[CO2-laser therapy of stigmatizing cutaneous lesions in tuberous sclerosis (Bourneville-Pringle) and in neurofibromatosis 1 (von Recklinghausen)]. / CO2-Laser-Therapie stigmatisierender Hautveränderungen bei tuberöser Sklerose (Bourneville-Pringle) und bei Neurofibromatose Typ I (von Recklinghausen).

Tuberous sclerosis (Bourneville-Pringle) and type I neurofibromatosis (von Recklinghausen) are familial multiple tumour syndromes. Both entities have in common that the cutaneous manifestations can stigmatize the carriers and considerably reduce the quality of life. CO2-laser surgery (vaporisation) consistently yields good to excellent aesthetic results in the treatment of adenoma sebaceum in tuberous sclerosis. Patients with type I neurofibromatosis who carry several hundreds neurofibromas usually benefit from a numerical reduction in their skin lesions, despite the less predictable aesthetic result, ranging from excellent to fair. The present study reports on the treatment outcome of CO2-laser surgery in 8 patients with adenoma sebaceum and 8 with neurofibromatosis.

[Use of lasers in therapy of malignant skin tumors]. / Einsatz von Lasern zur Therapie maligner Hauttumoren.

Malignant tumors of the skin may indicate different therapeutical approaches: Beside conventional surgery and other modalities, including modern methods as cytokines, laser therapy has been successfully introduced as a therapeutical approach. There are many different forms of laser application, not only due to the quality of the tumor but also due to the big number of different lasers and their different biophysical opportunities. The classical domain of laser therapy is the treatment of solar keratosis, mucosal precancerosis, epitheliomatous carcinoma and all benign tumors of the skin. In all the other tumorous entities of the skin, like melanoma and malignant tumors of blood vessels, laser is not the therapy of choice, except when used in a noncurative intention, mostly in combination with other modalities.

We report on a 51-year-old male patient who had been suffering from gouty arthritis and podagra with relapsing attacks of severe pain for the last 10 years. We treated two tophaceus deposits on the left heel with the CO2 laser and conventional enucleating surgery. These tophi had immobilized the patient for months and it had not been possible to control them by conservative therapy.
