RESUMO

BACKGROUND/PURPOSE: There is a paucity of comprehensive information about posterior open bite (POB) in patients with temporomandibular disorders (TMD) because of its rare prevalence. The purpose of this study was to investigate the etiologies, clinical characteristics, and treatment outcomes of patients with TMD presenting POB. MATERIALS AND METHODS: This study includes a careful review of medical records and imaging findings of 12 patients with TMD (seven men and five women, 50.9 ±â€¯19.2 years, 15-72 years) complaining of POB. RESULTS: In total, 11 had unilateral POB, whereas 1 had bilateral POB. In 11 patients, POB was caused by inflammatory disorders of temporomandibular joint (TMJ). In the remaining one patient, TMJ medial disc displacement (MDD) was responsible for POB. Of 11 patients with inflammatory conditions of TMJ, four patients had unilateral TMJ internal derangement (ID), two had bilateral TMJ ID, and one had rheumatism. POB was resolved in 10 of 11 patients with TMJ inflammation following the administration of non-steroidal anti-inflammatory drugs and self-management instructions. Prosthodontic treatment was needed in one patient to resolve POB. POB was resolved in the patient with TMJ MDD after stabilization splint therapy. CONCLUSION: POB in patients with TMD was mostly caused by inflammatory disorders of TMJ. TMJ MDD could also be a reason. Although almost all POB was resolved by conservative treatments including medications, the possibility of prosthodontic, orthodontic, or surgical treatments also must be considered.

RESUMO

Although there have been several reports regarding the association between hypoxic hepatic injury and clinical outcomes in patients who underwent conventional cardiopulmonary resuscitation (CPR), limited data are available in the setting of extracorporeal CPR (ECPR). Patients who received ECPR due to either in- or out-of-hospital cardiac arrest from May 2004 through December 2018 were eligible. Hypoxic hepatitis (HH) was defined as an increased aspartate aminotransferase or alanine aminotransferase level to more than 20 times the upper normal range. The primary outcome was in-hospital mortality. In addition, we assessed poor neurological outcome defined as a Cerebral Performance Categories score of 3 to 5 at discharge and the predictors of HH occurrence. Among 365 ECPR patients, 90 (24.7%) were identified as having HH. The in-hospital mortality and poor neurologic outcomes in the HH group were significantly higher than those of the non-HH group (72.2% vs. 54.9%, p = 0.004 and 77.8% vs. 63.6%, p = 0.013, respectively). As indicators of hepatic dysfunction, patients with hypoalbuminemia (albumin < 3 g/dL) or coagulopathy (international normalized ratio > 1.5) had significantly higher mortalities than those of their counterparts (p = 0.005 and p < 0.001, respectively). In multivariable logistic regression, age and acute kidney injury requiring continuous renal replacement therapy were predictors for development of HH (p = 0.046 and p < 0.001 respectively). Furthermore age, arrest due to ischemic heart disease, initial shockable rhythm, out-of-hospital cardiac arrest, lowflow time, continuous renal replacement therapy, and HH were significant predictors for in-hospital mortality. HH was a frequent complication and associated with poor clinical outcomes in ECPR patients.

RESUMO

OBJECTIVES: To investigate the relationships among four different gustatory function tests in healthy young adults: electrogustometry (EGM), filter paper disk (FPD), whole-mouth, and taste strip methods. The relationships of the results of gustatory function tests with salivary flow rate were also investigated. METHODS: Sixty healthy young adults (30 men, 26.9 ± 4.7 years; 30 women, 25.7 ± 4.6 years) who did not have disorders or conditions related with gustatory function were included. Four different gustatory function tests using the EGM, FPD, whole-mouth, and taste strip methods were performed in each participant with 2- to 3-day intervals between tests. The flow rates of unstimulated and stimulated whole saliva were measured. RESULTS: There were no significant differences between sexes in all the examined gustatory function tests. The levels of correlations between the gustatory function tests were low. The EGM threshold correlated with the taste score of the FPD method in the chorda tympani nerve area. Different chemical gustatory function tests did not correlate significantly in any of the four taste qualities. Salivary flow rates did not correlate with taste perception. CONCLUSIONS: The correlations between gustatory function tests were weak. A significant correlation was found between the results of EGM and FPD methods in the chorda tympani nerve area.

