RESUMO

Mining is responsible for the release of metallic pollutants and radioactive materials into the environment, which have the potential to disrupt ecosystems and pose significant risks to human health. Significant mining activity is concentrated in the municipality of Caetité (northeastern Brazil), where Latin America's only active uranium mine and significant iron ore deposits are located. Although previous studies have shown that the regional soil and water resources are highly contaminated by various toxic elements and that exposure to these elements is known to have adverse effects on human health, the health risks in this mining region have never been assessed. The aim of this unprecedented comprehensive investigation was to assess the health, radiological and ecological risks in this mining region, which is home to nearly 100,000 people. To achieve our goal, soil and water samples were collected in the vicinity of the mines and in the main settlements in the region. Fifteen metallic toxic elements were determined using Instrumental Neutron Activation Analysis and Inductively Coupled Plasma Optical Emission Spectrometry. The HERisk code, which follows the main methodological guidelines for risk assessment, was used to quantify human health, radiological and ecological indices. The average values of the total risk and cancer risk indices indicated that region falls into the moderate risk category (1.0 ≤ HItot < 4.0). However, 63% of the sites had high risk values, with Fe, Co and As being the metals contributing most to total and cancer risk, respectively. Near the mining areas, the potential ecological risk can be considered extreme (PERI ≥ 600). The values of the calculated radiological indices correspond to typical values ​​in natural uranium areas. However, in the communities near the mine, the dose values are slightly above the permissible limit (1 mSv y-1), so they must be continuously monitored, and risk mitigation measures must be taken.

RESUMO

Dual-energy CT has expanded the potential of thoracic imaging in both children and adults. Data processing allows material- and energy-specific reconstructions, which improve material differentiation and tissue characterization compared with single-energy CT. Material-specific reconstructions include iodine, virtual unenhanced, perfusion blood volume, and lung vessel images, which can improve assessment of vascular, mediastinal, and parenchymal abnormalities. The energy-specific reconstruction algorithm allows virtual monoenergetic reconstructions, including low-energy images to increase iodine conspicuity and high-energy images to reduce beam-hardening and metal artifacts. This review highlights dual-energy CT principles, hardware, and postprocessing algorithms; the clinical applications of dual-energy CT; and the potential benefits of photon counting (the most recently introduced iteration of spectral imaging) in pediatric thoracic imaging.

Abstract Introduction Temporal bone surgery is a unique and complicated surgical skill that requires extensive training. There is an educational requirement to maximize trainee experience and provide effective feedback. Objective We evaluate three temporal bone dissection scales for efficacy, reliability, and accuracy in identifying resident skill during temporal bone surgery. Methods Residents of various skill levels performed a mastoidectomy with posterior tympanotomy on identic 3D-printed temporal bone models. Four blinded otologic surgeons evaluated each specimen at two separate intervals using three separate dissection scales: the Welling Scale (WS), the Iowa Temporal Bone Assessment Tool (ITBAT), and the CanadaWest Scale (CWS). Scores from each scale were compared in their ability to accurately separate residents by skill level, inter- and intrarater reliability, and efficiency in application. Results Nineteen residents from 9 postgraduate programs participated. Assessment was clustered into junior (postgraduate year or PGY 1, 2), intermediate (PGY 3) and senior resident (PGY 4, 5) cohorts. Analysis of variance (ANOVA) found significant differences between cohort performance (p < 0.05) for all 3 scales considering the PGY level and the subjective account of temporal bone surgical experience. The inter-rater reliability was consistent across each scale. The intrarater reliability was comparable between the CWS (0.711) and the WS (0.713), but not the ITBAT (0.289). Time (in seconds) to complete scoring for each scale was also comparable between the CWS (42.7 ± 16.8), the WS (76.6 ± 14.5), and the ITBAT (105.6 ± 38.9). Conclusion All three scales demonstrated construct validity and consistency in performance, and consideration should be given to judicious use in training.

