RESUMO

At the actual stage of becoming of Russian stomatology, the problem of sustainable development of rural stomatological care, as system of local formations of complex medical social nature belongs to national priorities and is considered as one of most important directions of public social policy. The stomatological health of rural population is to be considered as indicator of national stomatological health. This is confirmed by spatial structure: rural territories (inhabited territories outside cities) cover 2/3 of territory of the Russian Federation with population of 37.3 million or 1/4 of total population. The spatial structure of the Belgorod Oblast is reliably similar to the all-Russian one. The number of national and foreign studies proved that accessibility, quality and timeliness of insurance (state) stomatology is lower for rural residents that can be considered as one of signs of social inequality. Depending on the social economic status of region, signs of social inequality in the stomatology field are conditioned by broad spectrum of factors. Some of them are discussed in the article.

RESUMO

The article considers the problem of access to stomatological care, in particular to prosthetic dentistry, for people with limited health capacities in conditions of the COVID-19 pandemic. The range of factors impeding to get quality dental services in state stomatological polyclinics and at home was established by empirical study organized in two Subjects of the Russian Federation located in the Central Federal Okrug.

RESUMO

The article presents the results of study concerning improving medical care quality in rural areas. The purpose was to investigate the characteristics of medical personnel training in the framework of the "Zemstvo Doctor" program. The activities of the University were analyzed concerning implementation of professional training of medical personnel and including continuing medical education. To achieve this goal, efficiency of educational programs development considering contribution of the "Zemstvo doctor" program implementation was evaluated. The study was carried out in 2013-2019 on the basis of the Medical Institute of Belgorod State National Research University. The structured interview was applied as methodology of collecting primary data. The fact of decreasing of medical profession prestige that occurs against the background of implementation of the state program "Zemstvo Doctor" was established. The overcoming of this situation is necessary to improve medical care quality in rural areas. The focusing on existing regional features of medical care support and taking into account characteristics of health of population are required. In this respect the key factor is the training of rural physicians to employ high-tech technologies, including digital simulation and telecommunications equipment. The relevance of the state program "Zemstvo Doctor" is obvious, but the medical education system has no official instructions concerning the need of correcting educational programs. The efficiency of changes applied to educational programs was determined by subjective assessment of the need and the clinical experience of physicians-lecturers who are involved in resolving scientific and practical problems of health care in rural areas.

The article presents the features of the influence of social interactions on the social activity of older people and people with disabilities and their choice of types of social services. The Laboratory of Social Projects of the Belgorod National Research University in February - June 2018 conducted a sociological survey among the elderly and disabled of the Belgorod Region of Russia (240 people) to reveal knowledge about dental health and the prevention of dental diseases. The research methods were, firstly, the content analysis of individual social rehabilitation cards for the elderly and the disabled, and secondly, the survey of the elderly and the disabled. The results of the study showed: that the more elderly and disabled are included in social interactions, the more often they choose dental services among the entire list of types of social services; people who are not included in social interactions seek dental services in cases of acute pain or severe physical limitation of chewing function; socially active elderly and disabled people seek dental services for a large list of reasons (psychological, aesthetic); self-esteem by the elderly and disabled their dental health was more positive than that of the dentist; the elderly and disabled living in inpatient organizations have better dental health and are more likely to receive dental services than people living at home.

On the basis of longitudinal studies in children of older preschool and early school-age boys and girls with low, medium and high habitual physical activity (NAPA, SPDA, VPDA) identified the basic characteristics of the cardiovascular system, physical capacity and the degree of tension of regulatory mechanisms at rest and during functional loading. Analysis of the statistical distribution of the trait given chi2 and lambda criteria and average data of various functional indicators in the total sample identified a fairly wide area and the divergent levels of physiological individuality in a healthy population and the desirability of allocating the three typological groups--functional types (FT-1 - NAPA, the FT-2 - SPDA, FT-3 - VPDA). Model parameters are allocated first and second order for each CFT, as well as performance tests of the third order, describing the current functional status. Determination of the physiological features of the FT-1 FT-2 FT-3 with the establishment of their functional reserves and adaptive capacity makes it possible to identify the different strategies of interaction with the environment, evaluate their advantages and disadvantages and a more grounded approach to the problem of optimization of physical condition and prenosological diagnosis.