RESUMO

A broad range of redox-regulated proteins undergo reversible disulfide bond formation on oxidation-prone cysteine residues. Heightened reactivity of the thiol groups in these cysteines also increases susceptibility to modification by organic electrophiles, a property that can be exploited in the study of redox networks. Here, we explored whether divinyl sulfone (DVSF), a thiol-reactive bifunctional electrophile, cross-links oxidant-sensitive proteins to their putative redox partners in cells. To test this idea, previously identified oxidant targets involved in oxidant defense (namely, peroxiredoxins, methionine sulfoxide reductases, sulfiredoxin, and glutathione peroxidases), metabolism, and proteostasis were monitored for cross-link formation following treatment of Saccharomyces cerevisiae with DVSF. Several proteins screened, including multiple oxidant defense proteins, underwent intermolecular and/or intramolecular cross-linking in response to DVSF. Specific redox-active cysteines within a subset of DVSF targets were found to influence cross-linking; in addition, DVSF-mediated cross-linking of its targets was impaired in cells first exposed to oxidants. Since cross-linking appeared to involve redox-active cysteines in these proteins, we examined whether potential redox partners became cross-linked to them upon DVSF treatment. Specifically, we found that several substrates of thioredoxins were cross-linked to the cytosolic thioredoxin Trx2 in cells treated with DVSF. However, other DVSF targets, like the peroxiredoxin Ahp1, principally formed intra-protein cross-links upon DVSF treatment. Moreover, additional protein targets, including several known to undergo S-glutathionylation, were conjugated via DVSF to glutathione. Our results indicate that DVSF is of potential use as a chemical tool for irreversibly trapping and discovering thiol-based redox partnerships within cells.

RESUMO

The aim of this study was to assess the states of watersheds in South Korea and to formulate new measures to improve identified inadequacies. The study focused on the watersheds of the Han River basin and adopted an adaptive watershed management framework. Using data collected during watershed investigation projects, we analyzed the management context of the study basin and identified weaknesses in water use management, flood management, and environmental and ecosystems management in the watersheds. In addition, we conducted an interview survey to obtain experts' opinions on the possible management of watersheds in the future. The results of the assessment show that effective management of the Han River basin requires adaptive watershed management, which includes stakeholders' participation and social learning. Urbanization was the key variable in watershed management of the study basin. The results provide strong guidance for future watershed management and suggest that nonstructural measures are preferred to improve the states of the watersheds and that consistent implementation of the measures can lead to successful watershed management. The results also reveal that governance is essential for adaptive watershed management in the study basin. A special ordinance is necessary to establish governance and aid social learning. Based on the findings, a management process is proposed to support new watershed management practices. The results will be of use to policy makers and practitioners who can implement the measures recommended here in the early stages of adaptive watershed management in the Han River basin. The measures can also be applied to other river basins.

RESUMO

We investigate the dependence of various efficiencies in GaN-based vertical blue light-emitting diode (LED) structures on the thickness and doping concentration of the n-GaN layer by using numerical simulations. The electrical efficiency (EE) and the internal quantum efficiency (IQE) are found to increase as the thickness or doping concentration increases due to the improvement of current spreading. On the contrary, the light extraction efficiency (LEE) decreases with increasing doping concentration or n-GaN thickness by the free-carrier absorption. By combining the results of EE, IQE, and LEE, wall-plug efficiency (WPE) of the vertical LED is calculated, and the optimum thickness and doping concentration of the n-GaN layer is found for obtaining the maximum WPE.

RESUMO

PURPOSE: The aims of this study were to suggest a method of fabrication of the record base using a light-polymerized resin by applying the two-phase fabrication method for the improvement of the fit of the record base and to compare the degree of fit according to the separation site. MATERIALS AND METHODS: In the edentulous cast of maxilla, four test groups were considered. In the first, second, third, and fourth test groups (n = 12 in each group) the separation was done at 0, 5, 10, and 15 mm, respectively below the alveolar crest along the palatal plane. For the control group, the record base was made without separating the two sections. The light-body silicone material was injected into the fitting surface of the record base. It was then placed onto the cast and finger pressure was applied to stabilize it in a seated position followed by immediate placement onto the universal test device. Finally, the mass of the impression material was measured after it was removed. ANOVA was performed using the SAS program. For the post-hoc test, the Wilcoxon Rank-Sum test and the Tukey-Kramer HSD test were performed (α = 0.05). RESULTS: The control group and Group 3, 4 showed significant differences. The Group 3 and 4 showed significantly smaller inside gaps than the control group which was not made with the two-phase fabrication method. CONCLUSION: The two-stage polymerized technique can improve the fit of the denture base particularly when the separation was made at 10 to 15 mm from the alveolar crest.
