RESUMO

The aim of this research was to assess the distribution, sources, contamination status, ecological risk, and human health risk of heavy metals (HMs) in soil and sediments of a used-automobile spare part market in Nigeria. Forty-three (43) soil samples were collected within a spare part market section (SPMS-17 samples), market-residential section (MRES-10 samples), traffic section (TRAS-10 samples), and non-market residential section (NMRS- 6 samples). Fifteen (15) stream sediments were collected within and around SPMS. Based on average concentrations, HMs (As, Cd, Cr, Cu, Fe, Mo, Pb, and Zn) had their highest values in SPMS, and their minimum values were observed in NMRS. The high concentration was as a result of contributions from anthropogenic activities such as the direct discharge of used-lubricant oil, scrap metals, tire wear, and traffic emission in the environment. However, Al, Co, and Mn were derived from the geology of the area. The same trend was observed in the stream sediment section (STSS), except that in addition to Al, Co and Mn in soils, Cr was also sourced from geogenic activity. There were moderate to high enrichment/contamination factors of the anthropogenically sourced HMs, especially in the soil of SPMS, MRES, TRAS and stream sediments (STSS). Similarly, high potential ecological risk (Eri) and ecological risks (RI) were observed for As, Pb, and Cd in SPMS and STSS, while these were moderate in MRES and TRAS. Assessment of health risks was within acceptable limit for most of the HMs in the different sections for both adults and children, except As, Cd, and Pb in SPMS and STSS, which were beyond the acceptable limit for children. The carcinogenic risk was within the acceptable limit.

BACKGROUND: Assessment of control is central to asthma management. The Asthma Control Test (ACT) and Global Initiative for Asthma (GINA) defined control are commonly used measures of asthma control. This study examined the agreement between the ACT and GINA defined control and determined the best cut-off point of ACT to predict GINA level of control. METHODS: It was a cross-sectional study of 82 consecutive [72% females, age 44 (IQR) 29-60 years] asthmatic patients in a tertiary teaching hospital. We compared ACT with GINA defined control using Cohen's kappa and receiver operating characteristics curve was computed to determine sensitivity and specificity. RESULTS: There was fair to moderate level of agreement between ACT and GINA defined control (kappa 0.34 to 0.46). To detect GINA-defined controlled asthma, the ACT cut-off point of =20 had a sensitivity of 81%, specificity of 74%, a positive predictive value of 52% and negative predictive value of 92%, with an area under the receiver operating characteristics curve(AUC) value of 0.82 [95% confidence interval (CI): 0.72-0.90]. ACT scores of = 19 predicted GINA-defined uncontrolled asthma with sensitivity of 87%, specificity of 51%, positive predictive value of 41%, negative predictive value of 91% with AUC values of 0.75 (95% CI: 0.75-0.84). CONCLUSION: In Nigerian asthmatics, ACT scores of =20 and =19 are useful for identifying patients with controlled and uncontrolled asthma respectively, as defined by GINA 2014 criteria. However, the former is more predictive than the latter.

OBJECTIVES: This study examined the barriers and facilitators to community belonging for immigrants in Alberta, Canada. STUDY DESIGN: The study used a qualitative descriptive research design. METHODS: A total of 53 immigrant service providers in the province of Alberta participated in interviews and focus groups. The sample was purposively recruited through immigrant service organizations in the province. Interviews lasted approximately 45 min, whereas focus groups lasted approximately 1.5 h. The interviews were audio recorded, transcribed verbatim, and thematically analyzed with the aid of NVivo qualitative software. RESULTS: Participants discuss two forms of community belonging in this study: (a) belonging to an ethnocultural group; and (b) belonging within mainstream Canadian society. Barriers to mainstream community belonging for immigrants include employment barriers, language barriers, and discrimination. Recent immigrants often experience a sense of belonging to their ethnic group within the host country before feeling connected to others in their local geographic community. A major factor contributing to this trend is the lack of ethnocultural diversity in local community organizations in the areas where immigrants live. Immigrant service agencies and religious institutions compensate for this deficiency through creating avenues for social connection within and across ethnocultural groups and to mainstream Canadian society. CONCLUSIONS: Local community organizations should address issues of ethnocultural diversity and discrimination to improve the mental health of immigrants by fostering community belonging. Supporting programs in immigrant service agencies and religious institutions to increase social participation and engagement would, also, help strengthen community belonging and improve immigrant mental health.

