RESUMO

We present a rare case of a common carotid artery wall hematoma after ultrasound-guided parathyroid fine-needle aspiration. The hematoma was detected within seconds from needle insertion, extending along the common carotid artery wall from the lower neck to the common carotid artery bifurcation, and it was quickly restricted with firm pressure. The patient only reported mild discomfort. A follow-up assessment by a vascular surgeon two hours later showed no signs of ischemic events and complete absorption of the hematoma without further clinical consequences. Physicians performing parathyroid fine-needle aspiration must be aware of this rare complication, which may be easily overlooked. Continuous meticulous sonographic surveillance during the fine needle aspiration procedure is the only way to directly diagnose this complication and apply immediate pressure to restrict it.

OBJECTIVE: Descending goiter has been a focus of controversy in thyroid surgery until nowadays. This study aims to investigate the diagnosis and treatment options of thyroid goiters extending into the mediastinum and the thoracic cavity. METHODS AND MATERIALS: A retrospective study was conducted assessing all cases of substernal goiter managed in a tertiary care referral center within 22 years. Demographics, clinical, operative, anatomical, and pathological data of the patients were recorded and analyzed. RESULTS: Among 3.028 total thyroidectomies, 212 procedures for substernal goiters were studied. All cases underwent total thyroidectomy. The surgical approach was cervical in all but two cases. A very low rate of complications and zero mortality were noted. Incidence of malignancy on permanent histology was 16%. CONCLUSION: Descending goiter constitutes a major indication for thyroid surgery. The overwhelming majority of descending goiters may be managed surgically through a neck incision. In experienced hands good results with low morbidity should be expected. Such cases should be considered as challenging, however, and therefore management in a referral center may be necessary in order to ensure optimal results.

Tissue angiogenesis is a complex phenomenon that results in the growth of new blood vessels from the microcirculation. This process has been known to play a crucial role in tumor growth as well as several benign diseases. The aim of this study was to assess mRNA expression of various angiogenic factors and chemokines in nasal polyps and compare the results to normal nasal mucosa. mRNA expression was measured using real-time RT-PCR for the following angiogenic factors and chemokines: VEGF, VEGFR-1, Ang-1, Ang-2, Tie-2A, Tie-2B, SDF-1α, SDF-1ß, CXCR4 and YY1. Biopsy specimens from nasal polyps in the polyposis group and middle turbinates in the control group were studied. A total of 18 nasal polyposis patients were studied and compared to 10 control subjects. Results showed VEGF, VEGFR-1, Ang-1, Ang-2, Tie-2A, Tie-2B, SDF-1α and SDF-1ß mRNA expression to be significantly higher in nasal polyposis patients compared to the control group (p<0.05). The findings of this study support the role of angiogenic growth factors in the pathogenesis of nasal polyposis. Further studies are required to confirm these results and evaluate potential clinical implications.

OBJECTIVE: To address the long-term results of our method that combines intranasal laser-assisted dacryocystorhinostomy with the use of surgical microscope. METHODS: The procedure is a modification of the West method. A retrospective series of the long-term results are presented in 105 patients that underwent 118 DCR procedures, over the past 6 years. Selection criteria were mainly acquired idiopathic nasolacrimal duct obstruction. RESULTS: 113 DCR cases (95.8%) were free of symptoms postoperatively (mean follow-up: 24 months). CONCLUSION: Our method when compared with traditional methods is correlated with excellent postoperative long-term results.

