RESUMO

INTRODUCTION: Compared with brain metastasis of malignant melanoma (MM), spinal metastasis, and in particular intramedullary spinal cord metastasis (ISCM), is extremely rare. CASE: A 78-year-old female patient suffered from disturbance of gait and mild dementia. Radiological investigations revealed multiple hemorrhagic lesions in the brain. She underwent surgical resection of a right parietal lesion, the diagnosis of which was MM. Re-examination of her past history revealed that the patient had undergone surgical resection of a nevus on her left cheek 3 years previously, the diagnosis of which had been MM. After she had died of the tumor 3 months later, complete autopsy was performed. Multiple ISCMs with hemorrhage were detected in the cervical, thoracic, and lumbar cord. DISCUSSION: We found 9 cases of ISCM of MM in the literature, 5 of which were located in the cervical cord, 3 in the thoracic cord, and 1 in the lumbar cord. One difference between the findings noted in the literature and those in the present case involved the cross-sectional location of metastases in the spinal cord. In the present case, it appeared that postoperative management for the left buccal melanoma, which did not include adjuvant therapy, affected the postoperative clinical course. The prognosis was also affected by overlooking of ISCM. The brain metastases in the present case induced deterioration of her neurological symptoms rapid enough that the possibility of ISCM was not considered. On evaluation of tumor spread from MM, it is important to take into account not only intracranial metastases but ISCM as well.

Single-rooted soybean leaves were used to investigate the suppression of photosynthesis through end-product inhibition during acclimation to CO(2 )enrichment. The photosynthetic activity was greater in leaves cultured at a CO(2) partial pressure of 70 Pa (high-CO(2)) than that in the leaves cultured at 35 Pa CO(2) (control) during the initial exposure to CO(2) enrichment but then decreased rapidly with a large accumulation of starch, to well below the level of the control leaves. The response curve of photosynthesis (A) to the intercellular CO(2) concentration (Ci) in the high-CO(2) leaves cultured long-term exhibited a significantly low initial gradient. However, on exposure to darkness for 48 h, the initial gradient of the A to Ci curve and rate of photosynthesis were completely restored, and almost all of the accumulated starch was expended. The ribulose bisphosphate carboxylase (RuBPcase) content and activation ratio in the high-CO(2) leaves remained high and roughly constant during the experiment, and were unchanged by the exposure, while this enzyme was slightly inactivated or inhibited after long-term exposure to CO(2) enrichment. The lower rate of photosynthesis in the high-CO(2) leaves could be linearly increased to a rate approaching the control level by increasing the external atmospheric [CO(2)], which thereby compensated for a reduced CO(2) transfer diffusion from the intercellular space to the stroma in chloroplasts. It is consequently concluded that, during the acclimation to CO(2 )enrichment, the suppression of photosynthesis through end-product inhibition was mainly caused by a lowering of the carboxylation efficiency of RuBPcase due to hindrance of CO(2) diffusion from the intercellular space to the stroma in chloroplasts brought about by the large accumulation of starch.

Lymphocytes obtained from mammary gland secretions (MGS) during lactation or the dry period of dairy cows were simultaneously analyzed and compared to ileal intraepithelial lymphocytes (IEL) and peripheral blood lymphocytes (PBL) using monoclonal antibodies (mAb) specific for bovine leukocyte differentiation antigens. The T-lymphocytes of MGS during lactation and those in IEL were predominantly CD8(+), while T-cells in MGS during the dry period were predominantly CD4(+). In addition, the proportion of gamma delta T-cells in MGS during lactation and IEL was fairly high. A large percentage of CD8(+) cells and T-cells coexpressed the activation molecule, ACT2, yielding a high proportion of ACT2(+) CD8 T-cells and ACT2(+) gamma delta T-cells, in MGS during lactation and IEL. However, both types of cells were found at an extremely low level in MGS during the dry period and in PBL. Thus, the predominant T-cell populations in MGS during lactation are phenotypically similar to those in IEL in the intestine.

RESUMO

From 1999 to 2000, NIHS Information and Computing Infrastructure (NICI) were newly renovated. The purposes of the renovation are (1) the improvement of the communication for business works in NIHS, (2) supporting for the research, (3) supporting for the administration work. The Internet connection speed was upgraded from 256 Kbps to 1.5 Mbps. The high quality network sever machines and database server machines were installed. The large-scale software systems were renewed their versions. Four experimental stations for medical plants at Hokkaido, Izu, Wakayama and Tanegashima connected to NIHS at Tokyo or Osaka branch by ISDN. We describe the providing information on NIHS home page, and how to utilize NICI for our research and official works.

