RESUMO

Ultrasound (US)-guided suprascapular nerve block (SSNB) is a widely used procedure and while describing the US-guided SSNB in the suprascapular notch, the suprascapular fossa is often visualized and injection is performed in that location. Although it can be done in both location, to inject the right area, the terminology should be settled and the visualization of these areas which are unclear and confusing in the literature should be clarified. In this sense, we showed the course of the nerve on a cadaver and briefly describe a protocol to correctly visualize the suprascapular notch with US.

Endometriosis is a common gynecological hurting disorder in which tissue is similar to the tissue that normally lines the inner layer of the uterus. It often causes fertility problems. Unfortunately, effective treatments are limited. Therefore it's important to explore an imperative and easily accessible treatment to alleviate the probable pathologies and preserve fertility in endometriosis. Consequently, we aimed to investigate the effects of metformin, letrozole, and atorvastatin on inflammation and apoptosis in experimentally induced ovarian and peritoneal endometriosis in rat models. In the present study, 35 rats were randomly divided into five groups. Group 1: sham-operated control group. Group 2: untreated endometriosis group. Group 3: given 100 mg/kg/day of oral metformin. Group 4: given 0.1 mg/kg/day of oral letrozole. Group 5: given 2.5 mg/kg/day of oral atorvastatin. At the end of the 28 days, we examined Ki67, Bax and Bcl-2 immunoexpressions in ovarian and peritoneal tissues, and IL-6, IL-8, and TNF-α levels were evaluated from the peritoneal fluid. All medical treatment groups showed a significant decrease in Ki67 expression. A significant increase in Bax expression was also observed in all samples from all medical treatment groups (other than the untreated endometriosis groups). Further, a significant decrease in Bcl-2 expression was found in all medical treatment groups. IL-6, IL-8, and TNF-α levels were significantly lower in all medical treatment groups than in the endometriosis groups. In conclusion; Metformin, letrozole, and atorvastatin showed apoptosis induction and anti-inflammatory effects on both ovarian and peritoneal endometriosis in experimental models.

PURPOSE: Previous studies have demonstrated neuronal dis-integrity in chronic smokers using diffusion tensor imaging (DTI). However, assessment of hippocampal DTI has not been performed in this group. The purpose of this study was to investigate the hippocampal integrity in chronic smokers and non-smokers and to compare this to memory performance. METHODS: We used in vivo DTI to measure the differences in hippocampal integrity between 88 chronic smoker and 88 non-smoking subjects. DTI was performed on a 3T scanner. We administered a verbal learning test to assess new verbal learning capabilities. The immediate recall (IR) was administered immediately after test procedure and delayed recall (DR) after 15min. RESULTS: Mean values of fractional anisotropy (FA) for non-smokers and smokers were 0.46 and 0.40, respectively (p<0.05). Mean IR word number for non-smokers and smokers was 11.6, 9.04, respectively (p<0.05). The mean word number of DR for non-smokers and smokers was 10.2, 7.2, respectively (p<0.05). CONCLUSIONS: This is the first study of hippocampal DTI assessment in the chronic smokers. We found that decreased hippocampal FA associated with neuronal dis-integrity and worse memory performance in chronic smokers than non-smokers. We also found a low positive correlation hippocampal FA values with memory performance in nonsmoking group.

BACKGROUND: Hydrocephalus is a serious complication of subarachnoid hemorrhage (SAH). Obstruction of the cerebral aqueduct may cause hydrocephalus after SAH. Although various etiologic theories have been put forward, choroidal artery vasospasm-related ependymal desquamation and subependymal basal membrane rupture as mechanisms of aqueductal stenosis have not been suggested in the literature. METHODS: This study was conducted on 26 hybrid rabbits. Five rabbits were placed in a control group, 5 were placed in a sham group, and the remaining rabbits (n = 16) were placed in the SAH group. In the first 2 weeks, 5 animals in the SAH group died. The other 21 animals were decapitated after the 4-week follow-up period. Choroidal artery changes resulting from vasospasm, aqueduct volume, ependymal cell density, and Evans index values of brain ventricles were obtained and compared statistically. RESULTS: Mean aqueduct volume was 1.137 mm(3) ± 0.096, normal ependymal cell density was 4560/mm(2) ± 745, and Evans index was 0.32 ± 0.05 in control animals (n = 5); these values were 1.247 mm(3) ± 0.112, 3568/mm(2) ± 612, and 0.34 ± 0.15 in sham animals (n = 5); 1.676 mm(3) ± 0.123, 2923/mm(2) ± 591, and 0.43 ± 0.09 in animals without aqueductal stenosis (n = 5); and 0.650 mm(3) ± 0.011, 1234/mm(2) ± 498, and 0.60 ± 0.18 in animals with severe aqueductal stenosis (n = 6). The choroidal vasospasm index values were 1.160 ± 0.040 in the control group, 1.150 ± 0.175 in the sham group, 1.760 ± 0.125 in the nonstenotic group, and 2.262 ± 0.160 in the stenotic group. Aqueduct volumes, ependymal cell densities, Evans index, and choroidal artery vasospasm index values were statistically significantly different between groups (P < 0.05). CONCLUSIONS: Ependymal cell desquamation and subependymal basal membrane destruction related to choroidal artery vasospasm may lead to aqueductal stenosis and hydrocephalus after SAH.

The aim of this study was to define the different courses and percentages of hepatic artery that were detected during preoperative evaluation of living liver donors by multidetector computed tomographic angiography (MDCTA). We evaluated 150 donors before hepatic transplantation. All of the donors were evaluated by multislice CT scan with 256 detectors. For each patient, arterial, portal and venous phase images were obtained. The hepatic arterial variations were evaluated by the same radiologist according to Michels' classification. Common hepatic arterial anatomy (type I) was observed in 95 donors (63.3%). Other arterial variations were determined in the remaining 55 donors (36.6%). The second common variation was type XI which did not match with the description of Michels' classification variation in 15 donors (10%). The remaining variations described in Michels' classification were seen at lower rates. Type VII or X variation was not seen. MDCTA is a useful method to identify the blood supply of the liver before the liver transplantations, and surgeons can make their plan on the basis of CT data.

PURPOSE: We evaluated the histomorphological properties of the coronary arteries, saphenous vein, and five arterial conduits for coronary artery bypass grafting, and compared them with each other. METHODS: All vessels were harvested from seven cadavers, eight autopsied individuals, and 14 patients. The length, lumen diameter, wall thickness, and histological structures were examined based on samples obtained from the coronary arteries and the conduits. The dimensions and wall thicknesses of the coronary arteries and the conduits were compared using a one-sided variance analysis. The similarities between the coronary arteries and the conduits were investigated by Duncan's multiple range tests. RESULTS: The internal mammary and intercostal arteries were elastic, while the others were muscular. The lengths of all conduits were adequate for use as coronary artery grafts. The dimensions and wall thicknesses of the coronary arteries and the conduits showed no statistically significant differences, except for the proximal portion of the saphenous vein. CONCLUSIONS: These findings emphasize the justification of continued use of the ideally matching internal mammary artery, either as an in situ or free graft, in coronary artery bypass grafting, although other arterial grafts can be used in coronary artery bypass grafting. In addition, the intercostal artery was found to have relatively favorable properties regarding its potential suitability as a coronary bypass conduit.
