BACKGROUND: The pathogenesis of mumps meningitis remains unclear. The aim of this study was to assess the relation between IgM and IgG levels in blood and their relationship with the picture of CSF. MATERIAL AND METHODS: We tested 40 children of both sexes aged 2-14 years. CSF was examined at admission (I) and after 12-14 days (II). Antibodies were measured four times: at admission (I), and after 2 (II), 4 (III), and 8 weeks (IV) by EIA (Behring). The results were expressed as delta absorbency. RESULTS: The mean IgM level was respectively 0.403; 0.424; 0.317; 0.220. Significant differences were demonstrated between tests I and III and between III and IV. The mean IgG level was respectively 0.923; 1.322; 1.381 and 1.257. Significant differences were noted between tests I and II and between III and IV. We observed significant positive correlations between the IgM levels in tests I and II, II and III, and III and IV. The IgG concentrations significantly correlated in tests I and II, II and III, and II and IV. Significant negative correlations were noted between the IgG levels at test I and the IgM levels at test II. A negative correlation also appeared between the IgM level and CFS pleocytosis at admission, and between the IgM concentration and CSF pleocytosis after two weeks. A positive correlation was found between the IgG level at test I and the CSF glucose level in test II. In this study a high IgG level probably resulted in a lower IgM level at the next test. The IgM concentration at week 8 weeks was about 50% lower than its highest concentration (week 2), while the IgG level decreased simultaneously. The IgM concentration at hospital admission and two weeks later influenced CSF pleocytosis during these periods (higher IgM level - lower pleocytosis). A high IgG level at admission resulted in a lower CSF glucose concentration at the second examination.

The fourth component of complement (C4) phenotype and allele frequencies were studied in 3 groups of unrelated children affected with hepatitis B virus (HBV) infection (25 with glomerulonephritis (GN), 39 with chronic active hepatitis (CAH), 24 with Gianotti-Crosti syndrome (GS-S)) and in 100 unrelated healthy individuals (H). Ten phenotypes in GN, 14 in CAH, 10 in GC-S and 23 in H were found. Some rare phenotypes appeared in the individual cases of ill children. Statistical analysis was performed by chi-square test. Each group was compared to the others. We found significantly higher frequency of the A3B2,Q0 phenotype in CAH than in H as well as significant increase of the A x 4 gene frequency in GC-S as compared to H. However, these events could be caused by limited number of tested patients. We suspect that the association between C4 alleles and immune-complex diseases, studied here, does not rather exist.

Four groups of children were tested for the distribution of C3 and BF phenotypes: HBV-infected patients with Gianotti-Crosti syndrome, with CAH and with glomerulonephritis, and healthy children. The frequency of the phenotype C3F was significantly higher in children with Gianotti-Crosti syndrome in comparison with healthy children. The frequency of the phenotype BFS was significantly higher in patients with glomerulonephritis than in individuals with CAH. The difference between the frequency of the BFS phenotype in glomerulonephritis patients and that in healthy subjects neared significance. We suggest that the carriers of these phenotypes are characterized by susceptibility to some immune complex diseases associated with HBV infection.

Children with mumps meningitis, with mumps without meningitis and healthy subjects were examined for Gm 1, 2, 3, 5, 21 allotypes and for the Km 1 allotype. The comparison of the frequencies of Gm and Km phenotypes in three tested groups (each to the other) did not reveal significant differences. An association between mumps meningitis and the Gm or Km phenotypes was not found.

RESUMO

Slowly developing tuberculosis in a BCG-vaccinated infant was analysed in a 2-month-old child suffering from cervico-lymphadenitis. It was diagnosed after 8-months period of non-specific treatment as tuberculosis. Further observation revealed severe forms of disease-miliary tuberculosis and meningoencephalitis. Familial contact with tuberculosis was established. The outcome after 9 months of anti-tuberculosis therapy was successful.

RESUMO

Patients with CAH, extrahepatic HBV manifestation and healthy children were studied for presence of Gm 1, 2, 3, 10, 21 factors and Km 1 factor. significantly higher frequency of Gm (1, 2, 3, 10, 21) phenotype was shown in CAH group as compared with the other two groups. Relationship between Km factors and examined groups was not found.

RESUMO

The frequency of HLA-DR antigens was studied in 54 patients with mumps meningitis, and in two control groups. The first control group comprised 333 randomly chosen subjects while the second group was composed of 46 subjects who have passed mumps without meningitis. The DR5 antigen was more frequent in patients than in the second control group with relative risk of 2.4 and significance at the level of p < 0.05. The p value became insignificant after the correction for the number of antigens investigated.

RESUMO

The newborn with sepsis (E. coli) and salmonellosis was described. After treatment with broad-spectrum antibiotics it was suffered from Candida albicans (Meningitis with Hydrocephalus internus, Chorioretinitis and Ostitis). The authors difficulties connected with diagnosis and anti-fungal therapy have showed. They have punctated the necessity of combined anti-mycotic therapy.

RESUMO

Sixty-eight children with the symptoms of infectious mononucleosis were examined. In 25 of them anti-EBV antibodies were found and in 10 heterophilic antibodies. In serum of 16 children sero-positive to EBV A, M, G, D immunoglobulins were determined as well as T and B lymphocytes. A statistically significant increase in the number of B-IgA, B-IgG and B-IgM lymphocytes, augmentation of T lymphocyte number and polyclonal hypergammaglobulinemia were detected.