BACKGROUND: Instability of the Microsatellite DNA Instability (MSI) and Loss of Heterozygosity (LOH) have been previously detected in sputum cells of COPD patients. However, the particular cell subpopulation exhibiting genetic instability in COPD was uncertain. The aim of this study was to determine which cell type expresses Microsatellite DNA Instability in sputum and BALF samples from COPD patients. METHODS: Thirty-five COPD patients and 30 non-COPD smokers were studied. Sputum was induced from 20 COPD patients and 20 non-COPD smokers and BALF was obtained from 15 COPD patients and 10 non-COPD smokers. The sputum cell pellet and BALF samples were processed using immunomagnetic technology to separate antibody-specific cell subpopulations, using CD45+ for leukocytes, Epithelial enrich (MACS) for sputum epithelial cells and HEA-human epithelial antigen-(Dynal) for BAL epithelial cells. Microsatellite DNA amplification was performed using specific primers, namely G29802, D6S2223, D6S344, D6S263, D5S207, D13S71, RH70958, and D17S250. The presence of MSI and/or LOH was analyzed with LI-COR Saga GT Microsatellite Analysis Software. MEASUREMENTS AND MAIN RESULTS: None of the non-COPD smokers exhibited any genetic alteration. MSI and LOH were found in 15 cases (8 MSI and 7 LOH) in sputum and BAL samples. MSI and/or LOH were revealed only in the epithelial barrier cells. LOH was detected in D5S207, D6S344, G29802 and D17S250 microsatellite markers, while MSI in D13S71, D5S207 and D6S344. The entire leukocyte subpopulation exhibited no genetic alteration. CONCLUSIONS: Our results support the hypothesis that chronic inflammation and oxidative burden in COPD can lead to DNA damage of the lung epithelial barrier cells, detected at the Microsatellite DNA level. Further studies are required to investigate the significance of these findings in the pathogenesis of COPD.

OBJECTIVE: The objective of the study was to compare and assess parameters related to thermal welding tonsillectomy (TWT) vs ligasure tonsillectomy (LT). STUDY DESIGN: This was a prospective randomized study. METHOD: A prospective study was conducted on 143 consecutive adult patients undergoing tonsillectomy. Indications included chronic tonsillitis and obstructive sleep apnea syndrome. Patients undergoing adenoidectomy or any procedure together with tonsillectomy and patients with peritonsillar abscess history or bleeding disorders were excluded. Patients were randomly assigned to either the TWT or LT group. Intraoperative bleeding, operative time, postoperative pain using a visual analogue scale, and complication rates were evaluated. RESULTS: There was no measurable intraoperative bleeding in any group. No significant difference regarding mean operative time was noted. Mean postoperative pain score was significantly lower in the TWT group. Late postoperative hemorrhage was noted in 1 subject of the TWT and 2 subjects of the LT group. Mild uvula edema was significantly lower in the TWT group. CONCLUSION: Both TWT and LT procedures provide sufficient hemostasis and reduced operative time, although lower postoperative pain and mild uvula edema were noted in TWT procedures. Furthermore, since both methods use single-use instruments, they provide safety against diseases such as Creutzfeld-Jakob disease.

BACKGROUND: Tetanus (lockjaw) is a rare disease in developed countries. METHODS: An otorhinolaryngologist can be the first physician to see a patient with tetanus. In our department, we have seen 10 such cases presenting with head and neck symptoms. We report 2 of these cases followed by an investigation of the immune status of the adult Cretan population against tetanus using 200 random patients who visited our clinic. RESULTS: A negative history of complete childhood immunization was recorded in 37% of individuals. Of the male population, 88.5% had received active immunization during adulthood, although 47% had received all three doses. Only 12.6% of the females had received full-dose immunization during adulthood. Overall, only 8.5% of the studied population had received either complete immunization or a scheduled booster dose during the last decade. CONCLUSIONS: In many cases, an otorhinolaryngologist can be the first physician to examine a patient with tetanus. The incidence of the disease in Crete, Greece, is significantly higher than in most parts of developed countries.

The objective of this work was to evaluate the diagnosis and management of patients with substernal goiter (SSG) on the basis of our experience. We conducted a retrospective study of all SSGs within a series of 591 thyroidectomies performed in a tertiary referral center over a period of 14 years, analyzing epidemiological data, diagnostic criteria, and surgical results. There were 37 (6%) patients with descending goiter: 28 women (mean age 57.1 years) and 9 men (mean age 61 years). All 37 patients underwent successful surgical treatment without any major postoperative complications. A postoperative histological examination revealed a 16.6% incidence of malignancy. Despite the size and close proximity to vital organs in the mediastinum, all of the SSGs were managed successfully. A neck approach was used in all except for one patient who was operated on via sternotomy. A thorough preoperative evaluation including computed tomography scan of the neck and mediastinum, and an appropriate surgical technique ensure a positive outcome for most patients with an SSG.

