RESUMO

This study investigated if kaempferol could attenuate the oxidative, inflammatory, and fibrotic damage of the left ventricles (LVs) in streptozotocin (STZ)-diabetic rats by modulating silent mating type information regulation 2 homolog 1 (SIRT1) signaling. Adult male rats were divided into 5 groups (n = 12/each) as control, control + kaempferol, STZ-induced diabetes mellitus (STZ-DM), STZ-DM + kaempferol, and STZ-DM + kaempferol + EX-527, a sirtuin 1 (SIRT1) inhibitor. Administration of kaempferol to diabetic rats significantly preserved the systolic and diastolic functions of the LVs that was associated with a significant reduction in ventricular collagen deposition, infiltration of inflammatory cells, and protein expression of Bcl2-associated X protein (Bax), cleaved caspase-3, and cytochrome-C. In both the control and diabetic rats, kaempferol attenuated the loss in body weights, reduced fasting glucose levels, and increased fasting insulin levels and HOMA-ß. Besides, kaempferol lowered the levels of reactive oxygen species (ROS), malondialdehyde (MDA), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6), downregulated the transforming growth factor-ß1 (TGF-ß1) and reduced the nuclear levels of NF-κB p65. In concomitant, kaempferol increased the LV levels of manganese superoxide dismutase (MnSOD) and glutathione (GSH) and stimulated the total protein levels of Bcl2, the nuclear activity of SIRT1, and nuclear levels of nuclear factor erythroid 2-related factor 2 (Nrf2). These events were associated with increased deacetylase activity and total levels of SIRT1 and a parallel decrease in the acetylation of Nrf2, NF-κB, smad2, and FOXO1. In conclusion: kaempferol attenuate diabetic cardiomyopathy in STZ-treated rats through its hypoglycaemic and insulin-releasing effects, as well as a cardiac independent mechanism that involves activation of SIRT1.

OBJECTIVES: To investigate the prevalence, imaging features, and predictors (age and gender) of upper cervical spine degenerative joint disease (DJD). METHODS: Two oral radiologists retrospectively reviewed the cone-beam computed tomography (CBCT) examinations conducted at a university-based imaging center over 1 year. They recorded the imaging findings related to the upper cervical spine (C1C2) and collected patient demographic data. Then the data of the patients with and without DJD findings in the upper cervical spine were compared. RESULTS: The prevalence of upper cervical DJD was 45.6%. There were no gender-based differences in upper cervical DJD. The prevalence of upper cervical DJD increased steadily with age. There was a statistically significantly difference in the prevalence of upper cervical DJD at all the age ranges. CONCLUSIONS: DJD of the upper cervical spine is a relatively common incidental finding frequently captured in CBCT scans acquired for dental purposes.

We report on the key influence of mesoscopic super-open-eye core-shell spheroids of TiO2- and LiFePO4-wrapped nanocarbon carved anode/cathode electrodes with uniform interior accommodation/storage pockets for the creation of fully reversible and dynamic Li-ion power battery (LIB) models. The mesoscopic core-shell anode/cathode electrodes provide potential half- and full-cell LIB-CR2032 configuration designs, and large-scale pouch models. In these variable mesoscopic LIB models, the broad-free-access and large-open-eye like gate-in-transport surfaces featured electrodes are key factors of built-in LIBs with excellent charge/discharge capacity, energy density performances, and outstanding cycling stability. Mesoscopic open-eye spheroid full-LIB-CR2032 configuration models retain 77.8% of the 1st cycle discharge specific capacity of 168.68 mA h g-1 after multiple cycling (i.e., 1st to 2000th cycles), efficient coulombic performance of approximately 99.6% at 0.1C, and high specific energy density battery of approximately 165.66 W h kg-1 at 0.1C. Furthermore, we have built a dynamic, super-open-mesoeye pouch LIB model using dense packing sets that are technically significant to meet the tradeoff requirements and long-term driving range of electric vehicles (EVs). The full-pouch package LIB models retain a powerful gate-in-transport system for heavy loaded electron/Li+ ion storage, diffusion, and truck movement through open-ended out/in and then up/downward eye circular/curvy folds, thereby leading to substantial durability, and remarkable electrochemical performances even after long-life charge/discharge cycling.

