RESUMO

A total of 165 patients presenting with neurosensory impairement of hearing and Meniere's disease were treated by supravascular (extracorporeal) laser irradiation of blood. The study undertaken to evaluate the efficacy of this treatment confirmed its beneficial effect in patients with acute hearing disorders. It was equally effective as regards elimination of labyrinthine hydropsis in patients presenting with Meniere's disease. The method proved less efficacious for the management of long-standing hearing impairement and chronic loss of hearing, but it can be used to prevent the development of these conditions.

RESUMO

The authors present pilot experience with transcranial duplex scanning (TCDS) of the cerebral vessels in 17 patients with secondary cochleovestibular neuritis combined with chronic leptomeningitis (primarily of the posterior cranial fossa). This method has determined extra- and intracranial hemodynamics, characterized arterial and venous blood flow qualitatively and quantitatively, outlined possible genetically determined factors in development of dyscirculatory disorders. Finally, hemodynamic defects of two types were revealed in 12 patients. Type 1 (3 patients) was characterized by asymmetric circulation in the territory of the middle, anterior and posterior cerebral arteries, by the absence of venous congestion. Type 2 (9 patients) was characterized by impaired venous outflow manifesting as higher speed of the blood flow and its phase response along the intracranial veins and sinuses. In 7 patients these defects combined with asymmetric circulation along brain stem arteries. These patients were diagnosed to have perilymphatic labyrinthine hydrops.

RESUMO

The experience gained by the authors in the treatment of Menière's disease and similar affections which are supposed to have 3 stages provided the following approach to their treatment: after examination of the cochleovestibular analyzer involvement good results may be expected of conservative treatment in stage I-II, chordoplexectomy is justified in stage I, drainage or decompression of the endolymphatic sac and vestibulotomy are indicated in stage II and III, respectively.

RESUMO

Patients with cochleovestibular disorders of central and peripheral origin were treated with monothreane (produced by Luitpold Werk, FRG). The drug was given to 70 patients (19 men and 51 women) at the age of 28 to 64 years, 28 patients of which had disorders of peripheral origin and 42 patients disorders of mixed origin. The patients were examined by clinical methods and special tests and hypertension was diagnosed in 32 patients. The therapy yielded good effects in 61 patients: complete recovery of vestibular disorders was seen in 28 patients and relief of headaches in 42 subjects. Side effects were observed in nine patients who showed reduction of perception thresholds which disappeared after drug withdrawal. Monothreane had a beneficial effect on the course of the hypertensive disease.
