We studied the spontaneous and metal induced oxidation of lipids and proteins in in vitro modeling ways of lipid peroxidation and blood proteins in the formation of malondialdehyde (MDA) and protein carbonyl groups (PCG) in 86 patients with chronic pyelonephritis (cPN) and 64 patients chronic glomerulonephritis(cGN) without prejudice excretory function of the kidneys. Installed the increase in the blood of patients with cPN MDAs 2 times, MDAe--14%, PCG 1.5 times; and cGN--MDAs 2.3 times, MDAe--29%, PCG--2 times. Found increased MDA content and PCG in the blood of patients with cPN and more expressive when cGN. Stimulation of in vitro peroxidation processes contributed significantly increased of production of MDA comparedwith baseline. In the modeling in vitro ascorbate-dependent and NADPH-dependent lipid peroxidation ways and the increase in protein production of MDA and PCG in both groups of patients, especially in the NADPH-dependent way, which must be considered in the correction of oxidative processes and antioxidant therapy appointment.

The article summarises data on the activity of tubular enzymes having marked organ-specific characteristics related to kidneys. They are N-acetyl-beta-D-glucosaminidase (NAG), its thermostable isoenzyme NAG B and beta-galactosidase localised in lysosomes. L-canavinine, ornithine aminotransferase having mitochondrial localization have also been discussed in the article. The authors have dealt also with the activity of lipoperaxidation processes having been assessed by an ammount of malonic dialdehyde in blood serum, erythrocytes and urine of 51 patients with progressive diabetes mellitus at the late stages of diabetic nephropathy.

Activity of tubular lysosomic (N-acetyl-beta-D-glucosaminidase--NAG, its thermostable isoenzyme NAG B and B-galactosidase--beta-GAL) and mitochondrial (L-canavanine: ornithine amidinetransferase--COAT) enzymes were measured in urine of 30 patients with diabetes complicated by diabetic nephropathy (DN). It was shown that activity of NAG, especially its thermostable isoenzyme NAG B and also beta-GAL in urine of DN patients was higher compare to those in healthy subject. Moreover COAT activity was registered in urine of DN patients while it is not presented in healthy persons. The precise dependence of NAG, NAG B, beta-GAL levels and COAT activity on the functional state of renal parenchyma in particular on tubular nephron nephrocytes was found that allows us to consider the given parameters as markers of diabetic process progressing in kidneys in patients with diabetes.

In patients with acute pyelonephritis, the efficiency has been studied of the blood transcutaneous laser irradiation and of the medicamentous membrane-stabilizing therapy, with n = 31 and n = 27 respectively. In the serous stage of acute pyelonephritis both methods appeared to be efficient while in the transitional stage it is the membrane-stabilizing therapy that has been shown to be superior to transcutaneous blood laser irradiation.

The activities of lipid peroxidation processes and enzymes of antioxidant system in the blood of patient with renal failure have been studied. The results demonstrated stimulation of lipid peroxidation processes in blood serum, cellular membranes of erythrocytes and in urine. The study of the antioxidant system discovered the inhibition of various enzymes: superoxide dismutase and peroxidase, inhibiting production of active oxygen forms; transferring and ceruloplasmin as regulators of Fe2+/Fe3+ ratio, glutathione reductase and glutathione peroxidase influencing the ratio of oxidated and reduced glutathione forms and the level of SH-groups. So, determination of lipid peroxidation processes and antioxidant enzymes in blood is the sensitive inform test in diagnostics of chronic renal failure.