The study examined the protective effect of exogenous administration of dehydroepiandrosterone (DHEA) against acetaminophen (APAP) -induced liver damage in rats and tested the underlying mechanism(s). Male rats were divided into 5 groups as control, control + DHEA, APAP, APAP + DHEA, and APAP + DHEA + EX-527 (SIRT1 inhibitor). Treatments were conducted for 10 days and then followed by intragastric administration of a single dose of APAP. DHEA not only reduced serum alanine transaminase (ALT) and aspartate aminotransferase (AST) but also preserved the liver structures. Besides, DHEA reduced hepatic levels of tumor necrosis factor-alpha (TNF-α), interleukin 6 (IL-6), Bax, cleaved caspase-3. In the livers of both the control and APAP-treated rats, DHEA suppressed the generation of reactive oxygen species (ROS) and malondialdehyde (MDA), increased levels of glutathione (GSH), MnSOD (SOD2), and Bcl-2 levels, lowered Bax/Bcl-2 ratio, enhanced the activity of nuclear factor erythroid-derived 2-like 2 (Nrf2), and inhibited nuclear factor kappaB (NF-κB) p65. All these effects coincided with a significant increase in the levels and activity of SIRT1 and a reduction in the acetylation of Nrf2, p53, forkhead box class O transcription factor 1 (FOXO1), and NF-κB p65. Except for Bcl-2, treating the rats with EX-527 abolished the beneficial effects of DHEA on all these markers. In conclusion, DHEA prevents APAP-induced liver damage by concomitant upregulation of Bcl-2 and SIRT1-dependent effect.

Modulation of lithium-ion battery (LIB) anodes/cathodes with three-dimensional (3D) topographical hierarchy ridges, surface interfaces, and vortices promotes the power tendency of LIBs in terms of high-energy density and power density. Large-scale meso-geodesics offer a diverse range of spatial LIB models along the geodetically shaped downward/upward curvature, leading to open-ended movement gate options, and diffusible space orientations. Along with the primary 3D super-scalable hierarchy, the formation of structural features of building block egress/ingress, curvature cargo-like sphere vehicles, irregularly located serrated cuticles with abundant V-undulated rigidness, feathery tube pipe conifers, and a band of dagger-shaped needle sticks on anode/cathode electrode surfaces provides high performance LIB modules. The geodetically-shaped anode/cathode design enables the uniqueness of all LIB module configurations in terms of powerful lithium ion (Li+) movement revolving in out-/in- and up-/downward diffusion regimes and in hovering electron density for high-speed discharge rates. The stability of built-in anode//cathode full-scale LIB-model meso-geodesics affords an outstanding long-term cycling performance. The full-cell LIB meso-geodesics offered 91.5% retention of the first discharge capacity of 165.8 mAhg-1 after 2000 cycles, Coulombic efficiency of ~99.6% at the rate of 1 C and room temperature, and high specific energy density of ≈119 Wh kg-1. This LIB meso-geodesic module configuration may align perfectly with the requirements of the energy density limit mandatory for long-term EV driving range and the scale-up commercial manufactures.

To control the power hierarchy design of lithium-ion battery (LIB) built-up sets for electric vehicles (EVs), we offer intensive theoretical and experimental sets of choice anode/cathode architectonics that can be modulated in full-scale LIB built-up models. As primary structural tectonics, heterogeneous composite superstructures of full-cell-LIB (anode//cathode) electrodes were designed in closely packed flower agave rosettes TiO2@C (FRTO@C anode) and vertical-star-tower LiFePO4@C (VST@C cathode) building blocks to regulate the electron/ion movement in the three-dimensional axes and orientation pathways. The superpower hierarchy surfaces and multi-directional orientation components may create isosurface potential electrodes with mobile electron movements, in-to-out interplay electron dominances, and electron/charge cloud distributions. This study is the first to evaluate the hotkeys of choice anode/cathode architectonics to assemble different LIB-electrode platforms with high-mobility electron/ion flows and high-performance capacity functionalities. Density functional theory calculation revealed that the FRTO@C anode and VST-(i)@C cathode architectonics are a superior choice for the configuration of full-scale LIB built-up models. The integrated FRTO@C//VST-(i)@C full-scale LIB retains a huge discharge capacity (~ 94.2%), an average Coulombic efficiency of 99.85% after 2000 cycles at 1 C, and a high energy density of 127 Wh kg-1, thereby satisfying scale-up commercial EV requirements.

Coccidiosis is one of the most dangerous diseases that affect poultry, resulting in worldwide economic losses. Plant extracts and essential oils have been used as potential alternatives for chemotherapeutics, because they don't have the negative consequence of creating tissue residue and drug resistance. Therefore, this study had been conducted to determine the efficacy of artemisinin liquid extract, cinnamon essential oil and clove essential oil against Eimeria stiedae in rabbits. Sixty New Zealand white rabbits were divided into six equal groups, where group 1 and group 2 represented the negative and the positive controls, respectively, and groups 3-6 were infected with Eimeria stiedae and received 15 ppm toltrazuril, 200 ppm artemisinin, 100 mg/kg cinnamon oil, and 100 mg/kg clove oil, respectively. The results showed that artemisinin had a significant beneficial role in protection against hepatic coccidiosis: it mitigated the clinical symptoms, reduced the mortality rates, improved body weight and feed conversion, decreased the oocyst output, prevented oxidative stress, improved biochemical parameters, and decreased the lesion formation. Moreover, it has been found that cinnamon and clove essential oils induced partial protection against hepatic coccidiosis. Our findings suggested that artemisinin liquid extract and cinnamon and clove essential oils could be used for protection against hepatic coccidiosis. However, further investigations are needed in order to elucidate the active components, optimal doses, and mode of action of these extracts and essential oils before their clinical applications.

