RESUMO

Diabetic mastopathy (DMP) is an uncommon collection of clinical, radiological, and histological features, classically described in premenopausal women with long-term insulin-dependent diabetes mellitus. This entity can mimic breast carcinoma, but, in the appropriate clinical and imaging setting, the diagnosis can be made by core biopsy, avoiding unnecessary surgeries. We report the case of a 34-year-old female, with a 12-year history of type 1 diabetes, who presented with bilateral breast lumps. Mammography, ultrasonography, and magnetic resonance imaging could not exclude the suspicion of malignancy, and a core biopsy was performed showing the typical histologic features of DMP. The literature is briefly reviewed.

OBJECTIVE: : The purpose of this study was to assess the effect of cervical conization on subsequent pregnancy and delivery outcomes. MATERIALS AND METHODS: : A retrospective case-control study was performed in patients who underwent conization from 2000 to 2005 and had a subsequent delivery. Two case controls were matched for each case studied. The obstetric outcomes of the 2 groups were compared. A subsequent analysis comparing the 2 excisional techniques used was performed. RESULTS: : The study population group included 87 women (29 cases and 58 controls). Overall, the mean gestation age at delivery (38 vs 39 wk, p = .003), prevalence of preterm birth (20.7% vs 5.2%, p = .025), mean birth weight (3,035 vs 3279 g, p = .018), and low birth weight (20.7% vs 1.7%, p = .02) were statistically different among the cases and controls.In the study group (n = 29), large loop excision of the transformation zone was used in 62% (n = 18) and laser was used in 38% (n = 11) of the cases. There was no significant difference in the medium depth of the excised tissue, the prevalence of preterm birth, low birth weight, and the cesarean delivery rate between the 2 techniques. CONCLUSIONS: : Despite the small number of cases, this study indicates that excision of the transformation zone is associated with an increased risk of overall preterm delivery and low-birth weight infants in subsequent pregnancies. No significant difference was found between the 2 cervical excision procedures.

OBJECTIVES: We assessed the accuracy of predicting severe twin birth weight discordance (>25%) using the estimated fetal weights (EFW) and abdominal circumference (AC) ratio. METHOD: A cohort of twin gestations underwent ultrasound examinations within two weeks from birth. We focused on the accuracy of EFW and on the diagnosis of severe birth weight discordance by the difference in EFWs and the AC ratio. RESULTS: The 661 eligible twin pairs included 51 (7.7%) severely discordant pairs. The accuracy of an EFW to predict the actual birth weight was quite poor, with an acceptable specificity (96.4%), but low sensitivity (28.6-40.5%), to detect severely discordant pairs, whereas an AC ratio of 1.3 detected these discordant pairs with sensitivity and specificity of 97.3-100% and 99.6-99.7%, respectively. CONCLUSION: By comparing EFWs, 59.5-71.4% of discordant pairs >25% are missed, whereas an AC ratio >1.3 would identify almost all cases.

BACKGROUND: Ovarian hyperstimulation syndrome (OHSS) is considered to be an iatrogenic complication of ovulation induction therapy. However, OHSS may rarely be associated with spontaneous ovulatory cycles, usually in multiple or molar gestations or hypothyroidism. Clinical manifestations vary and may be potentially fatal in severe cases. CASE REPORT: A 28-year-old primigravid, with no history of an infertility treatment, presented with enlarged ovaries and ascite. The patient had minor abdominal discomfort, without other complains. Ultrasound evaluation showed enlarged cystic ovaries and mild ascites. Hormonal profile revealed increased estradiol levels, with normal TSH and hCG. The patient was managed expectantly with no complications. DISCUSSION: Although spontaneous OHSS is a rare entity, it is important to consider it in cases of bilateral ovarian masses in pregnancy. Three different mechanisms responsible for the occurrence of spontaneous OHSS have been described. This syndrome is thought to be secondary to promiscuity of the FSH receptor for hCG and/or TSH. It may occur with normal or increased levels of hCG and/or TSH.

BACKGROUND: Tuberculosis remains a global health problem, being the genitourinary tract the second most common site for tuberculosis infection after the lungs. Genital tuberculosis is now undergoing a worrying recrudescence. CASE REPORT: We report two cases of postmenopausal women who presented with vaginal bleeding. General physical and gynecological examination detected no abnormality. Transvaginal pelvic ultrasound showed fluid in the endometrial cavity in both cases. The office hysteroscopy was suggestive of focal endometrial thickness and an endometrial biopsy was performed. The histopathologic examination of biopsies found epithelioid cell granulomas without malignant cells. Culture of the endometrium was positive for Mycobacterium tuberculosis. Involvement of other systems was not detected. The patients started receiving antituberculosis treatment. CONCLUSION: Genital tuberculosis is rare in postmenopausal women and responsible for only approximately 1% of postmenopausal bleeding. However it is a curable disease and an early diagnosis is important and may prevent unnecessary invasive procedures for the patient.

INTRODUCTION: Different studies have shown an association between smoking habits and the presence of abnormal spermograms. However these reports have not been consistent throw out the literature. OBJECTIVE: To analyze the effects of cigarette consumption in semen parameters from men attending our infertility outpatient clinic. METHODS: Retrospective comparative study of 327 men (417 spermograms) from an initial population of 372 men attending the infertility outpatient clinic of our institution during the year of 2007. The semen parameters were first compared between smokers and nonsmokers and then a heavy smokers/light smokers analysis took place. RESULTS: From the 327 men included in the study, 135 (41%) were smokers and 55 (40.7%) were heavy smokers. The demographic characteristics were similar between groups. The tobacco use was associated with an increased incidence of oligo/zoosperm, without difference in the other semen parameters. It was also noted a negative correlation between heavy smoking habits and semen volume abnormalities. CONCLUSIONS: The results of the present study suggest that smoking negatively affects male fertility.

We used a prospective cohort to analyze the effect of change in BMI rather than change in weight, in mothers carrying dichorionic twins from a population that did not receive any dietary intervention. A total of 269 mothers (150 nulliparas and 119 multiparas) were evaluated. The average change (%) from the pre-gravid BMI was 7.2+/-6.1, 17.4+/-8.2, and 28.7+/-10.8, at 12-14, 22-25, and 30-34 weeks, respectively, without difference between nulliparas and multiparas. The comparison between maternities below or above the average change from the pregravid BMI failed to demonstrate an advantage (in terms of total twin birthweight and gestational age) of an above average change from the pregravid BMI, even when the lower versus upper quartiles were compared. Our observations reached different conclusions regarding the recommended universal dietary intervention in twin gestations. A cautious approach is advocated towards seemingly harmless excess weight gain, as normal weight women may turn overweight, or even obese, by the end of pregnancy, and be exposed to the untoward effects of obesity on future health and body image.
