Background: We determined the prevalence of antimicrobial resistance (AMR) in polymicrobial pathogens in Pakistan. Methods: A total of 70,518 clinical samples were collected aseptically and confirmation of isolates and antibiogram were performed by the VITEK 2 system. Results: Of 70,518 samples, 441 (0.62%) were polymicrobial samples, with 882 (1.2%) polymicrobial pathogens with 689 (78.1%) Gram-negative rods (GNRs), 166 (18.8%) Gram-positive cocci and 27 (3.1%) Candida albicans. Among GNRs, 28.8% were Escherichia coli and 25.9% were Klebsiella pneumoniae. Majority, 15.1% of Pseudomonas aeruginosa and K. pneumoniae were found in combination. 30.1% of isolates were ESBL producers, 9.7% carbapenem-resistant organisms, 35.5% MRSA and 6.0% VRE. 100% of E. coli were resistant to ampicillin and 98% of K. pneumoniae were resistant to piperacillin. Conclusion: A high prevalence of AMR in polymicrobial pathogens was observed.


Infections caused by one or more types of bacteria, viruses, fungi or parasites ­ known as polymicrobial infections ­ are a threat to health. These infections cause serious illness and are linked to high numbers of deaths, long hospital stays and high costs of treatment. Usually, polymicrobial infections are treated with combinations of antimicrobials. However, microbes becoming less susceptible to antimicrobials (known as antimicrobial resistance) is an increasing problem. To find out how common resistance is in Pakistan, this study tested 70,518 clinical samples. Of these, 441 tested positive for polymicrobial infections. These included Candida albicans, Gram-positive cocci and Gram-negative rods infections. Many of these were resistant to widely used antibiotics such as penicillins, cephalosporins, quinolones and fluoroquinolones. This study concluded that hospitals in Pakistan have a high prevalence of resistance and that better cleanliness practices should be put in place to combat this.

Reverse osmosis (RO) spiral wound membrane generation reached 93.5% in 2020, resulting in 14,000 tons of used RO membranes being discarded annually into landfills, which is unprecedented. The current study aims to chemically convert the end-of-life RO membrane, followed by its performance evaluation and microbial removal efficiency on three different sources of water, i.e., tap water (TW), integrated constructed wetland permeate (ICW-P), and membrane bio-rector permeate (MBR-P), respectively. This was accomplished by selecting 6 years of spent Filmtech (LC-LE-4040) thin film composite type brackish water reverse osmosis (BWRO) membrane, followed by alkaline and acidic cleaning for 2 h. Finally, the conversion was carried out by 6% sodium hypochlorite (NaOCl) with 300,000 ppm/h exposure by active system (AS) using the clean in place CIP pump at 2 bars for 10 h duration. The membrane demonstrated 67% water recovery and 1% saltwater rejection, which means RO membrane now converted into recycled RO (R-RO) or (UF) by removal of the polyamide (PA) layer. Water recovery was 67% for TW, 68% for ICW-P, and 74% for MBR-P, respectively, with the consistent saltwater rejection rate of 1% being observed, while R-RO exhibited an effective COD removal of 65.79%, 62.96%, and 67.72% in TW, ICW-P, and MBR-P, respectively. The highest turbidity removal of 96% in the ICW-P was also recorded for R-RO. For morphological properties, SEM analysis of the R-RO membrane revealed a likewise appearance as a UF membrane, while pore size is also comparable with the UF membrane. The most probable number (MPN) also showed complete removal of total coliforms after passing through the R-RO membrane. These features made the R-RO membrane an excellent choice for drinking water treatment and wastewater treatment polishing steps. This solution can help developing nations to be efficient in resource recovery and contribute to the circular economy.

The effects of mutual coupling on the scanning characteristics of a four-element linear rectangular dielectric resonator antenna array (RDRA) are investigated for different inter-element spacing in this work. In particular, the gain and half-power beam width (HPBW) of an RDRA are studied for various scan angles in the E- and H-plane configurations. It is shown that for both the E and H planes, mutual coupling has an adverse effect on the performance of both phased array configurations. The H-plane array, however, is more stable than the E-plane array in terms of a gain and beam width performance comparison. The HPBW increases and gain decreases more in the E plane than the H plane when the scan angle is increased.