Surgical repair of the ulnar collateral ligament (UCL) of the thumb metacarpophalangeal (MCP) joint after complete rupture is usually protected by cast immobilization, which hinders return to work. The goal of this study was to determine the duration of sick leave for patients who had 6 weeks of K-wire immobilization instead of cast immobilization after surgical repair of the UCL. This prospective, observational, non-comparative study included patients who had surgical repair of the UCL of the thumb MCP joint followed by K-wire immobilization. Manual laborers were excluded. The main outcome measure was the duration of sick leave. The other outcomes were the need to adapt to the work duties and the patient's subjective assessment of the result. The data were collected by telephone interview. Twenty-one patients were included, 13 (62%) of whom returned to work within 7 days. The average sick leave duration was 3 weeks and 2 days; 10% of patients required adaptation of the work duties. The average satisfaction score was 4.4 out of 5. The average follow-up was 3 years and 5 months. There were no complications. In conclusion, K-wire immobilization after surgical repair of the UCL of the thumb MCP joint, along with precautionary measures is an option that allows early return to work, except for manual labourers.

Subcutaneous fat necrosis is an uncommon disease that may be complicated with potentially fatal hypercalcemia or with nephrocalcinosis. We report on the case of a patient with a history of significant perinatal asphyxia, hospitalized for a urinary tract infection. Lesions of subcutaneous fat necrosis were noted, with asymptomatic hypercalcemia at 3.9mmol/L. A renal ultrasound was performed and showed echogenic medullary pyramids bilaterally, consistent with nephrocalcinosis and left nephrolithiasis. The treatment of hypercalcemia included hyperhydration, a diuretic and corticosteroids. Progression was characterized by the total regression of skin lesions and normalization of serum calcium. Hypercalcemia is a rare complication of subcutaneous fat necrosis. It develops within days to weeks after the appearance of skin lesions. Nephrocalcinosis appears after several weeks or months. Hypercalcemia must be treated in due time to avoid the impact on the kidney.

Minimal-invasive, single-port laparoscopic surgery is a recent innovation that may improve surgical outcomes. In this chapter, we review published research on single-port surgery in gynaecology, and the different surgical instruments available. Challenges, advantages, indications and potential future rules of this new approach are also discussed. Sixty-five studies were available for review: 17 case reports, 32 case studies, 13 retrospective comparative studies, and three randomised-controlled trials (RCTs). The recent availability of advanced instruments has made single-port surgery safer and more feasible for most benign gynaecologic surgeries. Single-port surgery has many potential benefits, but comparative trials have found no differences between single-port surgery and conventional laparoscopy in postoperative complications, postoperative pain, hospital stay, and cosmetic results. Single-port surgery seems to provide another option in the area of minimal invasive surgery, and further development of this technique, along with robotics and natural orifice transgastric endoscopic surgery, will improve dissemination of this approach.

Mucolipidosis Type III, or pseudo-Hurler polydystrophy, is a rare genetic abnormality, the result of a mutation to one of two genes that encode the hexameric protein N-acetylglucosaminyl-1-phosphotransferase (Glc-NAc-PT). The abnormality results in the accumulation of unprocessed macromolecules in cell and tissue compartments throughout the body. In this case report, we describe the clinical and radiographic findings of a 15-year-old male with this disorder. He presented with bilateral ectopically developing mandibular molar teeth with enlarged follicles and multiple joint involvement, including the temporomandibular joints. The patient underwent surgical removal of the molar teeth and curettage of the associated follicles. The subsequent histopathological examination of the tissues revealed hyperplastic follicles suggestive of dentigerous cysts. This report presents the plain film and cone beam CT examinations of the patient.

There is no consensus in the literature as to the maximum delay for nerve repair following nerve injury. Our aim is to estimate the maximum delay at which a nerve can be successfully reconstructed. Eighteen patients (24 procedures) were operated at more than 12 months (12 to 36) post-injury. Mean age was 38 (17 to 74). There were 18 nerve transfers, four grafts and two secondary sutures for six target muscles. Evaluation of muscle power was scored and assessed against weights starting 12 months follow-up. Finally, two patients had contraction against 7 kg, two against 5 kg, six M4. Two were scored M3 and six were M2, thus 12 good results (67%). This study showed that excellent results could be obtained for nerve repair even after 24 months delay. Further study may be needed to determine the cut-off point of delay after which nerve recovery is unlikely.