The uptake and specificity of drugs and the bioavailability of poorly soluble drugs has been improved by means of targeted drug delivery using nanoparticles. Many platforms have been used for nanoparticulate drug delivery and these include liposomes, polymer conjugates, metallic nanoparticles, polymeric micelles, dendrimers, nanoshells, and protein and nucleic acid-based nanoparticles. Starch is the 2nd most abundant natural polymer and has found wide use in drug delivery systems as binder, disintegrant and filler. However, its application is limited by the poor functional properties of native starch. Starch nanocrystals of different shapes and sizes can be obtained based on the starch origin and isolation process involved. Nanocrystals with varying morphology have been reported; from nanocrystals of platelet-like shaped waxy maize starch with 5-7 nm thickness and 15-40 nm diameters, to those with round and grape-like shape from potato starch granules, with sizes ranging from 40 nm to 100 nm. This review describes different methods of obtaining starch nanoparticles, their modification and application in drug delivery.

Effect of honey on reproductive functions of male rats exposed to nicotine was examined in this study. Thirty-two adult male wistar rats (n=8/Group) were grouped as Control (distilled water), Nicotine (1.0mg/kg bwt), Honey (100mg/kg bwt) and Nicotine with Honey. The animals were orally treated for 35 days consecutively. Epididymis sperm motility, viability, morphology and counts were estimated, serum Follicle Stimulating Hormone (FSH), Leutinizing Hormone (LH) and Testosterone were assayed using ELISA method and testicular histology were also assessed. Significant reduction in percentage sperm motility, viability, morphology and counts were observed in nicotine group compared to control. Serum FSH, LH and testosterone levels were significantly reduced in nicotine group when compared with the control. There was significant improvement in sperm motility, viability, morphology, counts, FSH, LH and Testosterone in group co-treated with nicotine and honey  relative to nicotine group. Also, the degenerative seminiferous tubule architecture due to nicotine was improved by honey. In conclusion, honey may suppress nicotine toxic effect on reproductive functions in male Wistar rats.

Calcific uraemic arteriolopathy (CUA), a potentially life-threatening vasculopathy of the skin and subcutaneous tissues is rarely associated with advanced chronic kidney disease (CKD) particularly in patients on haemodialysis. It is more frequently reported in whites than in blacks and commonly accompanies hyperphosphataemia, elevated calcium-phosphate product and marked secondary hyperparathyroidism. We report a rare case of CUA that complicated end stage renal disease secondary to obstructive uropathy in a 68 year old Nigerian. The risk factors for CUA, diagnosis, management and our peculiar limitations were reviewed and discussed.

AIM: The patho-physiology of pre-eclampsia is not fully understood. This study determined the plasma levels of markers of oxidative stress, thyroid hormones, nitric oxide, C-reactive protein, and nutritional profiles in pre-eclamptic patients. METHODS: Thirty-two pregnant women with pre-eclampsia and 40 women with normal pregnancy (controls) participated in this study. The pre-eclamptics were recruited after 20 weeks of gestation. They presented with hypertension (systolic pressure = 169 ± 26.0 mmHg, diastolic pressure = 102 ± 11.0 mmHg), significant proteinuria (368 ± 39 mg/24 h), severe headache, abdominal pain and vomiting. The plasma levels of total antioxidant potential (TAP), total plasma peroxides (TPP), total cholesterol (TC), total protein (TP), albumin, globulin, nitric oxide (NO), C-reactive protein (CRP), total tri-iodotyronine (TT3) and thyroid stimulating hormone (TSH) were determined in them using spectrophotometry, radial immunodiffusion and enzyme linked immunosorbent assay (ELISA) methods, respectively. Oxidative stress index (OSI) was calculated as the percent ratio of TPP and TAP. RESULTS: The weight and body mass index of pre-eclamptics increased significantly (p < 0.05) when compared with the controls. The mean systolic and diastolic blood pressure increased significantly (p < 0.001) in pre-eclamptics when compared with the controls. Plasma mean values of TAP, NO, albumin, TP and TT3 decreased significantly (p < 0.05) in pre-eclamptics when compared with the controls. The plasma mean values of TSH, TPP, OSI, CRP, and TC increased significantly (p < 0.05) in pre-eclamptics when compared with the controls. There was no significant change in the plasma value of globulin when compared with the controls. TT3 correlated positively with plasma TP and globulin in the pre-eclamptics. CONCLUSION: It could be concluded that hypothyroidism, hypercholesterolemia, oxidative stress and deranged inflammatory responses are possible features of pre-eclampsia.

