RESUMO

OBJECTIVE: The aim of the study is the increasing the efficiency and improving the quality of dental surgical care for patients with tumor diseases of the blood system. MATERIAL AND METHODS: For the period from 2020 to 2022 The authors examined and treated 15 patients with tumor diseases of the blood system who were hospitalized at the National Medical Research Center for Hematology of the Ministry of Health of the Russian Federation. Of these, 11 provided dental surgical benefit. There were 5 men (33%), women - 10 (67%). The mean age of the patients was 52 years. A total of 12 surgical operations were performed: biopsies - 5; opening of the infiltrate - 3; imposition of secondary sutures - 1; bougienage of the salivary gland ducts - 1, removal of the salivary gland -1 and amputation of the tooth root - 1. Four patients underwent conservative treatment. RESULTS: The use of methods of local hemostasis made it possible to minimize the number of hemorrhagic complications. Thus, the development of external bleeding from the postoperative wound was observed in 1 (20%) (out of 5) patients in the group of acute leukemia. Hematoma was diagnosed in 2 patients. The sutures were removed on the 12th day. Finally, the wounds were epithelialized by an average of 17 days. CONCLUSION: The authors believe that the most common surgical intervention in patients with tumorous blood diseases is a biopsy, which should be performed with partial resection of the tissue surrounding the tumor. In the course of dental interventions, hematological patients may develop complications caused by suppression of immunity and fatal bleeding.

RESUMO

INTRODUCTION: The goal of our study was to create a cross-cultural adaptation of the questionnaire « National Eye Institute Visual Function Questionnaire ¼ (NEI-VFQ-25) for the Tunisian population (Tu-VFQ-25) and to test its psychometric properties in a group of patients with chronic uveitis. PATIENTS AND METHODS: The study consisted of two steps. The first was translation of the NEI-VFQ-25 and its cross-cultural adaptation to the Tunisian dialect (Tu-VFQ-25) according to the guidelines of Beaton et al. The second step consisted of studying the psychometric properties of the Tu-VFQ-25. Enrolled patients, diagnosed with chronic uveitis, were interviewed face-to-face by two different investigators, followed by a telephone interview by the first investigator 48hours later. Feasibility, internal consistency and reproducibility of the Tu-VFQ-25 were then tested using statistical methods. RESULTS: The mean time taken to administrate the questionnaire to each of the 45 patients was 11min 40 s (± 2min 24 s). Internal consistency was acceptable, with Cronbach's alpha coefficients above 0.7 except the "Ocular Pain" and "Driving" subscales (0.645 and 0.559 respectively). Inter-observer reliability was excellent, with intra-class correlation coefficients (ICC) ranging from 0.857 to 0.985 for all subscales. Test-retest reliability, assessed for 43 patients, was good to excellent, with ICC above 0.80 for all subscales except "General Vision," which had a value of 0.786. CONCLUSION: As with previous versions, the Tu-VFQ-25 is an easy and reliable tool for assessing vision-related quality of life of Tunisian patients with chronic uveitis.

RESUMO

Behçet's disease is a chronic multisystem disorder characterized by relapsing inflammation for which the underlying histopathology is an occlusive vasculitis. The disease has a high prevalence in the Mediterranean region, Far and Middle Eastern countries. It is clinically characterized by oral and genital ulcerations, ocular inflammatory involvement, skin lesions, vascular involvement and numerous other systemic manifestations. Uveitis is by far the most common ocular manifestation of Behçet's disease. It typically presents in the form of relapsing-remitting panuveitis with retinal vasculitis that may result in severe visual loss. Management of Behçet's uveitis relies on corticosteroid therapy, conventional immunosuppressive drugs and biological agents and requires a close collaboration between ophthalmologist and internist. A better understanding of the auto-inflammatory process and of the role of cytokines implicated in the pathogenesis of Behçet's disease will enable researchers to develop more specific and effective therapy.

From 1983 to 1989, 50 patients have been treated by spleen conservation. Traumatism was iatrogenic in 9 cases, caused by public highway accidents in 31 cases, domestic accident in 9 cases and penetrating trauma in one case. Spleen conservation includes: 26 partial splenectomies, 11 cauterisations, 10 sutures, 5 auto-transplantations, and 3 abstensions after laparotomy and little spleen trauma. Three deaths were observed secondary to cerebral trauma, and two complications have been seen: one during the operative solved by total splenectomy, and one rebleeding after splenorraphy, reoperated and splenectomy was performed with good evolution. The follow-up of partial splenectomy showed that splenic remnant keeps an epuration activity.

RESUMO

Analysis of the operative reports of hospitals in the central area of Tunisia allowed to define the epidemiology of echinococcosis in children and adolescents. Four hundred forty-three patients under 20 years of age were operated on between 1980 and 1985. In children less than 10 years of age, boys are more often concerned than girls, whereas prevalence in girls occurs after 10 years of age. Liver involvement is more frequent in girls, whatever their ages. Prevention programs should consider the high frequency in young children.
