RESUMO

OBJECTIVES: To emphasize the diagnostic difficulty in some cases of intradiverticular transitional cell tumor of the bladder. METHODS/RESULTS: A patient with intradiverticular transitional cell tumor is described. The clinical features and diagnostic aspects are discussed. Despite the use of different diagnostic procedures (X-ray, US, CT, retrograde cystography), diagnosis could only be made after surgery. CONCLUSION: In patients suspected as having intradiverticular transitional cell tumor, the definitive diagnosis can only be made following surgery.

OBJECTIVE: We report another case of splenic abscess after nephrectomy, a condition which must be considered in the differential diagnosis of post-operative fever. METHODS/RESULTS: A 20-year-old man presented with lumbar pain and fever. A diagnosis of xanthogranulomatous pyelonephritis was made and he was treated by subcapsular nephrectomy. Thirty days later the patient presented with a lumbar fistula from a splenic abscess and was treated by splenectomy. CONCLUSIONS: Fistulas and abscesses are more common after nephrectomy for xanthogranulomatous pyelonephritis. In our view, the subcapsular approach is an additional risk factor for the foregoing complications.

OBJECTIVES: We reviewed our series of epidermoid penile cancer to determine the predictive index of lymph node invasion and the incidence of phimosis in these patients. METHODS: We reviewed the records of 11 patients with epidermoid cancer of the penis. Six patients had been evaluated according to a protocol that included preoperative biopsy of the lesion, penile and abdominal ultrasound evaluation, CT and diagnostic lymphadenectomy for those cases suspected as having lymph node invasion. The predictive index (PI) was determined in relation to the grading of cell differentiation (G) and tumor stage (S) [PI = G+T] and the incidence of phimosis was sought during the interview. RESULTS: Lymph node invasion was confirmed in 2 patients with PI = 6 and in 1 patient with PI = 5. Diagnostic lymphadenectomy showed no evidence of lymphatic spread in 1 patient with PI = 3 and in another patient with PI = 4. The remaining 6 patients with PI < 2 did not undergo lymphadenectomy. Phimosis was associated in 73% of the cases. CONCLUSIONS: PI is a reliable index for lymph node invasion. Patients with PI < 4 do not require therapeutic lymphadenectomy. Phimosis should be operated early in childhood.

RESUMO

A case of transitional cell carcinoma of the prostate is described which had been diagnosed following TUR of the prostate due to cervico-prostatic obstruction. Subsequent cystoscopic and cytological controls, as well as multiple bladder biopsies have discarded a concomitant urothelial tumor. We underscore the rarity of transitional cell tumor of the prostate, its response to chemotherapy and the possibility of suspecting this tumor type when there is massive invasion of the prostate following TUR.

RESUMO

Six cases of eosinophilic cystitis are described, one of which had no previous surgery or manipulation of the bladder. The literature is reviewed and the clinical and anatomopathological features are described. We discuss the diagnostic criteria, treatment, prognosis and the hypothesis of an immediate-type of anaphylactic reaction as a pathogenic mechanism.

RESUMO

Two cases of retroperitoneal abscess that had recently been treated in our service are described. The primary infection was localized to the psoas muscle in one patient with a previous injury to the right lumbar region. The other case was secondary to an ascending urinary infection. Staph aureus were isolated in the first case and E. Coli in the second. The different clinical course and treatment of each case are described.

RESUMO

We report a case of uterovesical fistula that had been diagnosed by cystoscopy, vaginoscopy and ultrasonography. Fistulorrhaphy via the transperitoneal transvesical approach resolved the passage of urine to the vagina. The decision to perform a conservative or more aggressive approach is determined by the site of the fistula.