Despite the rarity of femoral aneurysm and the plethora of other causes of swelling in and around its anatomic location, the possibility of its existence must always be borne in mind. A rare case of metachronously bilateral femoral aneurysms mimicking soft tissue sarcoma in a young Nigerian man is presented. Caution in the diagnosis of groin masses is hereby re-emphasized.

Water is a valued natural resource for the existence of all living organisms. Management of the quality of this precious resource is, therefore, of special importance. In this study river water samples were collected and analysed for physicochemical and bacteriological evaluation of pollution in the Unity Road stream segment of Asa River in Ilorin, Nigeria. Juvenile samples of Clarias gariepinus fish were also collected from the experimental Asa River and from the control Asa Dam water and were analysed for comparative histological investigations and bacterial density in the liver and intestine in order to evaluate the impact of pollution on the aquatic biota. The water pH was found to range from 6.32 to 6.43 with a mean temperature range of 24.3 to 25.8 °C. Other physicochemical parameters monitored including total suspended solids, total dissolved solids, biochemical oxygen demand and chemical oxygen demand values exceeded the recommended level for surface water quality. Results of bacteriological analyses including total heterotrophic count, total coliform and thermotolerant coliform counts revealed a high level of faecal pollution of the river. Histological investigations revealed no significant alterations in tissue structure, but a notable comparative distinction of higher bacterial density in the intestine and liver tissues of Clarias gariepinus from Asa River than in those collected from the control. It was inferred that the downstream Asa River is polluted and its aquatic biota is bacteriologically contaminated and unsafe for human and animal consumption.

The erythrocytic parameters during pregnancy and pseudopregnancy in bitches were studied and compared in 8 bitches aged 2 -3 years and weighing 10-12 kg. Blood samples were collected from the bitches before mating, during the three trimesters of pregnancy and the post partum period. The packed cell volume (PCV %), haemoglobin concentration (Hb gm/dl), red blood cell count(x10(6)/µl) were determined using standard methods. The mean corpuscular volume (MCV) and mean corpuscular haemoglobin concentration (MCHC) were then calculated. Six of the bitches were pregnant and 2 were pseudopregnant. The results showed that in pregnant bitches, the PCV decreased significantly from the premating values of 51.37+0.94% to 34.00+8.04% during the third trimester of pregnancy (P<0.05). There was also a significant decrease in Hb values (P<0.05) from the premating period (16.30 ± 0.20gm/dl) to the third trimester of pregnancy (11.25±1.80gm/dl). The values of Red blood cells (RBCx10(6)/µl) during the premating period (12.70+3.15) were not significantly different from the values during the first second and third trimesters (11.13+3.87, 10.38+4.54 and 12.24+3.15, respectively). The trend of decrease in PCV and Hb values were not observed in the bitches with pseudopregnancy. This shows that these erythrocytic parameters can be used to detect and differentiate between pregnancy and pseudopregnancy in bitches as early as the first 20 days post mating.

The quantitative effects of a new gum, used as a binder, on the mechanical and release properties of paracetamol tablet formulations were analyzed in a 23 full factorial experiment. Cissus gum extracted from Cissus populnea Guill. & Perr. (Vitaceae) was compared with official gelatin. The individual and interaction effects of type of binder, concentration of binder and packing fraction on the friability, tensile strength, brittle fracture index, disintegration time and drug release profile of tablets were determined. Changing the binder from gelatin to cissus gum led to an increase in friability and a decrease in tensile strength, brittle fracture index (BFI) and drug release variables. Increasing binder concentration from 2.0w/w to 4.0%w/w, and increasing relative density from 0.80 to 0.90, led to increases in lamination tendency and release rate of the formulations. Tablets containing gelatin had higher tensile strength, lower friability, longer disintegration time and a greater tendency to laminate than those with cissus gum. Hence, care must be taken in choosing a suitable binder for tablet formulations, with respect to their mechanical and release characteristics. The study suggests that cissus gum should be preferred to gelatin in tablet formulations that tend to cap or laminate or in formulations meant for rapid drug release


