO

Objective To investigate the effects of Xuebijing combined with probiotics on levels of inflammatory indexes of procalcitonin (PCT), C-reactive protein (CRP) and microcirculation index D-Dimer in patients with severe acute pancreatitis (SAP). Methods Ninety-two SAP patients admitted to the Affiliated Wujiang Hospital of Nantong University from June 2014 to July 2017 were enrolled, according to the difference in treatment, they were divided into three groups: Xuebijing group (n = 31) using 50 mL Xuebijing injection added into 100 mL 0.9% normal saline, intravenously dripped, each time lasting for 30-40 minutes, twice a day; probiotics group (n = 30) applying lactobacillus complex capsules (each capsule weight 0.33 g), 2 capsules once, 3 times a day; Xuebijing combined with probiotics group (n = 31) using the same as above two methods in combination. The routine therapies were given to the patients in the three groups, including anti-acid agents, anti-infection, anti-secretion of pancreas, fluid resuscitation, etc. The venous blood was collected before and after treatment for 5 days and 7 days, the serum PCT, CRP and plasma D-Dimer were detected and the differences in the above indexes among the three groups were compared. Results The serum PCT, CRP and plasma D-Dimer levels were significantly reduced in the three groups after treatment, and with the prolongation of therapeutic time, the reduction in the above index levels was more obvious, the amplitudes of descent in levels of indicators were mostly pronounced in Xuebijing combined with probiotics group on the 7th day of treatment, being significantly greater than those of single Xuebijing group or single probiotics group [PCT (μg/L): 1.21±1.05 vs. 2.37±0.84, 2.43±0.69, CRP (mg/L): 12.57±1.39 vs. 25.24±3.06, 27.32±2.74, D-Dimer (mg/L): 0.61±0.17 vs. 1.02±0.34, 0.94±0.39], however , the levels of PCT, CRP and D-Dimer were of no significant differences between Xuebijing group and probiotics group (all P > 0.05). Conclusions The Xuebijing injection combined with probiotics for treatment of patients with SAP can significantly reduce the levels of PCT, CRP and D-Dimer, ameliorate the inflammatory response, and the therapeutic effect was more evident compared with Xuebijing or probiotics treatment alone. Moreover, this combination therapy can provide early protection for SAP patients.

RESUMO

OBJECTIVETo explore whether docetaxel-resistant cells (MCF-7/Doc) and doxorubicin-resistant cells (MCF-7/ADM) can secrete Exosomes and their potential role in cell-cell drug-resistance transfer.

METHODSExosomes were extracted from the cell culture supernatants of MCF-7/Doc and MCF-7/ADM cells by fractionation ultracentrifugation, and were identified by transmission electron microscopy and Western blot analysis. GFP-MCF-7/S, a breast cancer parental sensitive cell line stably expressing green fluorescent protein (GFP), was constructed by recombinant lentiviral vector with GFP. Then the resistance experiment of cells and the experiment of resistance transfer by exosomes were designed to observe the phenomenon of cell-to-cell drug-resistance transfer.

RESULTSSimilar to the breast cancer parental sensitive cells (MCF-7/S), the breast cancer resistant sublines could secrete exosomes, which exhibited round or elliptic shape ranging from 30 to 100 nm in diameter with intact membrane, and only expressed the protein marker of exosomes, Tsg101, did not express the endoplasmic reticulum marker calnexin. After MCF-7/S, MCF-7/DOC and MCF-7/ADM cells we cocultured with GFP-MCF-7/S cells for 72 h, there were no significant differences in the expression of fluorescence-labeled cells among the four groups. When treated by the drug ADM or DOC for 24 hours, the MCF-7/DOC+GFP-MCF-7/S group was in favor of a significant higher survival rate of fluorescence-labeled cells compared with the MCF-7/S+GFP-MCF-7/S group (65.5% vs. 25.5%, P < 0.001), and so did the MCF-7/ADM+GFP-MCF-7/S group (53.6% vs. 25.4%, P < 0.001). The exosomes extracted from MCF-7/S, MCF-7/DOC and MCF-7/ADM cells were cultured with the GFP-MCF-7/S cells for 48 h. Among these groups, no significant differences in the expression of fluorescence-labeled cells were found. After treated by the drug ADM or DOC for 24 hours, the exosomes extracted from MCF-7/DOC+GFP-MCF-7/S group was associated with a significant higher survival rate of fluorescence-labeled cells compared with the exosomes extracted from MCF-7/S+GFP-MCF-7/S group (59.9% vs. 32.4%, P < 0.001), and so did the exosomes extracted from the MCF-7/ADM)+GFP-MCF-7/S group (58.3% vs. 27.2%, P < 0.001).

