RESUMO

OBJECTIVE: To assess the impact of intraoperative intravenous dexamethasone on the reduction of postoperative morbidity in children undergoing adenotonsillectomy. METHODS: A double blind randomized controlled trial conducted among children undergoing adenotonsillectomy at a tertiary hospital in Korea from November 2018 to June 2019. Children were randomly assigned to receive dexamethasone (0.5 mg/kg, maximum dose 24 mg) or placebo intravenously after induction of anesthesia. The primary endpoint was the reduction of postoperative pain and postoperative nausea and vomiting (PONV); secondary endpoints were adverse effects like postoperative hemorrhage. RESULTS: The study included 105 children, and 67 were male. Their mean age was 6.2 ±â€Š2.1 years. There was no significant difference between the groups in terms of demographic data or the operation time. The pain scores of the dexamethasone group were lower than those of the control group, but no significant difference was found (all P > .05). The average pain visual analog scale (VAS) during the study period (day 0-7) was 3.67 ±â€Š1.59 and 4.40 ±â€Š2.01 in the dexamethasone group and control group, respectively (P-value = .107). When we compared early pain VAS (day 0-2) and late pain VAS (day 5-7), the dexamethasone group showed significantly lower early mean VAS compared to the control group (4.55 ±â€Š1.78 vs 5.40 ±â€Š2.05, P-value = .046). The mean VAS for PONV was significantly lower in the dexamethasone group than in the control group (1.89 ±â€Š2.22 vs 3.00 ±â€Š2.37, P value = .044). CONCLUSION: In children undergoing adenotonsillectomy, dexamethasone decreased the early postoperative pain and PONV without increasing postoperative hemorrhage.

RESUMO

The assembly of Rh(CO)(2)Cl(amine) via molecular orbital symmetry interactions, resulting in the formation of one-dimensional molecular wires, has been discovered. The assembly behavior was quite dependent on the type of amine. The interchain distances and optical properties could be controlled by changing the length of the alkyl chain in the amine. We believe that this discovery can be applied not only to preparing more diverse one-dimensional molecular wires via the introduction of predesigned amines but also to gaining a deeper understanding of the physical properties of molecular metals.

RESUMO

A new building block containing an imidazolium salt was synthesized and used for the construction of supramolecular networks with metal ions. We discovered the concomitant formation of the N-heterocyclic carbene-copper complex (CN = 2) in the self-assembly of imidazolium dicarboxylates and copper nitrates in N,N-dimethylformamide under heating. The proton in the 2 position of the imidazolium salt was abstracted, and Cu(II) was reduced to Cu(I) during the self-assembly process.

RESUMO

[reactions: see text] A rhodium complex of N-heterocyclic carbene (NHC) has been developed for intra- and intermolecular [4 + 2] and intramolecular [5 + 2] cycloaddition reactions. This is the first use of a transition-metal NHC complex in a Diels-Alder-type reaction. For the intramolecular [4 + 2] cycloaddition reactions, all the dienynes studied were converted to their corresponding cycloadducts in 91-99% yields within 10 min. Moreover, up to 1900 turnovers have been obtained for the intramolecular [4 + 2] cycloaddition at 15-20 degrees C. For the intermolecular [4 + 2] cycloadditions, high yields (71-99%) of the corresponding cycloaddition products were obtained. The reaction time and yield were highly dependent upon the diene and the dienophile. For the intramolecular [5 + 2] cycloaddition reactions, all the alkyne vinylcyclopropanes studied were converted to their corresponding cycloadducts in 91-98% yields within 10 min. However, the catalytic system was not effective for an intermolecular [5 + 2] cycloaddition reaction.

Co/Rh (Co:Rh = 2:2) heterobimetallic nanoparticles derived from Co(2)Rh(2)(CO)(12) react with alkynes and alpha,beta-unsaturated aldehydes such as acrolein, crotonaldehyde, and cinnamic aldehyde and release products resulting from [2 + 2 + 1]cycloaddition of alkyne, carbon monoxide, and alkene. alpha,beta-Unsaturated aldehydes act as a CO and alkene source. These reactions produce 2-substituted cyclopentenones.