The Brazilian coast is rich in monazite which is found in beach sand deposits. In this study, the composition of the monazite sands from beaches of State of Espírito Santo, Brazil, was investigated. The concentrations of rare earth elements (REEs), Th, and U were determined by inductively coupled plasma mass spectrometry (ICP-MS). In the studied region, the mean concentration of investigated elements increased in the following order: Tm < Yb < Ho < Lu < Eu < Er < Tb < Dy < U < Y < Th < Gd < Sm < Pr < Nd < La < Ce. The sampling sites were classified into three clusters and discriminated by the concentrations of REEs, Th, and U found. In general, the radiological risk indices were higher than the established limits, and the risk of developing cancer was estimated to be higher than the world average.

BACKGROUND: Assessment of regional aortic wall deformation (RAWD) might better predict for abdominal aortic aneurysm (AAA) rupture than the maximal aortic diameter or growth rate. Using sequential computed tomography angiograms (CTAs), we developed a streamlined, semiautomated method of computing RAWD using deformable image registration (dirRAWD). METHODS: Paired sequential CTAs performed 1 to 2 years apart of 15 patients with AAAs of various shapes and sizes were selected. Using each patient's initial CTA, the luminal and aortic wall surfaces were segmented both manually and semiautomatically. Next, the same patient's follow-up CTA was aligned with the first using automated rigid image registration. Deformable image registration was then used to calculate the local aneurysm wall expansion between the sequential scans (dirRAWD). To measure technique accuracy, the deformable registration results were compared with the local displacement of anatomic landmarks (fiducial markers), such as the origin of the inferior mesenteric artery and/or aortic wall calcifications. Additionally, for each patient, the maximal RAWD was manually measured for each aneurysm and was compared with the dirRAWD at the same location. RESULTS: The technique was successful in all patients. The mean landmark displacement error was 0.59 ± 0.93 mm with no difference between true landmark displacement and deformable registration landmark displacement by Wilcoxon rank sum test (P = .39). The absolute difference between the manually measured maximal RAWD and dirRAWD was 0.27 ± 0.23 mm, with a relative difference of 7.9% and no difference using the Wilcoxon rank sum test (P = .69). No differences were found in the maximal dirRAWD when derived using a purely manual AAA segmentation compared with using semiautomated AAA segmentation (P = .55). CONCLUSIONS: We found accurate and automated RAWD measurements were feasible with clinically insignificant errors. Using semiautomated AAA segmentations for deformable image registration methods did not alter maximal dirRAWD accuracy compared with using manual AAA segmentations. Future work will compare dirRAWD with finite element analysis-derived regional wall stress and determine whether dirRAWD might serve as an independent predictor of rupture risk.

A sanitation method that could continually clean and disinfect the air and surfaces in a hatchery could provide a second layer of microbial reduction on top of routine cleaning and disinfection. A gaseous dry hydrogen peroxide (DHP) system has been used in other facilities for this purpose and could have potential for use in chicken hatcheries. Because the DHP is a true gas and can permeate through the entire hatchery space, contact with eggs during storage and incubation could potentially interfere with normal hatching processes. Therefore, the aim of this study was to evaluate the effects of the DHP system on hatching parameters and chick quality. A total of 3,960 hatching eggs were collected from an ∼40-week-old Ross 308 broiler breeder flock and distributed in 2 treatments: treated and nontreated. For the treated group, the egg cooler was cleaned, and 1 DHP generator was placed inside. Two other DHP generators were placed in the common area outside as well. Both areas were treated for 7 D before placement of eggs, and then eggs were collected and placed inside the cooler over a 4-day period. Eggs were then stored for an additional 3 D after the last collection. Dry hydrogen peroxide levels were recorded each day during storage. For the nontreated group, all DHP machines were removed from the cooler and external room, and the egg cooler was cleaned. Eggs were collected in the same way for the control group as the treated group. After storage, eggs were placed into a single stage Natureform incubator. The eggs exposed to DHP showed higher (P < 0.05) hatchability of fertile eggs and lower (P < 0.05) early embryonic dead than eggs from the nontreated group. No other parameters evaluated were different between groups. Based on this work, the DHP treatment of fertile eggs had no detrimental effect on any performance parameter, with potential positive effects seen on hatch of fertile eggs and early embryonic dead embryos.

