[Right obstructive pyelonephritis due to supra-piriform herniation of the pelvic ureter: a clinical case]. / Pyélonéphrite obstructive droite par étranglement herniaire supra-piriforme de l'uretère pelvien: cas clinique.

Ureteric herniations are extremely rare. We are reporting the case of a 57-year-old female presenting with an upper urinary tract sepsis due to a pelvic ureteral herniation into the supra-piriform sciatic foramen. The diagnosis was made by the CT scan and the treatment was performed in two steps: first the upper urinary tract was drained, and then the hernia was surgically treated. Although ureteral hernias are exceptional causes for nephritic colic, urologists must be aware of the specificity of the diagnosis and of the treatment in order to avoid complications.

[Cockett's syndrome caused by an ectopic kidney. Apropos of two cases]. / Syndrome de Cockett par rein ectopique. Rapport de deux cas.

Two original cases of Cockett's syndrome caused by an ectopic and/or malformed kidney are reported. Intravenous pyelography and computed tomography allow establishing the diagnosis. The choice of the therapy depends on the patency of the left common iliac vein.

[Acute necrotizing enterocolitis in adults. Analysis of a homogeneous series of 12 cases. Review of the literature]. / Entérocolite aiguë nécrosante de l'adulte. Analyse d'une série homogène de 12 cas. Revue de la littérature.

A retrospective study of 12 cases of Acute Necrotic Enterocolitis in adults is realized. The role of disease association, the diversity of clinical aspects encountered and the diagnostic value of metabolic acidosis are emphasized. In spite of early and aggressive surgery combined with intensive assisted visceral reanimation, mortality is higher than 60 p. 100.

[Iliac artery-ureteral fistula after aorto-iliac reconstructive surgery (apropos of 3 cases)]. / Fistules artérielles ilio-urétérales après chirurgie reconstructrice aorto-iliaque (à propos de 3 cas).

The authors report 3 cases of ilio-ureteric fistulae following aorto-iliac reconstructive surgery. They also review 6 similar cases published in the worldwide literature. All of these cases have in common the appearance of a uretero-arterial fistula several years after the vascular operation on the iliac artery. The postoperative course of iliac thromboendarterectomy appears to be principally responsible, as in the 3 cases presented here, resulting in a pseudoaneurysm over the arteriotomy closure, especially as a patch angioplasty had been performed. This complication is exceptional. The diagnosis must be suspected in the case of profuse haematuria and a suggestive history and should be confirmed, when possible, by intravenous urography, retrograde ureteropyelography, arteriography, or even computed tomography. Treatment is complex and consists of a vascular and urological procedure depending on the degree of urgency and the possibilities of reconstruction.

[Flash antibiotic prophylaxis in endoscopic resections of the prostate: cefamandole versus ceftriaxone]. / Antibioprophylaxie "flash" dans les résections endoscopiques de la prostate: céfamandole versus ceftriaxone.

The aim of this study is to compare the efficiency of Cefamandole and Ceftriaxone in short-term antibioprophylaxis after transurethral prostatectomy. Ceftriaxone reduces significatively postoperative febrile reactions, although no difference is seen between the 2 regimens: 27% in group Cefamandole versus 18% in group Ceftriaxone. Postoperative urinary tract infections do not depend neither on time resection nor on weight of resected tissues. Bladder catheter removal should be treated by short-term antibiotics in order to further reduce postoperative infections.
