RESUMO

In contemporary times, geopolitical risk, and natural resources prices are susceptible due to the Russian-Ukraine conflict. In the meantime, emerging economies are struggling to explore the factors that could reduce ecological challenges and enhance environmental management. This research aims to analyze several economic, environmental, political, and institutional variables to ascertain their influence on greenhouse gas emissions in China. Covering the latest period from 1990 to 2022, various time series tests, including normality, stationarity, and cointegration tests. The results confirm that the variables studied have a stable pattern over time and are connected in the long run. The non-normal distribution of variables leads to opt novel moment quantile regression, where the results are tested for robustness via parametric approaches. The empirical results asserted that economic growth, natural resource prices, and trade significantly enhance ecological challenges (emissions). However, globalization, geopolitical risk, and institutional quality significantly reduce such environmental challenges. The results are robust, and both unidirectional and bidirectional causal associations confirm the importance of these variables in environmental management. Based on the results, this study recommends engagement in environmentally-friendly trading, investment in clean and green energy, and strengthening institutional quality for the region's environmental recovery.

The Sampar Coalfield in Northeastern India is a source of plentiful coal reserves, which are burnt for energy production and industrial applications, resulting in the release of pollutants such as sulphur , arsenic, and lead, which are hazardous to the environment and public health. In this work, samples from the Sampar coalfield have been analyzed to understand the origin, distribution, and various forms of sulphur and their ability to detect toxic heavy metals. The total sulphur concentration ranged from 4.31% to 6%, with organic sulphur being the predominant form at 69.21%, followed by pyritic sulphur at 16.49% and sulphate sulphur at 14.28%. With high sulphur content, this coal indicates a marine influence in the peat-forming swamps. The samples have also been examined for petrographic and elemental analysis, which have revealed the presence of vitrinite, liptinite, inertinite, carbon, hydrogen, nitrogen, oxygen, and mineral matter. In addition, the same coal sample has also been used for electrochemical sensing-based detection of toxic heavy metals like arsenic and lead, and the findings indicate an improved efficacy. These results are expected to have significant implications in the development of effective bio-based remediation strategies in the region to mitigate the harmful effects of coal-related pollution.

RESUMO

A 45-year-old female, a new case of acute promyelocytic leukemia (APL), received induction chemotherapy containing all-trans retinoic acid (ATRA) and idarubicin. On the sixth day of therapy, she developed sinus bradycardia and differentiation syndrome (DS). Electrolytes and cardiac imaging were normal. The patient achieved normal sinus rhythm after ATRA was withheld, and DS was treated.

Apart from maintaining healthy bones, vitamin D is also required for cell differentiation, cell growth inhibition, and immune modulation. Vitamin D deficiency is common in the Indian subcontinent. Vitamin D presenting toxicity, leading to hypercalcemia, acute kidney injury, and altered sensorium is very rare. Here we present a case of a 65-year-old man who presented to emergency with persistent vomiting, altered sensorium, and acute kidney injury. The cause of which was an unchecked intake of vitamin D for non-specific musculoskeletal pain. When treated with intravenous fluid, diuretics, calcitonin, and steroids, the patient improved clinically. Therefore, for any patient presenting with persistent vomiting, altered sensorium, and hypercalcemia, with normal to low parathyroid hormone levels, a diagnosis of an overdose of vitamin D should be considered. Early treatment of this condition not only improves the symptoms but also prevents further kidney damage.

Snake bite is an acute life threating medical emergency and is included amonst neglected tropical diseases.1 Every year in India 200,000 people bitten by snakes and >25% are fatal.2In Maharashtra, Konkan area, the green pit viper (Trimeresurus gramineus) is uncommon, though this snake has become more common. Herein, we developed a synthetic strategy to functionalize Ni-S nanostructures (NS) using a facile precipitation method at moderate temperature. The surface functionality of NS is controlled by varying amount of mixed surfactants to achieve the pH-responsive selective adsorption of anionic and cationic dyes and the adsorption of ciprofloxacin (CIP) and tetracycline (TC) antibiotics. Powder XRD diffraction pattern revealed the phase of NS was changed from α-NiS to mixed phases after functionalization. The surface area of functionalized NS was significantly enhanced by ~5 times of that unfunctionalized NS as 6.6 m2g-1 to 30.3 m2g-1. The NS selectively adsorbed methyl orange (MO) at pH 4.5 and methylene blue (MB) at pH 11.5 with separation efficiency values of 94.2% and 97.9% respectively. The maximum adsorption capacity for MO, MB, TC and CIP are obtained as 1526.3, 1031.2, 1540.8 and 632.4 mg g-1, respectively. The electrostatic interaction is predominantly involved in the adsorption of dyes whereas adsorption of antibiotics changed to hydrogen bonding and metal coordination. Thermodynamics parameters indicated exothermic and spontaneous adsorption of dyes. The optimized adsorbent is easily recyclable. Thus, the developed strategy of functionalization of nanostructures unveils a practical approach towards selective and efficient adsorption of organic pollutants.

An adsorbent Ni-Co-S/CTAB nanocomposites have been synthesized at low temperature in aqueous medium using nickel acetate, cobalt acetate, thioacetamide and hexadecyltrimethyl ammonium bromide (CTAB) as reagents. The nanocomposites exhibited exceptionally high adsorption capacity towards anionic adsorbates with high selectivity. The maximum adsorption capacity of nanocomposites were 1995.02 mg g-1 for Congo red (CR), 2223.15 mg g-1 for Methyl orange (MO) anionic dyes and 790.69 mg g-1 for Cr2O72- metal anion. They exhibit negligible adsorption ability towards cationic dyes 2.33 mg g-1 for MB and 42.05 mg g-1 for RhB. The nanocomposite is able to adsorb anionic dyes from a binary mixture of cationic and anionic dyes with high separation factor. It also shows good results with synthetic effluents. The removal of adsorbates followed modified Zhu and Gu isotherm model. FTIR and Zeta-potential measurement confirmed that electrostatic interactions are predominating factor for the adsorption of anionic adsorbates followed by hydrophobic interactions between adsorbates. Moreover, ethanol is used to regenerate the adsorbent and reused up to five times with good adsorption capacities. Thus, the nanocomposite can be used as an efficient adsorbent for the removal and seperation of anionic adsorbates from binary mixtures as well as synthetic effluents.