Os efeitos quantitativos de uma nova goma como agente agregante nas propriedades mecânicas e de liberação de formulações de comprimidos de paracetamol foram avaliados utilizando um modelo fatorial 23 experimental. A goma foi obtida de Cissus populnea Guill. & Perr. (Vitaceae) e comparada com a gelatina oficial. Os efeitos individuais e de interação do tipo de agregante, concentração de agregante, porcentagem de friabilidade, resistência à tração, índice de fratura e tempo de desintegração e liberação do fármaco dos comprimidos foram determinados. A mudança do agente agregante de gelatina para goma de cissus levou a um aumento da friabilidade e a uma diminuição na resistência à tração, no índice de fratura e nos parâmetros de liberação do fármaco. O aumento da concentração do agregante de 2,0% p/p para 4,0% p/p e da densidade relativa de 0,80 a 0,90 levou a um aumento na tendência de laminação e na taxa de liberação das formulações. Comprimidos contendo gelatina apresentaram maior resistência à tração com menor friabilidade e maior tempo de desintegração e dissolução com maior tendência para laminado, quando comparados com a goma de cissus. Por isso, cuidados devem ser tomados na escolha de um agente agregante para formulações de comprimidos no que diz respeito às características mecânicas e de liberação. O estudo sugere que a goma de cissus pode ser empregada ao invés de gelatina em formulações de comprimidos que apresentam tendência de formar laminado ou decapeamento ou em formulações para liberação controlada de fármacos.

INTRODUCTION: Cystic lesion of the omentum and mesentery are rare. The incidence of both cyst types has been variously reported to vary from 1/27,000-100,000 hospital admission. Omental cysts occur three to ten times less frequently than mesenteric cyst. Preoperative diagnosis is infrequently made because of lack of characteristic symptoms and signs. CASE PRESENTATION: We present our diagnostic and management challenges in a 43-year-old man with an unusually giant omental cyst confirmed as fibrosarcoma at histology. The cyst gave the abdomen an anteroposterior diameter of about 74 cm that could not be penetrated sufficiently by X-ray photons to produce diagnostic image even at maximum attainable output. Patient benefited from surgical excision. The removed cyst contained about 35 litres of fluid. CONCLUSION: Neglected omental cysts as in this case may grow to enormous size, undergo malignant transformation and poses serious diagnostic and surgical challenges.

INTRODUCTION: Foetus-in-foetu is a very rare congenital abnormality where a malformed foetus is included within the body of another foetus. Less than 200 cases have been reported with over 80% occurring in the abdomen. Only three cases of cervical foetus in foetu have been reported. The present case of giant orocervical foetus-in-foetu appears to be an index case. CASE PRESENTATION: This is a report of an extremely rare orocervical foetus-in-foetu with grotesque oddity diagnosed on prenatal ultrasonography at 35 weeks gestational age in a 28-year-old, G2P1+0, Nigerian woman who was unsure of her last menstrual date or month. The included foetus had two eyes, cranium, nose, long bones and a spine. The mother's attempts at vaginal delivery rather than the elective Caesarean delivery she was offered resulted in obstructed labour and intrauterine foetal demise. CONCLUSION: Giant cervical foetus-in-foetu is extremely rare. It could result in obstructed labour if vaginal delivery is attempted.

Two cases of cutis verticis gyrata (CVG) one in association with a saccular aneurysm of the internal carotid artery and another with intraventricular ependymoma are reported. Not only is CVG rare, its association with intracranial aneurysms has not been reported in the literature. This paper documents those rare associations and gives a short review of CVG.

