RESUMO

Structural characteristics and Na, K-ATPase activity of erythrocyte membranes were studied by the spin probe method in patients with an angiographically verified damage to the coronary arteries. The rise of the cholesterol/phospholipid ratio in patients with coronary heart disease was associated with the increased orderliness of the fatty acid chains of erythrocyte membrane lipids, leading to inhibition of Na, K-ATPase activity. The data point to the same line of changes in erythrocyte membranes and smooth muscle cells during the development of coronary heart disease.

Activity of Na+, K+-ATPase and the molar ratio "cholesterol/phospholipids" were estimated in erythrocyte membranes of patients with ischemic heart disease, in which an impairment of coronary arteries was documented by means of angiography. Distinct inhibition of the enzymatic activity as well as an increase in the ratio "cholesterol/phospholipids" was observed in erythrocyte membranes of all the patients; the alteration of these parameters was maximal in the patients with IIa and IIb forms of hyperlipoproteinemia. Reverse correlation (r = -0.604) was found between the enzymatic activity in erythrocyte membranes and the "atherogeneity coefficient" in blood plasma lipoproteins. The data obtained suggest that blood plasma lipoproteins are responsible for regulation of cholesterol content in cell membranes and, according to the membrane hypothesis of atherogenesis, this phenomenon is important in development of atherosclerosis.

Secretions of hormones responsible for carbohydrate metabolism, STH, insulin, S-peptide and somatostatin, were measured in coronary patients in order to investigate hormonal mechanisms involved in disordered carbohydrate metabolism. A persistent hyperinsulinemia was found in patients with disordered carbohydrate metabolism, both in cases of glucose stimulation and insulin inhibition, where glandular function was assessed on the basis of S-peptide concentration as opposed to the control level. Baseline somatostatin concentrations were beyond the method's sensitivity limit in most of the patients. There was a tendency to elevated hormonal levels in the insulin test, whereas glucose administration produced an opposite response. Blood basal STH levels did not differ significantly in the study groups, however the increase in hormonal secretion following insulin administration was less pronounced in patients with disordered carbohydrate metabolism as compared to other groups.

In patients with coronary heart disease and in healthy individuals, the characteristics of platelet aggregation were compared to those of circulating immune complexes. Platelet aggregation and the level of immune complexes were found to be significantly increased as compared with healthy donors. No correlation was revealed between platelet aggregation parameters and the concentration of immune complexes in the serum treated with 3.5% polyethylene glycol. A significant reverse correlation was revealed upon comparing the results of platelet aggregation with the level of small immune complexes detectable in 7% polyethylene glycol. A mechanism is suggested by which platelet aggregation is suppressed by small immune complexes.

The activity of some blood hormones was determined by radioimmune analysis in 58 patients with chronic ischemic heart disease (CIHD), 60 patients with myocardial infarction (MI) and in 24 practically healthy individuals. Increased activity of pituitary hormones (ACTH and TTH) with simultaneous increase in the blood STH content in the absence of essential changes in the FSH and LH content was established. These shifts in the production of pituitary hormones are evidently due both to disorders in the hypothalamo-hypophyseal area and to changes in the activity of systems coordinated to the pituitary gland in patients with CIHD. In patients with MI the activity of these hormones in blood does not differ from that in the control group. In patients with CIHD, the activity of the thyroid is diminished due to a decrease in its thyroxin-producing ability against the background of high activity of the pituitary thyreotropic function. The discussed shifts in the activity of some hormones in CIND and MI are conducive to the advancement of atherosclerosis and changes in myocardial metabolism.

It was established that the content of primary (acylhydroperoxide) and secondary (intermollecular "seams" in aminophospholipids) products of lipid peroxide oxidation in blood of patients with ischemic heart disease is increased against the background of hyperlipidemia and hypercholesterolemia. It is suggested that intensification of lipid peroxide oxidation may play a role in the pathogenesis of atherosclerosis.

RESUMO

Clinical-functional analysis of the efficacy of propranolol was conducted in 32 patients with ischemic heart disease and stable angina pectoris (with angiographically verified stenosing coronary atherosclerosis) depending on the initial myocardial contractility. A marked antianginal effect of propranolol in a dose of up to 160 mg daily was demonstrated irrespective of the initial myocardial contractile function. Treatment with propranolol was not attended with signs of cardiac insufficiency or aggravation of its subclinical symptoms. The results of the study provide convincing data that optimum doses of blocking agents of beta-adrenergic receptors of the myocardium causing a negative inotropic effect even in patients with diminished myocardial function may be used more widely.

It was established that in blood of patients with ischemic heart disease due to atherosclerosis of the coronary arteries (the diagnosis was verified in selective coronaroangiography) the content of acylhydroperoxides grows while the activity of glutathione-peroxidase II decreases. In blood of patients with no damage to the coronary vessels (according to the results of angiography), glutathione-peroxidase II activity does not differ significantly from the values in the control group. The decrease of glutathione-peroxidase II activity was most marked in patients with ischemic heart disease and hypercholesterolemia. It is suggested that the increase in the peroxide content in blood of patients with ischemic heart disease may be due to the sharp decrease in the activity of glutathione-peroxidase II.

The activity of the pituitary hormones (ACTH, STH, TTH, FSH, LH), the adrenal hormones (cortisol, aldosterone), the kidney hormone (renin), and the thyroid hormones (thyroxine tri-iodthyronine), the thyroxine binding capacity of blood proteins and the activity of the hormones of the pancreas (insulin) and the sex glands (testosterone, estradiol) were studied in 26 males suffering from ischemic heart disease verified by means of selective coronarography and in 20 healthy males with no atherosclerosis of the coronary arteries of the heart. Patients with ischemic heart disease were found to be marked by increased activity in the blood of ACTH, TTH, cortisol, aldosterone, insulin, and estradiol and reduced concentration of STH, thyroxine, and testosterone. These shifts in the activity of the hypothalamo-hypophyseal system and in its subordinate hormonal systems play an important role in the origin of the atherosclerotic process and assosiated ischemic heart disease.

RESUMO

A distinct increase in the aggregative property of blood platelets, displayed by an increased degree of aggregation and enlargement of the platelet aggregates, was revealed in patients with ischemic disease of the heart. The more severe the atherosclerotic process in the coronary arteries (according to the findings of selective coronarography), the more is the aggregative property of platelets intensified. The stimulating effect of smoking on platelet aggregation was also established.