Dysphagia is a symptom associated with various diseases of the upper gastrointestinal and respiratory tract, and it may be the presenting symptom of numerous tumors of the head and neck. Plexiform neurofibromas (PNFs) are benign tumors of the peripheral nerves and connective tissue, which are usually associated with neurofibromatosis type 1. We present a rare case of a 52-year-old woman presenting with dysphagia and weight loss due to a massive PNF in the subcutaneous adipose tissue of the posterior neck, associated with C1-C2 dislocation, scoliosis of the vertebral column, and 2 meningoceles consistent with the diagnosis of neurofibromatosis type 1. The combination of large PNF and cervical spine dysplasia may cause compression of the upper gastrointestinal tract and chronic progressive dysphagia.

OBJECTIVE: To introduce and assess a new method of tonsillectomy with thermal welding technology (TWT) using the thermal ligating shear (TLS(2)), in pediatric population. METHOD: TWT provides a new surgical instrument that combines heat and pressure to simultaneously coagulate and divide tissue. Among the headpieces available for the TWT generator, the TLS(2) handpiece was selected. A prospective study was conducted in our Department, on 60 children who underwent tonsillectomy with the use of TWT. Inclusion criteria were obstructive sleep apnea syndrome, peritonsilar abscess history and chronic tonsillitis. Patients undergoing adenoidectomy, or any other procedure together with tonsillectomy were excluded from this study. All patients' data, including intraoperative blood loss, operation time and complication rates were recorded in a database. RESULTS: Our series consisted of 60 patients (39 male and 21 female). There was no measurable bleeding during surgery in any of the cases. The mean operative time was 20 min. Only one case of postoperative hemorrhage occurred. Mild uvula edema was noticed in 25 patients. CONCLUSION: TWT is a safe method for tonsillectomy. Among the several handpieces available for the TWT generator, the TLS(2) is very effective and easy to use in tonsillectomy procedures, providing sufficient heamostasis and diminished operative time.

Malignant myoepithelioma is a rare salivary gland neoplasm that can arise either de novo or within a pre-existing pleomorphic adenoma. We report a case of malignant myoepithelioma arising from a pleomorphic adenoma of the soft palate. A 70-year-old woman presented in our department with a very large tumor of the soft palate. The patient had a history of a pleomorphic adenoma at the same location that was surgically removed 2 years ago. A second operation, with intraoral excision of the tumor was performed. Histological examination of the recurrence revealed a malignant spindle cell neoplasm with an infiltrative growth pattern and a high mitotic rate. Immunohistochemical investigation confirmed the diagnosis of a low-grade carcinoma of myoepithelial origin with free surgical margins. The patient remains free of disease for more than twelve months after the end of treatment.

RESUMO

We report the case of a 57-year-old man complaining of headaches and adult onset seizures. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a process which proved to be a huge frontal sinus mucocele, eroding the posterior wall of the frontal sinus and compressing the left frontal lobe. An osteoplastic flap procedure with cranialization and obliteration of the frontal sinus was performed. The seizures and headache disappeared postoperatively. Clinical manifestations, diagnosis and surgical approaches for grand frontal sinus mucoceles are briefly discussed.

The use of angiotensin-converting enzyme inhibitors (ACEIs) has been implicated in many cases of angioedema, but, given the potential mechanism of this complication, it was not expected to be caused by angiotensin II receptor blockers (ARBs). However, in the past few years, scattered reports of angioedema associated with ARBs have appeared in the medical literature. We performed a retrospective chart review from January 1, 1998, through June 30, 2003, and a review of the literature. During this time, we managed head and neck angioedema induced by ACEIs (n = 27) and ARBs (n = 4) in 31 patients. All of them had significant mucosal swelling, and in some of them dyspnea and dysphagia coexisted. The most frequently involved areas were the oral tongue (13 cases), uvula and soft palate (5 cases), and larynx, mouth floor, and lips (3 cases each). Angioedema may be a more common complication of ACEI and/or ARB use than originally thought. This complication may occur after long-term use of these drugs. We advise that ARBs not be prescribed to patients with a history of angioedema, particularly that due to the use of ACEIs.