Background: Myeloid sarcoma (MS) or granulocytic sarcoma is a rare tumor consisting of myeloid blasts with or without maturation occurring at anatomic sites other than the bone marrow. MS can involve any organ system but shows a predilection for skin, bone, and soft tissues of head and neck region. Case report: We report four cases of MS occurring at unusual sites, out of which three were de novo and one was associated with acute myeloid leukemia (AML). Conclusion: Although MS is associated with AML, it can rarely present without any existent hematologic disease. Differential diagnosis of a soft tissue mass should include MS even in the absence of leukemia. Establishment of the correct diagnosis depends on morphologic, histochemical, and immunohistochemical examination.

AIM: To determine whether addition of simvastatin could be an important pharmacological rescue therapy for carvedilol non-responders. METHODS: One hundred and two consecutive patients of cirrhosis of liver with significant portal hypertension were included. Hepatic venous pressure gradient (HVPG) was measured at the base line and after proper optimization of dose; chronic response was assessed at 3 mo. Carvedilol non-responders were given simvastatin 20 mg per day (increased to 40 mg per day at day 15). Carvedilol plus simvastatin was continued for 1 mo and hemodynamic response was again measured at 1 mo. RESULTS: A total of 102 patients with mean age of 58.3 ± 6.6 years were included. Mean baseline HVPG was 16.75 ± 2.12 mmHg and after optimization of dose and reassessment of HVPG at 3 mo, mean reduction of HVPG from baseline was 5.5 ± 1.7 mmHg and 2.8 ± 1.6 mmHg among responders and non-responders respectively (P < 0.001). Addition of simvastatin to carvedilol non-responders resulted in significant response in 16 patients (42.1%) and thus overall response with carvedilol and carvedilol plus simvastatin was seen in 78 patients (80%). Two patients were removed in chronic protocol study with carvedilol and three patients were removed in carvedilol plus simvastatin study due to side effects. CONCLUSION: Addition of simvastatin to carvedilol non-responders may prove to be an excellent rescue therapy in patients with portal hypertension.

CD20 is a B cell differentiation antigen with variable expression in B cell precursor acute lymphoblastic leukemia (BCP-ALL). The significance of CD20 expression has been evaluated in BCP-ALL with conflicting results. There is paucity of data regarding CD 20 expression in BCP-ALL in Indian patients. We retrospectively analyzed 100 patients of BCP-ALL for CD20 expression. CD20 positivity was defined as expression of CD20 to be more than or equal to 20 % in the blast population. 62 % patients expressed CD20 while 38 % patients were negative for CD20. The positivity ranged from negative to dim (35.5 % patients), moderately bright (19.3 % patients) to bright (45.2 % patients). Additional prospective studies are needed to determine the optimal use of rituximab in treatment of CD20-positive BCP-ALL.

Mucormycosis is an invasive opportunistic fungal infection associated with a high mortality rate, and normally occurs in immunocompromised patients. It can be encountered during neutropenia following chemotherapy in acute leukemia patients. The common sites involved are rhinocerebral, pulmonary, gastrointestinal and spleen. Hepatic involvement has been reported rarely. We hereby report a case of acute lymphoblastic leukemia who while on chemotherapy developed hepatic mucormycosis and was managed successfully despite its treatment being quite challenging, especially in the context of surgery in a neutropenic sick patient.

RESUMO

Castleman's disease represents an atypical lymphoproliferative disorder which is non-clonal but can turn malignant in the form of lymphoma, Kaposi's sarcoma or plasma cell dyscrasia. It has been reported in association with diseases like polyneuropathy organomegaly endocrinopathy monoclonal gammopathy skin changes syndrome and myeloma and very rarely it can be associated with cutaneous vasculitis as in our case. It relatively runs an aggressive course and has a shorter survival. No standard of care therapy is yet established for multicentric Castleman's disease accompanying myeloma. We successfully treated one such patient with bortezomib-based therapy.

RESUMO

Non-Hodgkin's lymphoma (NHL) is predominantly a disease of lymph nodes, but extranodal involvement is not very uncommon. Palatal involvement by NHL is rare. Mucormycosis is a devastating fungal infection commonly seen in immunocompromised individuals, including those with NHL, but it is affecting the same region has been reported very rarely. Simultaneous infiltration of hard palate by NHL and mucormycosis is extremely unusual. Herein we describe a patient who presented with palatal hole with histopathological examination revealing presence of lymphoma with colesional mucormycosis. The identification of mucor was vital because chemotherapy alone in the absence of antifungals would have had devastating consequences as the mortality of untreated mucormycosis is high.

RESUMO

Rosai-Dorfman disease, also known as sinus histiocytosis with massive lymphadenopathy, is a benign indolent disorder, characterized by enlarged lymph nodes filled with histiocytes. Extranodal involvement is uncommon. The disease rarely affects the nose and paranasal sinuses. We report a case that presented with a right nasal mass, extending into all the paranasal sinuses and right orbit without any accompanying lymphadenopathy. Because of the absence of lymphadenopathy it posed a diagnostic challenge until the pathology was confirmed on histopathological examination.
