BACKGROUND: The purpose of this study was to evaluate the prevalence of foot involvement in psoriatic arthritis and to describe its different clinical and radiological features. PATIENTS AND METHODS: We conducted a cross sectional study including 40 patients with psoriatic arthritis over a period of 12 months. Anamnesis, clinical examination of feet, podoscopic examination, X-rays of feet and heels, and ultrasound in B mode and power Doppler mode were done for each patient. RESULTS: Foot involvement was found in 95% of cases. It was symptomatic in 70% and inaugural of the disease in 20% of cases. The hindfoot and the forefoot were the sites most affected (77.5% and 47.5% respectively). The involvement of the midfoot was rarer (25%). Dactylitis was found in 17.5% and deformities of forefoot were found in 22.5% of cases. Antalgic gait was noted in 17.5% and static disorders of foot at podoscopic examination were identified in 35% of cases. Feet dermatological manifestations were found in 45% of cases. Diagnosis of different rheumatological manifestations was based on clinical findings and caracteristic radiological images on X-rays. We demonstrate he sensitivity of ultrasound in the detection and the diagnosis of different foot lesions including enthesitis, synovitis and tenosynovitis, dactylitis, bone erosions and psoriatic nail dystrophy.

This study focuses on the characterization and regulation of glycolipid metabolism of polysaccharides derived from biomass of Phyllostachys nigra (Lodd. ex Lindl.) root (PNr). The extracts from dilute hydrochloric acid, hot water, and 2 % sodium hydroxide solution were characterized through molecular weight, gel permeation chromatography, monosaccharides, Fourier transform infrared, and nuclear magnetic resonance spectroscopy analyses. Polysaccharide from alkali extraction and molecular sieve purification (named as: PNS2A) exhibited optimal inhibitory of 3T3-L1 cellular differentiation and lowered insulin resistance. The PNS2A is made of a hemicellulose-like main chain of →4)-ß-D-Xylp-(1→ that was connected by branches of 4-O-Me-α-GlcAp-(1→, T-α-D-Galp-(1→, T-α-L-Araf-(1→, →2)-α-L-Araf-(1→, as well as ß-D-Glcp-(1→4-ß-D-Glcp-(1→ fragments. Oral delivery of PNS2A in diabetes mice brought down blood glucose and cholesterol levels and regulated glucose and lipid metabolism. PNS2A alleviated diabetes symptoms and body weight and protected liver and kidney function in model animals by altering the gut microbiome. Polysaccharides can be a new approach to develop bamboo resources.

Key Clinical Message: Hydatidosis is a parasitic infection caused by the larval form of "Echinococcus granulosis." Bone localization is rare even in endemic areas. We report an unusual case of an extensive hydatidosis of the right sacrum and femur with muscle involvement. Abstract: We report the case of a man, with a history of visceral hydatidosis, who developed after 14 years an extensive hydatidosis of the sacrum and femur with muscle involvement. The treatment of visceral hydatidosis was chemotherapy with albendazole for a long term. Conservative surgical treatment is considered for bone locations.

RESUMO

Cord compression and ischemic non-compressive myelopathy are a complication of spinal Paget's disease (SPD). SPD usually touches a single spine level. We report an unusual case with bilevel spinal cord compression and dysfunction which was medically treated due to resolution of the vascular steal syndrome.

The estimation of the slowness vector of infrasound waves propagating across an array is a critical process leading to the determination of parameters of interest such as the direction of arrival. The sensors of an array are often considered to be located in a horizontal plane. However, due to topography, the altitudes of the sensors are not identical and introduce a bias on the estimate if neglected. However, the unbiased 3D estimation procedure, while suppressing the bias, leads to an increase of the variance. Accounting for an a priori constraint on the slowness vector significantly reduces the variance and could therefore improve the performance of the estimation if the introduced bias by incorrect a priori information remains negligible. This study focuses on measuring the benefits of this approach with a thorough investigation of the bias and variance of the constrained 3D estimator, which is not available in the existing literature. This contribution provides such computations based on an asymptotic Gaussian approximation. Simulations are carried out to assess the theoretical results both with synthetic and real data. Thus, a constrained 3D estimator is proposed yielding the best bias/variance compromise if good knowledge of the propagation wave speed is accessible.

An original micellar electrokinetic chromatography (MEKC) method using in-capillary derivatization with a chiral labeling reagent was developed for the separation of amino acid (AA) derivatives. The potential of (-)-1-(9-fluorenyl)-ethyl chloroformate (FLEC) as in-capillary derivatization agent is described for the first time. Several parameters for in-capillary derivatization and subsequent MEKC separation were systematically investigated using experimental designs. Firstly experimental conditions for in-capillary derivatization were optimized using face-centered central composite design (FCCD). Mixing voltage and time as well as concentration of the labeling solution were investigated. Efficient labeling was achieved by sequential injection of AAs and FLEC labeling solution followed by the application of a voltage of 0.2 kV for 570 s. The background electrolyte (BGE) composition was then optimized in order to achieve selectivity. A FCCD was performed with two factors, namely the sodium dodecyl sulfate (SDS) concentration and the percentage of propan-2-ol (IPA). The separation of 12 pairs of derivatized AA (FLEC-AA) diastereomers was achieved with resolution values comprised between 3 and 20. Furthermore, an efficient derivatization and separation of 29 FLEC-AA derivatives were achieved in a single run using a buffer made up of 40 mM sodium tetraborate, 21 mM SDS and 8.5% IPA. The method was successfully applied to the analysis of spiked artificial cerebrospinal fluid (aCSF) sample.

A novel dual chiral CE method was developed for the separation of l- and d-amino acids (AAs), using in-capillary derivatization with 9-fluoroenylmethyl chloroformate (FMOC). Firstly, using pre-column derivatization, the enantioseparation of FMOC-AAs was optimized according to the nature of cyclodextrins (CD). A background electrolyte (BGE) composed of 30 mM ß-CD, 30 mM octakis(2,3-dihydroxy-6-O-sulfo)-γ-CD (OS-γ-CD), 40 mM tetraborate and 15% isopropanol (IPA) was selected and led to 17 baseline resolved pairs (R(s)=1.7-5.8) and two partially resolved pairs (Lys, R(s)=0.5 and Arg, R(s)=1.2). Experimental conditions for in-capillary derivatization were then optimized. Several parameters, such as mixing voltage and time, concentration of labeling solution and the length of the spacer plug were studied. The optimal conditions for in-capillary derivatization procedure were obtained using successive hydrodynamic injections (30 mbar) of AAs for 2s, borate buffer for 4s and 10mM FMOC solution for 6s, followed by a mixing at 3 kV for 72 s and wait time of 1 min. Moreover, a particular attention was paid to improve separation chemoselectivity. The effect on stereoselectivity and chemoselectivity of different factors, such as decrease of pH and tetraborate concentration and the addition of sodium dodecyl sulfate (SDS), was investigated using the in-capillary derivatization procedure. The best separation of a standard mixture of ten AA racemates was observed using a BGE containing 30 mM ß-CD, 30 mM OS-γ-CD, 25 mM SDS, 40 mM sodium tetraborate and 17% IPA.
