RESUMO

PURPOSE: We assessed the value of standard biochemical coagulation parameters in predicting bleeding, thrombosis and mortality in adult Intensive Care Unit (ICU) patients with haematological malignancies. METHODS: We screened all patients with acute leukaemia and myelodysplastic syndrome admitted to a university hospital ICU during 2008-2012. Data were obtained from the clinical chemistry laboratory database and patient files. We graded bleeding according to the World Health Organisation (WHO)-system within 24-h, within 5-days and during the whole ICU stay. We analysed the predictive values of laboratory parameters using multiple logistic regression and receiver operator characteristics (ROC) curves. As we previously have established that platelet count at admission was associated with bleeding, we focused on International Normalised Ratio (INR), activated pro-thrombin time (APTT), anti-thrombin, D-dimer and fibrinogen, and markers of infection (C-reactive protein, pro-calcitonin), kidney function (creatinine) and tissue damage (lactate dehydrogenase (LDH)). RESULTS: We included 116 patients; 66 (57%) had at least one bleeding episode and 11 (9%) patients had at least one thrombotic event. The differences in coagulation values when bleeding compared to baseline values were minor. INR was the only variable we found associated with subsequent bleeding within 24 h from admission to ICU (odds ratio 2.91, 95% CI: 1.01-8.43, P = 0.048). ROC analyses did not show predictive value of any of the other variables with regards to bleeding and none of the variables were associated with thrombosis in adjusted analyses. Increased levels of LDH at admission were associated with increased 7-day and 30-day mortality. CONCLUSIONS: Increased INR at admission was associated with a higher rate of bleeding in ICU patients with haematological malignancies. No other biochemical coagulation or other parameter had any association with bleeding, thrombosis or mortality except increased LDH, which at ICU admission was associated with increased 30-day mortality.

Pro human neutrophil peptides (proHNP)s are proforms of α-defensins produced by precursors of human neutrophils. They are secreted to bone marrow plasma in large amounts by myelocytes. We hypothesized that the plasma concentration of proHNPs might serve as a specific marker of myelopoietic activity, heralding the onset of normal myelopoiesis before reappearance of neutrophils, in the setting of bone marrow regeneration. To investigate this, plasma levels of proHNPs were measured by enzyme-linked immunosorbent assay in blood samples collected from patients undergoing allogeneic (n=11) or autologous (n=16) stem cell transplantations (SCTs) and patients receiving chemotherapy for acute leukemia (n=14). To compare proHNPs with previously suggested myeloid markers, myeloperoxidase (MPO), lysozyme and neutrophil gelatinase-associated lipocalin (NGAL) were also assayed. In all but one patient, chemotherapy led to the complete disappearance of ProHNPs from plasma. It reappeared in plasma on average 6.3 days before reappearance of neutrophils in the allogeneic setting, whereas this was reduced to an average of 2.8 days in the autologous SCT patients who received granulocyte colony-stimulating factor. Patients with acute myeloid leukemia (n=19) had significantly lower levels of plasma proHNPs than healthy controls, indicating that proHNPs are not produced by leukemic blasts. We conclude that plasma concentration of proHNPs is a clinically useful marker of normal myelopoiesis.

The implementation of clinical pathways has a proven positive effect on the diagnostic workup and initiation of therapy in osteoporotic fracture patients. Unlike in most countries, fracture care in Germany is provided by so-called trauma surgeons. Therefore, it is essential to focus on the trauma surgeon for correct diagnostic workup and therapy initiation after a fragility fracture. A questionnaire was mailed to 409 departments of traumatology inquiring about the existence of a standardized clinical pathway for diagnosis and treatment of patients with fragility fractures. One of the central issues of the survey was whether those pathways comply with national guidelines. Only institutions that stated that they followed a clinical pathway were analyzed. 80% of institutions took part in our survey, 35% of which reported following a defined clinical pathway. Diagnostic workup is in concordance with the national guidelines in 30%, and therapy is guideline-based in 51%, with 12% basing both diagnostic workup and therapy on the guidelines. Thus, the vast majority of German traumatology departments do not follow national guidelines regarding osteoporosis diagnostics and therapy in patients with fragility fractures, leading to a great opportunity to improve fragility fracture care by means of both education and interdisciplinary cooperation.