The present work investigates a novel four-port, multiple-input multiple-output (MIMO), single element dielectric resonator antenna (DRA) for sub-6 GHz band. The DRA is designed and fabricated into a symmetric cross shape and fed using a coplanar waveguide (CPW) feed. A single radiator with four ports is rarely found in the literature. The -10 dB impedance bandwidth covered by the antenna is from 5.52 GHz to 6.2 GHz (11.6%) which covers fifth generation (5G) new radio (NR) bands N47 and wireless local area network (WLAN) IEEE 802.11a band. The isolation between orthogonal ports is about 15 dB while the isolation between opposite ports is 12 dB. The radiation pattern of the proposed antenna is bidirectional due to the absence of a ground plane below the DRA. The orthogonal modes excited in the DRA are [Formula: see text] and [Formula: see text] through the four symmetrical CPW feeds. The simulated and measured results of the proposed design show that MIMO characteristics are achieved by pattern diversity between the ports. Due to the perfect symmetry of the design, the proposed work could be extended to MIMO array applications as well.

Triazinoindole bearing thiadiazole derivatives (1-25) have been synthesized and characterized through different spectroscopic techniques such as 1H, 13C-NMR and HREI-MS. The purpose of the study was to investigate the anti-diabetic activity of the synthesized triazinoindole bearing thiadiazole derivatives by inhibition of α-glucosidase. All synthesized analogues showed outstanding inhibition of α-glucosidase enzyme with IC50 values ranging from 2.5 ± 0.10 to 38.10 ± 0.10 µM as compared to the standard drug acarbose (IC50 = 38.45 ± 0.80 µM). Analogue 4 (IC50 = 2.5 ± 0.10 µM) was identifies as the most potent analogue in the series with fifteen folds more active than standard acarbose. Structure activity relationship (SAR) studies suggested that α-glucosidase activities of triazinoindole bearing thiadiazole are primarily dependent upon on number and position of different substitutions present on phenyl parts. Molecular docking study were conducted of the optimized compounds (i.e., compound 4, 6, and 3 etc. using MOE default parameters), the results revealed that compound 4, 6, and 3 showed numerous key interactions with the target protein, which indicate the high potential of these compounds against the target compound. All these compounds were screened for cytotoxic activity against normal normal Vero cell line and found non-toxic.

Degradation in the polymeric insulators is caused due to the environmental stresses. The main aim of this paper is to explore the improved aging characteristics of hybrid samples by adding nano/micro silica in High Temperature Vulcanized Silicone Rubber (HTV-SiR) under long term accelerated aging conditions for 9000 hours. As HTV-SiR is unable to sustain environmental stresses for a long time, thus a long term accelerated aging behavior is an important phenomenon to be considered for field application. The aging characteristics of nano/micro filled HTV-SiR are analyzed by using techniques such as Scanning Electron Microscopy (SEM), Leakage Current (LC), Fourier Transform Infrared Microscopy (FTIR), Hydrophobicity Classification (HC), and breakdown strength for the aging time of 9000 hours. FTIR and leakage currents are measured after every cycle. All the co-filled samples revealed escalated aging characteristics as compared to the neat sample except the SN8 sample (8% nano-silica+20% micro-silica) after 9000 hours of aging. The highest loading of 6% and 8% nano-silica with 20% micro-silica do not contribute to the improved performance when compared with the neat and hybrid samples. However, from the critical experimental analysis, it is deduced that SN2 sample (2% nano-silica+20% micro-silica) is highly resistant to the long term accelerated aging conditions. SN2 has no cracks, lower loss percentages in the important FTIR absorption peaks, higher breakdown strength and superior HC after aging as compared to the unfilled and hybrid samples.

Commercial membranes typically suffer from fouling and wetting during membrane distillation (MD). In contrast, reverse osmosis (RO) can be subject to the fouling issue if applied for highly saline feed solutions containing foulants (e.g., organics, oils, and surfactants). Among the diverse treatment options, the nanomaterial-based membranes have recently gained great interest due to their advantageous properties (e.g., enhanced flux and roughness, better pore size distribution, and higher conductivity). This review focuses on recent advances in the mechanical properties, anti-fouling capabilities, salt rejection, and economic viability of metal oxide (SiO2, TiO2, and ZnO) and carbon nanomaterial (graphene oxide/carbon nanotube)-based membranes. Current challenges in applying nanomaterial-based membranes are also discussed. The study further describes the preparation methods, mechanisms, commercial applications, and economical feasibility of metal oxide- and carbon nanomaterial-based membrane technologies.