OBJECTIVE: The object of this report is to highlight some of the less known atypical features of spinal tuberculosis (TB) in the hope of facilitating early diagnosis. Pure neural arch and sacral TB is rare and the co-existence of these two as widely separated skip lesions in the same patient is even rarer. CLINICAL PRESENTATION: An unusual case of tuberculous process affecting the sacrum as well as the neural arches of upper cervical vertebrae is presented. Neither the clinical features nor the imaging techniques, including radiography, bone scintigraphy, computed tomography, and magnetic resonance imaging, were helpful in establishing the diagnosis. The destructive lesion of the sacrum with a rectally palpable presacral mass was thought to be a chordoma or chondrosarcoma until the patient developed upper cervical cord compression with an extradural myelographic block. Development of this second destructive lesion involving the posterior spinal elements (the neural arch) led to a diagnosis of malignant spinal metastasis. The true diagnosis was only revealed by the histology of the solid tumor-like extradural mass in the upper cervical region and demonstration of acid-fast bacilli (AFB) in the lesion. Anti-TB chemotherapy resulted in complete resolution of sacral and cervical lesions as well as the neurologic deficits. CONCLUSION: Differential diagnosis of the obscure spinal lesion should include tuberculosis, specifically the atypical forms; especially because complete cure is possible with early treatment and neurologic morbidity is high in neglected cases.

Fourteen children (of Arab ethnic origin) with Sotos syndrome are described. They were referred to King Khalid University Hospital, Riyadh between July 1992 and June 1997. Their phenotypic characteristics were compared with established diagnostic criteria. There was a male:female ratio of 1.3:1 and a high rate of consanguinity (36%) among parents. At birth, 54% were large and about one-third showed increased height and occipitofrontal head circumference (OFHC). The neonatal histories revealed respiratory and feeding problems in 21%, followed later by delayed motor milestones and speech development in 57%. During childhood, weight, height and OFHC increased further to > 97th centile in 71%, 71% and 93%, respectively. A seizure disorder affected 43%, and 75% had mental retardation (IQ < 70). A non-specific EEG abnormality was found in half of those with seizures. Cranial CT/MRI showed ventricular dilatation in 15% and one patient had corpus callosum dysgenesis. Abdominal ultrasound revealed hydronephrosis in two patients. Radiological cephalometric measurements showed relative prognathism in cases of Sotos syndrome compared with controls (p = 0.003). The study highlights the importance of considering Sotos syndrome in children who present with psychomotor delay.

RESUMO

We describe the findings in a six-year-old female with typical clinical symptoms of Moyamoya phenomena, but with an unusual angiographic appearance. The vertebral artery showed segmented occlusion in the upper cervical region, with reconstitution via collateral circulation through muscular branches. The carotid circulation showed involvement of the external carotid artery (ECA) branches and marked narrowing of the internal carotid arteries (ICA) bilaterally, suggesting that this disease is systemic in nature.

A retrospective review of the 17 cases of acromegaly, recorded in the hospital and with available CT scans was undertaken in order to determine the presence and of cutis verticis gyrata (CVG) frequency, and the spectrum of other associated radiographic appearances of CVG. The hormone levels were analysed, and the histopathology of the surgical specimens reviewed. Five cases were uncovered. In these, the scalp in CVG was markedly thickened with convoluted, gyriform or cog-wheel appearances. There was no correlation in the age, sex, duration of symptoms or the hormone levels in these two groups of acromegalics: with or without CVG. All cases were histologically pituitary adenoma. It is concluded that CVG is not uncommon in acromegaly. Its presence on a CT scan should caution the radiologist to the presence of pituitary adenoma or any of the other neurological causes of CVG.

A total of 82 patients who underwent spinal computed tomographic scan, in an 8-month period at the King Khalid University Hospital, Riyadh, Saudi Arabia, were analyzed for age, sex, the presence/absence, site, and type of ossification of the ligamentum flavum (OLF) as well as associated diseases. The OLF was radiographically present (bilaterally or unilaterally) as linear (74.5%) or nodular types (25.5%). The linear type was especially seen at the inner aspects of the ligamentum flavum. OLF was present in 35.4% of patients, and was seen at single and multiple levels in 51% and 41% of them, respectively. OLF was identified in a total of 47 levels in the study group and was associated with other disease in the spine in 38 (81%) of these levels. In none of the patients was OLF the cause of myelopathy. The incidence of OLF and its pathogenesis and significance are discussed. This paper is the first report of OLF from the Middle East. OLF in this part of the world is usually asymptomatic.