In Japan we are very short of community resources for persons with mental disability. The authors, a board of resettlement, thought patients' actual conditions of living and needs for rehabilitation to let them live in the community had to be evaluated. Then in March 1993 the actual conditions of living and needs for rehabilitation of schizophrenic outpatients were investigated nationally. This article is the report on the patients' actual conditions of living. Investigation papers were sent to 358 institutions (286 hospitals and 72 clinics) which agreed to cooperate with the investigation. The 5186 investigation papers were received from 313 institutions. All of the papers except 18, which were lack of their ages, were analyzed. All of the schizophrenic outpatients, who consulted psychiatrists on one day during investigation, were considered objects of this research. Those who consented were included the research and psychiatrists filled in investigation papers. These institutions had a policy of intensive social resettlement activities and so on. Male patients were 55% and females were 45%. There were patients in the forties and females were older than males. Thirty-nine percent of them had been hospitalized once or twice. Thirty-four percent of them had been hospitalized for less than one year. Eighteen percent of them had not been hospitalized. Fifty percent of patients answered there was no friend and acquaintance, and had a tendency to stand alone. Fifty-three percent of patients lived with their parents, 21% with their spouses, and 17% alone. But 32% of females lived with their husbands. Sixteen percent worked for full-time jobs, 8% worked part-time jobs, 12% attended day care center 4% went to sheltered-workshops and only 1% went to rehabilitation-workshops for outpatients. While 13% didn't have a right to receive disability pension, the sources of income were job (30%), disability pension (30%), and welfare benefits (12%). Regarding the ability for living, both socially independent patients and daily independent were 36%. On the other hand, patients who needed support in daily life were 27%. This ability wasn't related to sex and age significantly (chi-square test). It is clear most of the patients were in isolation and depended on their families, so they needed support in income, accommodation and daily life.

The infectivity of neutralized IBDV by normal chicken serum (NCS) was detected in day-old and 3-week-old chicken spleen adherent (CSA) cells, and that of neutralized IBDV by maternal antibody (MN-Ab) was detected in 3-week-old CSA cells. Moreover, CSA cells from day-old chickens had complement receptor (CR), and CSA cells from 1-week-old had both CR and Fc receptor (FcR). However, the infectivity of neutralized IBDV by MN-Ab was confirmed on CSA cells which were blocked for FcR on CSA cells by heat-aggregated NCS (56 degrees C, 60 min). These results indicated that infection of neutralized IBDV by NCS on CSA cells occurred via CR, and neutralized IBDV by MN-Ab was infected via FcR. In day-old specific pathogen free (SPF) chickens, the antibody level in NCS treated and non-treated IBD live vaccine subcutaneously inoculated groups was higher than the levels in the MN-Ab-treated IBDV inoculated group, and detected until 28 days old. Moreover, subcutaneously inoculated chickens were protected against the challenge of wild IBDV at 21 days old, whereas subcutaneously inoculated chickens were infected with MN-Ab-treated IBD live vaccine. In commercial layers which had MN-Ab, antibody levels of subcutaneously vaccinated group were higher than both the non-vaccinated and orally vaccinated groups, and virus isolation and viral antigen were positive with high detection rates on peripheral lymphocytes of each subcutaneously vaccinated group of SPF and commercial chickens.(ABSTRACT TRUNCATED AT 250 WORDS)

Abnormalities of the p53 tumour suppressor gene occur in many types of cancer including approximately 60% of colorectal carcinomas. This study investigates in 47 colorectal carcinomas the relationship between stabilised p53 protein detected by immunocytochemistry (ICC), and p53 mutation. 27 cases stained positively with the antibody PAb1801. Sequencing of exons 5-8 revealed 19 mutations in 18 of these cases (one tumour contained two different mutations). A rapid, non-radioactive method was developed to screen for mutations in this region of the gene involving Single Strand Conformational Polymorphism analysis (SSCP) and a MspI restriction digestion. This screen detected 17/19 (89%) of the sequenced mutations, and a further four mutations in 20 PAb1801 negative cases that were confirmed by sequencing. Reproducibility of ICC in detecting stabilised protein was assessed by restaining the 47 cases with the antibody DO7 after pre-treatment to optimise detection. Fewer cases were negative with DO7 although overall concordance with PAb1801 was good. A substantial proportion of carcinomas with stabilised p53 as detected by ICC do not contain mutations in exons 5-8, whilst some mutations (the majority in exon 6) are not associated with stabilisation.

Statistical analyses of clinical observations made on outpatients visiting the Department of Oral Diagnosis, Meikai University School of Dentistry Hospital from June, 1970, to August 1986 were carried out. The total number of new outpatients during this period was 73,708. The results were as follows: 1) The largest number of patients was found in 1978. 2) There was a tendency for the number of new patients to increase in March, June, and August of every year. The smallest number of patients came in December. 3) The greatest number of new patients visiting the hospital was in the 0-9 age category, followed by those in the 20-29 and 30-39 age categories. 4) The greatest chief complaint of patients visiting the hospital was pain, followed by masticatory disturbance and swelling. 5) The number of outpatients coming to the hospital for cosmetic and temporomandibular joint problems has been increasing on a yearly basis over the past 16 years.

The course of infection with an attenuated strain of fowlpox virus (FPV), which is known to induce antibody-independent activation of complement via the alternative pathway, was investigated in 1- to 3-day-old chickens and 14-day-old chicken embryos by treatment with cobra venom factor (CVF). CVF was found to inhibit complement activity transiently via the alternative pathway but not via the classical pathway. In chickens treated with CVF, virus growth in the skin was enhanced, and pock lesions tended to disseminate, leading to fatal infection in some birds. Histologically, an acute inflammation at an early stage of infection (within 3 days) was inhibited, and virus content in the pock lesion was increased. In chicken embryos with immature immune capacities, CVF treatment caused changes in pock morphology from clear pocks to diffuse ones, an increase in virus content in the pock, and inhibition of cell infiltration. Thus, FPV infection was aggravated in both CVF-treated chickens and chicken embryos. These results are discussed in relation to roles of complement in the elimination of virus at an early stage of FPV infection.