Direct-contact membrane distillation (DCMD) can be eminent solution for oily wastewater treatment if the membrane provided is slippery and tolerant to low surface tension complex solutions. This study describes preparation of an anti-oil-fouling omniphobic polyethersulfone membrane using fluorinated silica nanoparticles (F-SiO2@PES) combined with perfluorodecyl triethoxysilane and polydimethylsiloxane for application against oil-In-water (o/w) emulsions. Feed solutions consist of different concentrations of oil (hexadecane), different charge surfactants (anionic sodium dodecyl benzenesulfonate, non-ionic Tween 20, and cationic hexadecyltrimethylammonium bromide, and salt (NaCl). The hierarchical re-entrant micro structured surface of the omniphobic F-SiO2@PES membrane and functional groups are confirmed by atomic force microscopy, scanning electron microscopy, and Fourier-transform infrared spectroscopy. The anti-oil-fouling and anti-wetting performance of omniphobic F-SiO2@PES membranes are investigated using contact-angle, sliding angles, DCMD tests with multiple foulants of surfactants. Omniphobic F-SiO2@PES membrane exhibited effective anti-oil-fouling and anti-wetting performance against emulsions as no severe fouling and a conductivity rises were evident regardless of surfactant charge and the concentration of components. Flux reduction and rejection rates for the omniphobic F-SiO2@PES membranes are in a range of 5-15% (only) and >99%, respectively, for various combinations of feed solution components.

The current study describes synthesis of diindolylmethane (DIM) derivatives based-thiadiazole as a new class of urease inhibitors. Diindolylmethane is natural product alkaloid reported to use in medicinal chemistry extensively. Diindolylmethane-based-thiadiazole analogs (1-18) were synthesized and characterized by various spectroscopic techniques 1HNMR, 13C-NMR, EI-MS and evaluated for urease (jack bean urease) inhibitory potential. All compounds showed excellent to moderate inhibitory potential having IC50 value within the range of 0.50 ± 0.01 to 33.20 ± 1.20 µM compared with the standard thiourea (21.60 ± 0.70 µM). Compound 8 (IC50 = 0.50 ± 0.01 µM) was the most potent inhibitor amongst all derivatives. Structure-activity relationships have been established for all compounds. The key binding interactions of most active compounds with enzyme were confirmed through molecular docking studies.

New triazinoindole bearing thiazole/oxazole analogues (1-21) were synthesized and characterized through spectroscopic techniques such as HREI-MS, 1H and 13C NMR. The configuration of compound 2i and 2k was confirmed through NOESY. All analogues were evaluated against α-amylase inhibitory potential. Among the synthesized analogues, compound 1h, 1i, 1j, 2a and 2f having IC50 values 1.80 ±â€¯0.20, 1.90 ±â€¯0.30, 1.2 ±â€¯0.30, 1.2 ±â€¯0.01 and 1.30 ±â€¯0.20 µM respectively, showed excellent α-amylase inhibitory potential when compared with acarbose as standard (IC50 = 0.91 ±â€¯0.20 µM). All other analogues showed good to moderate inhibitory potential. Structural activity relationship (SAR) has been established and binding interactions were confirmed through docking studies.

The goal of this study was to prepare a robust anti-wetting and anti-fouling polyethersulfone (PES) membrane for the rejection of a highly saline (NaCl and CaCl2·2H2O) feed solution containing humic acid (HA) in direct contact membrane distillation (DCMD). Response surface methodology (RSM) was used to determine the optimum formulation of the used materials. The variable factors selected were polydimethyl siloxane (PDMS) and silica (SiO2); liquid entry pressure (LEP) and contact angle (CA) were selected as responses. Scanning electron microscopy (SEM) analysis confirmed the SiO2 deposition and Fourier-transform infrared spectroscopy (FTIR) test evidenced the new functional groups i.e., Si-OH, siloxane, and C-F bond vibrations at 3446, 1099 cm-1, and 1150-1240 cm-1 respectively on the membrane surface. The average roughness (Ra) was increased four times for the coated membranes (0.202-0.242 µm) as compared to that for pristine PES membrane (0.053 µm). The optimum PES-13 membrane exhibited consistent flux of 12 LMH and salt rejection (> 99%) with anti-fouling characteristic in DCMD using the feed solution of 3.5 wt% NaCl + 10 mM CaCl2·2H2O + 10 mg L-1 HA. The PES-13 membrane may therefore be a key membrane for application in DCMD against CaCl2·2H2O-containing salty solutions with HA.

4-Thiazolidinone analogs 1-20 were synthesized, characterized by (1)H NMR and EI-MS and investigated for urease inhibitory activity. All twenty (20) analogs exhibited varied degree of urease inhibitory potential with IC50 values 1.73-69.65µM, if compared with standard thiourea having IC50 value of 21.25±0.15µM. Among the series, eight derivatives 3, 6, 8, 10, 15, 17, 19, and 20 showed outstanding urease inhibitory potential with IC50 values of 9.34±0.02, 14.62±0.03, 8.43±0.01, 7.3±0.04, 2.31±0.002, 5.75±0.003, 8.81±0.005, and 1.73±0.001µM, respectively, which is better than the standard thiourea. The remaining analogs showed good to excellent urease inhibition. The binding interactions of these compounds were confirmed through molecular docking studies.