There is interest in the enrichment of poultry meat with long-chain n-3 polyunsaturated fatty acids in order to increase the consumption of these fatty acids by humans. However, there is concern that high levels of n-3 polyunsaturated fatty acids may have detrimental effects on immune function in chickens. The effect of feeding increasing levels of fish oil (FO) on immune function was investigated in broiler chickens. Three-week-old broilers were fed 1 of 4 wheat-soybean basal diets that contained 0, 30, 50, or 60 g/kg of FO until slaughter. At slaughter, samples of blood, bursa of Fabricius, spleen, and thymus were collected from each bird. A range of immune parameters, including immune tissue weight, immuno-phenotyping, phagocytosis, and cell proliferation, were assessed. The pattern of fatty acid incorporation reflected the fatty acid composition of the diet. The FO did not affect the weight of the spleen, but it did increase thymus weight when fed at 50 g/kg (P < 0.001). Fish oil also lowered bursal weights when fed at 50 or 60 g/kg (P < 0.001). There was no significant effect of FO on immune cell phenotypes in the spleen, thymus, bursa, or blood. Feeding 60 g/kg of FO significantly decreased the percentage of monocytes engaged in phagocytosis, but it increased their mean fluorescence intensity relative to that of broilers fed 50 g/kg of FO. Lymphocyte proliferation was significantly decreased after feeding broiler chickens diets rich in FO when expressed as division index or proliferation index, although there was no significant effect of FO on the percentage of divided cells. In conclusion, dietary n-3 polyunsaturated fatty acids decrease phagocytosis and lymphocyte proliferation in broiler chickens, highlighting the need for the poultry industry to consider the health status of poultry when poultry meat is being enriched with FO.

AIM: Brachial plexus surgery constitutes a long and complex procedure. The aim of our study is to assess the interest of having a double operating team throughout the duration of this surgery. PATIENTS AND METHODS: Seventeen patients with brachial plexus palsy underwent surgery operated by a double team. The operating time corresponding to each step of the procedure and the total operating time were measured for each patient. The separate values were added so as to obtain a simulation of the total duration value for a single surgeon. The operative time of this virtual group of patients (Group I) was compared to that of the real group operated by the double team (Group II). Both values were compared to assess any statistical significance. RESULTS: The mean operating time was 259 min with surgery operated by a double team and 371 min in the group I, a difference found to be statistically significant (p<0.05). Exploration and preparation of the cervical region lasted 70 min in average with the double team versus 132 min in the group I (p<0.05). No perioperative complications were noted. DISCUSSION AND CONCLUSION: Brachial plexus surgery performed by a double team allows the reduction of the operating time and thus minimizes the drawbacks associated with lengthy surgery such as perioperative bleeding and infection. Microsurgical suturing which is the crucial part of the surgery is easier when performed at the end of a shortened intervention and shared by two senior surgeons with, subsequently, less fatigue. This new organization that improves the operating conditions guarantees best results.

BACKGROUND: Repetitive transcranial magnetic stimulation (rTMS) applied over left temporoparietal cortex has been reported to have a long-term therapeutic effect on tinnitus. We compare the impact of 1 and 25 Hz rTMS delivered either contralateral or ipsilateral to symptoms in 62 patients with unilateral chronic tinnitus. MATERIAL AND METHODS: Patients were randomly assigned to one of four treatment groups: with stimulation at 1 or 25 Hz applied either ipsilateral or contralateral to symptoms. Two thousand pulses per session were given daily for 2 weeks. Changes in tinnitus handicap inventory (THI), self-rating scores of loudness, awareness, and annoyance were measured monthly for 10 months. Duration of residual inhibition (RI) and psychiatric morbidity were evaluated monthly for 3 months. RESULTS: There was a significant main effect of time (P < 0.0001) and a significant time x side interaction (P = 0.032) between groups. This was because of the fact that contralateral stimulation had a greater effect on THI than ipsilateral stimulation; it was also superior to left side stimulation (P = 0.027). Ratings of loudness improved more after contralateral rTMS (P = 0.037). Twenty patients had no remaining tinnitus after 3 months; the remainder had a significant increase in RI. Patients with the shortest history of tinnitus tended to respond better to rTMS. There was a significant correlation between changes in THI score and changes in Hamilton anxiety and depression scores. CONCLUSION: Ten daily treatments of 1 and 25 Hz rTMS contralateral to the side of tinnitus have a greater beneficial effect on symptoms than either ipsilateral or left side stimulation.
