BACKGROUND: Constipation is a common problem among advanced cancer patients; however, many of them find limited effective from current therapies. Thus, we aimed to test the effect of a traditional Chinese herbal formula, modified MaZiRenWan (MZRW), by comparing with placebo among palliative cancer patients with constipation. METHODS: This is a randomized, double-blind, placebo-controlled trial. Participants aged over 18 were recruited and randomized to MZRW or placebo group in addition to current prescriptions (including ongoing laxatives treatment) for two weeks. Exclusion criteria included cognitive impairment, presence of a colostomy or gastrointestinal obstruction and estimated life expectancy of less than one month. Individualized modification of MZRW was allowed according to the traditional Chinese medicine (TCM) pattern of patient. The primary outcome was the global assessment of improvement, which reflected whether the constipation had improved, remained the same or worsened. RESULTS: Sixty patients, with mean age 75.2 years (range 47-95 years), were randomized to MZRW or placebo group. Among the MZRW group, 59.3% (16/27) had improvement in the global assessment score, as compared with 28.6% (8/28) of the placebo group (p-value = 0.022). Besides, the MZRW group had significant increase in stool frequency, and reduction in constipation severity and straining of defecation (p-value < 0.05). No serious adverse event was reported due to the research medication. CONCLUSION: This pilot trial suggests modified MZRW is well-tolerated and effective for relief of constipation in patients with advance cancer. It could be considered as a potential treatment option for constipation in palliative care. RESUMO

The complete mitochondrial genome of Dermanyssus gallinae isolated from China is reported for the first time in this study. Its entire mitogenome is 16, 184 bp in length, contained 13 protein-coding genes, 2 ribosomal RNA genes, 21 transfer RNA genes, and 1 non-coding region. The phylogenetic analysis by maximum likelihood method show that D. gallinae isolated from China is in the same clade with the genus of Psoroptes. This is the first complete mitochondrial genome of D. gallinae.

The survival of microglia depends on the colony-stimulating factor-1 receptor (CSF1R) signaling pathway under physiological conditions. Ki20227 is a highly selective CSF1R inhibitor that has been shown to change the morphology of microglia. However, the effects of Ki20227 on the progression of ischemic stroke are unclear. In this study, male C57BL/6 mouse models of focal cerebral ischemic injury were established through the occlusion of the middle cerebral artery and then administered 3 mg/g Ki20227 for 3 successive days. The results revealed that the number of ionized calcium-binding adaptor molecule 1/bromodeoxyuridine double positive cells in the infarct tissue was reduced, the degree of edema was increased, neurological deficits were aggravated, infarct volume was increased, and the number of peri-infarct Nissl bodies was reduced. The number of terminal deoxynucleotidyl transferase dUTP nick-end labeling-positive cells in the peri-infarct tissue was increased. The expression levels of Bax and Cleaved caspase-3 were up-regulated. Bcl-2 expression was downregulated. The expression levels of inflammatory factors and oxidative stress-associated factors were increased. These findings suggested that Ki20227 blocked microglial proliferation and aggravated the pathological progression of ischemia/reperfusion injury in a transient middle cerebral artery occlusion model. This study was approved by the Animal Ethics Committee of Lanzhou University Second Hospital (approval No. D2020-68) on March 6, 2020.

Eight rice varieties suitable for local cultivation were selected and field tests were carried out to compare the accumulation, absorption, and transport characteristics of Cd. The rice varieties with low Cd accumulation were screened, and the factors affecting the accumulation degree and stability of rice were analyzed. The results showed that:①According to the comprehensive accumulation index(PN) evaluation method, the rice varieties with low PN ability for Cd were selected as follows:Chuanyou 3203, Chuanyou 6203, Dejing 6, and Shenyou 17. ②Considering the Cd content and yield of rice, Chuanyou 3203 and Chuanyou 6203 are suitable for planting in this region, which guarantees a safe Cd content in and high yield of rice. ③The contents of Cd in Chuanyou 3203 and Chuanyou 6203 were significantly lower than those of other rice varieties and stable under different pH and total Cd ranges and soil conditions. ④The difference between rice varieties had a significant effect on rice enrichment ability, but little effect on translocation from stem to leaf. The low enrichment coefficient of Chuanyou 3203 and Chuanyou 6203, combined with low translocation, demonstrated the low Cd enrichment capacity of rice. ⑤Correlation analysis showed that the Cd content was most affected by the enrichment coefficient of rice, and least affected by the total Cd content of rhizosphere soil.

Phytochelatins (PCs) can chelate heavy metal ions due to their large number of thiols and play an important role in heavy metal accumulation and detoxification. A. hypochondriacus K472, a cadmium (Cd) enriched plant, was selected as the research object. Six Cd treatment concentrations, namely 0 (CK), 10 (T1), 25 (T2), 50 (T3), 100 (T4), and 200 mg ·kg-1 (T5), were used to analyze the variation of PCs in different growth stages under different degrees of Cd stress and to explore the mechanism by which PCs chelate and detoxify Cd. The results showed that the plant height, root length, and biomass of K472 decreased significantly with increasing Cd concentration, and the range of decrease gradually became less pronounced with the growth and development of K472. K472 exhibited the maximum ability to enrich Cd during the middle vegetative growth period. The maximum concentration was 6695.35 mg, and the maximum bioconcentration factor was 6.3. In addition, with increasing Cd concentration, the Cd content of K472 roots, stems, and leaves was positively correlated with the concentration of PCs. PC3 had the strongest response to Cd stress in roots and stems, whereas PC2 responded to stress in leaves. For practical applications, harvesting K472 in the middle of vegetative growth is an optimal strategy for the remediation of Cd-contaminated soil.

<p><b>Background</b>The influence of different right ventricular lead locations on ventricular arrhythmias (VTA) in patients with a cardiac resynchronization therapy (CRT) is not clear. This study aimed to evaluate the influence on VTA in patients with a CRT when right ventricular lead was positioned at the right ventricular middle septum (RVMS) and the right ventricular apical (RVA).</p><p><b>Methods</b>A total of 352 patients implanted with a CRT-defibrillator (CRT-D) between May 2012 and July 2016 in the Department of Cardiology of Anhui Provincial Hospital were included. Two-year clinical and pacemaker follow-up data were collected to evaluate the influence of the right ventricular lead location on VTA. Patients were divided into the RVMS group (n = 155) and the RVA group (n = 197) based on the right ventricular lead position. The VTA were compared between these two groups using a Kaplan-Meier curve and Cox multivariate analysis.</p><p><b>Results</b>When the left ventricular lead location was not considered, RVMS and RVA locations did not affect VTA. However, the subgroup analysis results showed that when the left ventricular lead was positioned at the anterolateral cardiac vein (ALCV), the RVMS group had an increased risk of ventricular arrhythmias and appropriate defibrillation (hazard ratio [HR] = 3.29, P = 0.01 and HR = 4.33, P < 0.01, respectively); when the left ventricular lead was at the posterolateral cardiac vein (PLCV), these risks in the RVMS group decreased (HR = 0.45, P = 0.02 and HR = 0.33, P < 0.01, respectively), and when the left ventricular lead was at the lateral cardiac vein, there was no difference between the two groups. In regard to inappropriate defibrillation, there was no significant difference among all these groups.</p><p><b>Conclusions</b>When the left ventricular lead was positioned at ALCV or PLCV, the right ventricular lead location was associated with VTA and appropriate defibrillation after CRT. Greater distances between leads not only improved cardiac function but also may reduce the risk of VTA.</p>