CONCLUSIONOur results suggest that drug-resistance can be transferred between breast cancer cells, and exosomes are probably the transporter of the drug resistance.

RESUMO

Objective To investigate the compliance with endocrine therapy(ET) and assess the factors associated with treatment accuracy for breast cancer. Methods 379 patients with hormone receptor-positive breast cancer undergoing complete treatment from Jun.2006 to Jun.2008 in Jiangsu Cancer Hospital were followed up.Factors related to compliance were analyzed. Results Among the 322 (85.0%) patients successfully interviewed,15 (4.7%) patients did not receive ET,43 (13.4%) patients stopped taking drugs after discharge,14 (4.3%)patients had intermittent ET,and the rest 250 patients obeyed 5-year oral ET regularly. The treatment accuracy was 77.6% (250/332).We found that majority of withdraw occurred within 2 months and within 2 years, accounting for 39.5% and 48.8%,respectively.Job and education status were relevant to these patients. Moreover, too much concern of adverse drug effects and difficulty of long-term medication were the main reasons to noncompliance. Conclusions ET is effective in hormone receptor-positive breast cancer patients and has been used as a conventional therapy. However, due to the long therapeutic period and lack of medical supervision after discharge, its treatment accuracy and compliances are becoming low, resulting in decreased efficiency. It is therefore necessary to investigate such patients' management to improve the compliance and treatment accuracy of ET.

RESUMO

OBJECTIVE@#To study the effect of miR-342-3p on the chemotherapy sensitivity in triple-negative breast cancer (TNBC).@*METHODS@#Tissue samples were collected from January 2011 to August 2013 samples in Jiangsu Cancer Hospital from a total of 32 triple-negative breast cancer patients with preoperative chemotherapy, with 5 cases of complete response (CR) and 27 cases of partial response (PR). We detected miR-342-3p expression of TNBC with RT-PCR. We transfected has-miR-342-3p mimic and inhibitor into breast cancer cell lines MDA-MB-231 by lipofection transfection and set up negative control mim-NC and inhi-NC. Group of mimic, mim-NC, inhibitor and inhi-NC were cultivated with 2 μmol /L paclitaxel, cisplatin or 4 μmol/L doxorubicin for 48 h. The cell growth rates were measured by CCK8 reagent kit, and the cell apoptosis rate by flow cytometry.@*RESULTS@#The expressions of miRNA-342-3p in TNBC tissue of CR were higher than those of PR. The cell growth rates of mimic were lower and cell apoptosis rates were higher than those of min- NC after cultivating with paclitaxel or cisplatin for 48 h, with significant difference (P0.05).@*CONCLUSION@#TNBC with high expression of miR-342-3p are more sensitive to chemotherapy. miRNA-342-3p may regulate the sensitivity of breast cancer cell line MDA-MB-231 to chemotherapy drugs paclitaxel and cisplatin, but can not affect the chemotherapy sensitivity of doxorubicin.

RESUMO

OBJECTIVETo explore the influence of miR-34a on Notch1 expression in breast cancer cells, and to explore the role of miR-34a in the sensitivity of breast cancer cells to Adriamycin (ADR).

METHODSThe expression of miR-34a in MCF-7 and MCF-7/ADR cells were detected by real-time fluorescence quantitative PCR. Cells were transfected with miR-34a mimics or inhibitor using Lipofectamine 2000. Expression of Notch1 in the two groups before and after transfection was detected by real-time fluorescence quantitative PCR and Western blot. MTT test and apoptosis assay were performed to observe the impact of miR-34a expression on the sensitivity of breast cancer cells to ADR. Statistical analysis of the results was conducted using SPSS 16.0 software. The experiment was repeated independently for at least three times, and had a good reproducibility. The data were shown as mean ± standard deviation. The analysis of variance between groups used one-way ANOVA test. Correlation between groups was analyzed using Spearman rank test. P < 0.05 was considered statistically significant.

