RESUMO

Respiration, central hemodynamics, gas and acid-alkaline balance of the blood were examined in the course of hyperbaric oxygenation (HBO) sessions in 30 patients in the acute period of severe craniocerebral injury, including 15 patients with a favourable outcome and 15 patients with a subsequent lethal outcome. In the first group, HBO gave rise to a decrease in the minute respiratory volume (MRV) and, in particular, in the minute blood volume (MBV). Acid-base and gas balance of the blood remained unchanged or improved. This attests to intactness of oxygen reactivity of the brain and body on the whole and allows establishment of a favourable prognosis of the further course and outcome of disease. The second group manifested a different dynamics: the initial MRV was greater, further increasing in the course of HBO sessions, acid-alkaline and gas balance of the blood worsened towards the development of non-compensated respiratory alkalosis which reflects ineffective hyperventilation and disturbed perversed oxygen reactivity.

RESUMO

Respiratory regulation at rest and under the influence of increasing hypercapnia was investigated in patients with chronic nonspecific pulmonary diseases (CNPD) with restrictive and obstructive types of lung ventilation capacity disorder. An increase in the inspiratory activity of the central mechanisms of respiratory regulation was revealed even at the early stages of disease. The determination of this type of activity in comparison with respiratory pattern parameters could be of diagnostic value. CNPD patients with congenital immunopathies (Kartagener-Sievert syndrome, CNPD with alpha 1 antitrypsin deficiency) demonstrated concomitant respiratory regulation disorders in the form of low inspiratory activity in response to hypercapnia suggesting a complicated course of disease in these patients.

RESUMO

The parameters characterizing lung ventilation and electrical activity of expiratory muscles were investigated in six healthy young men exposed to the positive intrapulmonary pressure 20, 30 or 40 mm Hg. Gas mixtures with a varying oxygen content were used. The effect of two compensatory devices that applied different counterpressures to the body surface was evaluated. It was found that the parameter that can best characterize the respiratory function in the case of a positive intrapulmonary pressure is electrical activity of expiratory muscles. The hypoxic effect makes no important contribution to the change of respiratory parameters during short-term exposures to the positive intrapulmonary pressure 20, 30 or 40 mm Hg. The different methods of applying compensation do not influence changes in the above respiratory parameters.

RESUMO

The authors have shown that in early postoperative period pronounced changes in the mechanics of respiration were observed in patients subjected to a partial lung resection. The dynamic pliability of lungs was considerably decreased, general pulmonary resistance became higher, the specific and minute respiration work was increased respectively. The degree of arterial hypoxemia was closely associated with the increasing general lung shunt of the blood. It was concluded that such disorders in the early postoperative period could be considerably reduced by methods of active sanitation of the tracheobronchial tracts.
