BACKGROUND: Dose area product in water (DAPw) in small fields relies on the use of detectors with a sensitive area larger than the irradiation field. This quantity has recently been used to establish primary standards down to 5 mm field size, with an uncertainty smaller than 0.7%. It has the potential to decrease the uncertainty related to field output factors, but is not currently integrated into treatment planning systems. PURPOSE: This study aimed to explore the feasibility of converting DAPw into a point dose in small fields by determining the volume averaging correction factor. By determining the field output factors, a comparison between the so-called "DAPw to point dose" approach and the IAEA TRS483 methodology was performed. METHOD: Diodes, microdiamonds, and a micro ionization chamber were used to measure field output factors following the IAEA TRS483 methodology on two similar linacs equipped with circular cones down to 6 mm diameter. For the "DAPw to point dose" approach, measurements were performed with a dedicated and built-in-house 3 cm diameter plane-parallel ionization chamber calibrated in terms of DAPw in the French Primary Dosimetry Standards Laboratory LNE-LNHB. Beam profile measurements were performed to generate volume averaging correction factors enabling the conversion of an integral DAPw measurement into a point dose and the determination of the field output factors. Both sets of field output factors were compared. RESULTS: According to the IAEA TRS483 methodology, field output factors were within ±3% for all detectors on both linacs. Large variations were observed for the volume averaging correction factors with a maximum spread between the detectors of 26% for the smallest field size. Consequently, deviations of up to 15% between the "IAEA TRS483" and the "DAPw to point dose" methodologies were found for the field output factor of the smallest field size. This was attributed to the difficulty in accurately determining beam profiles in small fields. CONCLUSION: Although primary standards associated with small uncertainties can be established in terms of DAPw in a primary laboratory, the "DAPw to point dose" methodology requires volume averaging correction to derive a field output factor from DAPw measurements. None of the point detectors studied provided satisfactory results, and additional work using other detectors, such as film, is still required to allow the transfer of a DAP primary standard to users in terms of absorbed point dose.

RESUMO

The results of phase II and III trials on Stereotactic Body Radiation Therapy (SBRT) increased adoption of SBRT worldwide. The ability to replicate clinical trial outcomes in routine practice depends on the capability to reproduce technical and dosimetric procedures used in the clinical trial. In this systematic review, we evaluated if peer-reviewed publications of clinical trials in SBRT reported sufficient technical data to ensure safe and robust implementation in real world clinics. Twenty papers were selected for inclusion, and data was extracted by a working group of medical physicists created following the ESTRO 2021 physics workshop. A large variability in technical and dosimetric data were observed, with frequent lack of required information for reproducing trial procedures. None of the evaluated studies were judged completely reproducible from a technical perspective. A list of recommendations has been provided by the group, based on the analysis and consensus process, to ensure an adequate reproducibility of technical parameters in primary SBRT clinical trials. Future publications should consider these recommendations to assist transferability of the clinical trial in real world practice.

PURPOSE: To assess efficacy, toxicity, and their predictive factors for dynamic conformal arc arteriovenous malformations (AVM) stereotactic radiosurgery. METHOD: Data concerning 90 consecutive patients were retrospectively studied. Clinical, radiological, dosimetrical data and quality indexes were computed. RESULTS: AVM median volume was 1.06cc. Median prescribed dose was 22Gy. Total occlusion was obtained for 69% of patients. Post-radiosurgery annual hemorrhage rate was 2.2%. Predictive factor for total occlusion was delivered dose. Undesirable events occurred for 28% of patients. Predictive factors for adverse events were AVM revealing mode with seizure or headache, age≤28, AVM diameter≥3cm Spetzler-Martin score≥4, V12Gy≥2cc, large target volume and low homogeneity index (p<0.05). Brain parenchymal radiological reactions concerned 23% of patients, and their predictive factors were AVM revelation by seizure, deep localization, AVM diameter≥3cm, Spetzler-Martin score≥4, previous radiosurgery, numerous embolization, target volume, V12Gy and low homogeneity index (p<0.05). CONCLUSION: Occlusion rate and toxicities are comparable to other series. Specific attention must be paid on pre-treatment clinical data, and target volume should be as small as possible, without reducing the delivered dose.