High-throughput DNA sequencing facilitates the analysis of large portions of the genome in nonmodel organisms, ensuring high accuracy of population genetic parameters. However, empirical studies evaluating the appropriate sample size for these kinds of studies are still scarce. In this study, we use double-digest restriction-associated DNA sequencing (ddRADseq) to recover thousands of single nucleotide polymorphisms (SNPs) for two physically isolated populations of Amphirrhox longifolia (Violaceae), a nonmodel plant species for which no reference genome is available. We used resampling techniques to construct simulated populations with a random subset of individuals and SNPs to determine how many individuals and biallelic markers should be sampled for accurate estimates of intra- and interpopulation genetic diversity. We identified 3646 and 4900 polymorphic SNPs for the two populations of A. longifolia, respectively. Our simulations show that, overall, a sample size greater than eight individuals has little impact on estimates of genetic diversity within A. longifolia populations, when 1000 SNPs or higher are used. Our results also show that even at a very small sample size (i.e. two individuals), accurate estimates of FST can be obtained with a large number of SNPs (≥1500). These results highlight the potential of high-throughput genomic sequencing approaches to address questions related to evolutionary biology in nonmodel organisms. Furthermore, our findings also provide insights into the optimization of sampling strategies in the era of population genomics.

Physical mapping of the human immunoglobulin lambda locus (IGL) on chromosome 22q11 has shown the existence of at least 52 variable region gene segments. These Vlambda genes are associated with EcoRI fragments detectable in Southern blots of genomic DNA samples. The current physical map of the IGL locus includes a unique Vlambda8 gene (IGL8a, accession no. Z73650) in a 3.7 kb EcoRI fragment. However, our Southern blot-EcoRI-restriction fragment length polymorphism studies on the Brazilian population using a specific probe for the Vlambda8 gene (pVL8 probe) have revealed the presence of two additional fragments bearing Vlambda8 sequences (8.0 kilobase (kb) at 100% frequency and 6.0 kb at 10% frequency). We have used human/rodent somatic cell hybrid DNAs to locate these new Vlambda8 genes outside the major locus on chromosome 22q11.2. Polymerase chain reactions using specific primers for the IGLV8a gene on the somatic hybrid panel showed that chromosome 8 (besides 22q11) also comprises Vlambda8 sequences. This finding represents evidence for the dispersion of the human IGLV8 gene family outside the major locus (orphan genes).

Aunque la diarrea aguda es frecuente, los casos de enfermedad autolimitada, leve, no precisan de evaluacion o cualquier otro tratamiento distinto a la reposicion de liquidos y electrolitos. Los episodios de diarrea prolongada, diarrea con presencia de sangre, fiebre, o antecedentes de un viaje reciente o empleo de medicamentos, casi siempre requieren de investigacion, acompanada de terapia especifica. En la diarrea cronica liquida se necesita una investigacion mas amplia y tal vez de tratamiento empirico si no se puede establecer la causa especifica.

In most countries of the third world, strategies for development in the health sector include efforts to upgrade the skills of village level health care workers, including traditional birth attendants (TBAs). In spite of several decades of experience, training programs for TBAs have not been particularly successful. Drawing on data from several years of ethnographic fieldwork with Maya midwives in Yucatan and on participation in government-sponsored training courses for indigenous midwives, this paper examines some of the reasons underlying this failure. Paramount among these are differences in world view and the misapplication of didactic modes of teaching in situations where learning in the apprenticeship mode is more appropriate and culturally customary.