RESULTSCompared with the control MCF-7 cells, the relative expression level of miR-34a in MCF-7/ADR cells was significantly decreased to 0.02 (P < 0.05). In the MCF-7 ADR-resistant cells, Notch1 mRNA expression was increased compared with that in the MCF-7 ADR-sensitive cells (2.10 ± 0.20-fold, P < 0.05). The expression levels of Notch1 mRNA and protein in MCF-7/ADR cells transfected with miR-34a mimics were significantly up-regulated. On the contrary, the expressions of Notch1 mRNA and protein in MCF-7 cells transfected with miR-34a inhibitor were down-regulated. The IC50 value of ADR in the control MCF-7/ADR cells and cells transfected with miR-34a mimics was (116.33 ± 16.80) and (39.28 ± 4.12) µmol/L, respectively (P < 0.05). The IC50 value of ADR in control MCF-7 cells and cells transfected with miR-34a inhibitor was (0.29 ± 0.21) and (0.51 ± 0.03) µmol/L, respectively (P < 0.05). Further, overexpression of miR-34a significantly enhanced apoptosis in the ADR-treated MCF-7/ADR cells, and miR-34a inhibitor reduced apoptosis in the ADR-treated MCF-7 cells.

CONCLUSIONSmiR-34a negatively regulates the expression of Notch1 at both mRNA and protein levels. Overexpression of miR-34a may sensitize MCF-7/ADR cells to adriamycin.

RESUMO

A new target in cancer treatment involves tumor-associated macrophages (TAMs) which are infiltrated in microenvi-ronment. By secreting a wide range of cancer-promoting cytokines, such as vascular endothelial growth factor, interleukins, and matrix metalloproteinase, TAMs can promote the proliferation, invasion, and metastasis of cancer cells. Cyclooxygenase-2 (COX-2), an inflam-matory factor that is significant for angiogenesis and immunoregulation within the local microenvironment, may also contribute to can-cer progression and act as a novel target for breast cancer therapy. Several COX-2 inhibitors, such as celecoxib, have shown potential as suppressors of TAMs and the microenvironment. Hence, the current study discusses the crosstalk between TAMs-delivered important cytokines and COX-2 in the breast cancer microenvironment, and then analyzes the value of homologous antagonists alone or in combi-nation with COX-2 inhibitors. This paper aims to provide the theoretical principle of multi-target selection in TAMs blockage, and offer a new direction for biological targeted therapy in breast cancer treatment.

RESUMO

The overexpression of P-glycoprotein(P-gp)and its coding gene mdrl is regarded as the classic mechanism of muhidrug resistance(MDR).Recent studies find that pregnane X receptor(PXR)can mediate the expression of P-gp.It is confirmed that PXR can also inhibit apoptosis of tumor cells. Therefore, preventing the activation of PXR specifically may be a new method to prevent MDR.

RESUMO

Objective To summarize our experience on diagnosis and treatment of bilateral primary breast cancer (BPBC). Methods Clinicopathologic records, including clinical manifestations, diagnosis and immunohistochemical expressions of 16 patients with BPBC, treated from 2001 to 2009 in Jiangsu Provincial Cancer Hospital, were analyzed retrospectively. Results The 16 patients with BPBC accounted for 0.47% of all patients diagnosed as malignant tumors of breast during the same period in our hospital. All of the 16 patients were women, with the median age of 53 years (ranged 41-69 years). 7 patients were in bilateral synchronous breast cancer (BSBC), with the median age 47 years (ranged 41-54 years), among whom 5 patients got BSBC just before menopause. 9 patients were in bilateral asynchronous breast cancer (BABC), with the median age 58 years (ranged 43-69 years), among whom 1 patient got BABC before menopause. The median interval between the first and second breast carcinoma of BABC was 52 months (ranged 14-196 months). Conclusions Compared with BABC, the occurrence age of the patients with BSBC was smaller. BABC was more common in premenopausal women. The interval time of BABC was irregular. The active follow-up after the occurrence of contralateral breast cancer and endocrine therapy for estrogen receptor-positive patients were recommended.
