RESUMO

An experimental generator of microsecond pulses with an output current amplitude of ∼1 kA and an output voltage amplitude of ∼1 kV was developed. The generator is able to operate on a load whose electrical resistance changes significantly during operation. Under these conditions, the generator ensures high stability of output pulse energy and effectively uses energy that is reflected from the load. Using this generator, a high voltage generator with an output voltage pulse amplitude of ∼30 kV was developed. The high voltage generator is designed to be used in electric discharge technologies. The results of the experiments are given in which the high voltage generator produced discharges in water at a repetition rate of 500 Hz. The amplitude of the discharge current pulse was ∼600 A, and the pulse rise time was ∼150 ns.

RESUMO

High-voltage silicon dynistors, which are turned on by means of initiating a shock ionization process in silicon, have unique capabilities with regard to switching of current pulses of a nanosecond duration. This paper describes the design of shock-ionized dynistors (SIDs) optimized to reduce the energy loss during switching. The use of these SIDs allows for a twofold reduction in the triggering generator's power consumption. The results of this study show that it is possible to effectively increase the power switched in the nanosecond range through the use of SIDs with a higher operating voltage as well as to effectively reduce the energy loss in the SIDs by increasing the areas of their semiconductor structures. Moreover, it is possible to create switches that are able to switch nanosecond pulses with a power of more than 10 MW based on SIDs connected in series.

RESUMO

In the review published data on what modern antithrombotic drugs are used to support percutaneous coronary intervention in the treatment of acute coronary syndrome with ST segment elevation on the electrocardiogram in patients of older age groups. It was shown the effectiveness and safety of new pharmacological combinations using both anticoagulants and antiplatelet agents as in patients with acute coronary syndrome in elderly. It was highlighted also the need to find new combinations of anticoagulants and antiplatelet agents, to improve the prognosis in elderly persons subjected to percutaneous coronary intervention in patients with ACS.

RESUMO

One of the main difficulties in cardiology is how to correctly choose the volume and method of treatment in elderly patients with ischemic heart disease. 165 elderly patients with IHD were included into the study. 83 patients received partial myocardial revascularization by PTCA and stenting and 82 patients were on the conservative medical treatment. The state of coronary flow was estimated by means of the Holter-monitoring and treadmill-test results. Contraction ability of the heart was evaluated by echocardiography. The Quality of life level in patients was measured by SF-36 questionnaire. The results prove that elderly patients with IHD can benefit from the early partial myocardial revascularization which increases coronary circulation, quality of life level and preserves left ventricular function.

RESUMO

Coronary artery disease (CAD) is one of the most frequent diseases of geriatric patients, which substantially deteriorates the quality of life and leads to lethal outcomes. Coronary angioplasty (CA) is an effective method of CAD treatment in this group of patients. CA is the method of choice for geriatric patients with contraindications for surgical revascularization. No consensus about revascularization volume in multivessel disease has been reached in the worldwide practice. Peculiarities of myocardial blood supply in elderly patients allow partial revascularization with good periprocedural and long-term results.

RESUMO

One of the main difficulties in cardiology is how to correctly choose the tactickof surgical or interventional treatment in elderly patients with IHD. It is widely known that cardiovascular diseases are the main cause of mortality and invalidization all over the world. Atherosclerosis, by WHO data, takes the leading place in morbidity of industrial countries population. In USA and Europe it has become possible to reduce mortality from cardiovascular diseases by 30% due to active application in clinical practice of hi-tech methods of IHD treatment in elderly patients-coronary angioplasty and CABG.
