RESUMO

Twenty four hour blood pressure and ECG monitoring, assessment of sexual function (questionnaires, penile artery Doppler flowmetry) were carried out in male hypertensives treated with atenolol, metoprolol, or bisoprolol. Compared with atenolol and metoprolol bisoprolol was found to be more effective and safe. The use of bisoprolol was associated with no negative changes and even with improvement of some parameters of sexual function. Sildenafil could be used for correction of male sexual dysfunction occurring during treatment with other beta-blockers.

RESUMO

The efficiency of up-to-date medical technologies is closely related to the development of methods and tools of objective control over the patients' state in the process of treatment. The problem of continuous control over diagnostic information occupies a special place in the medicine of critical states, since the monitoring of the patient's current state can be of vital importance. The development of tools for the monitoring of the patients' state is based on the recording of physiological data and their further evaluation with the purpose of determination of indices characterizing the operation of the most important systems of the organism.

RESUMO

The authors present the results of preoperative identification of coronary diseases in patients with abdominal surgical diseases. Latent coronary diseases in surgical patients were detected by transesophageal electric stimulation (TEES) of the left atrium. Thirty-eight patients aged 46 to 82 were examined. Manifest symptoms of myocardial ischemia (changed ST segment, T wave inversion) were revealed in 47.3% patients, which is an evidence of the high resolving power of the method. Hence, noninvasive unsophisticated TEES technique is a reliable tool for identifying coronary diseases in surgical patients. The results of such examinations help adequately and purposefully prepare the patients to surgery and select the method of total anesthesia involving the minimal risk.

RESUMO

The results of studying the spontaneous oculomotor responses and those induced by vestibular and visual stimulation in 30 test subjects exposed to a 3-day dry immersion are presented. The subjects were tested every day under conditions of free diving and during additional tactile-proprioceptive, optokinetic and vestibulo-optokinetic stimulation. It is indicated that an exposure to immersion is inherent in an increased amplitude and rate of spontaneous floating movements of the eyes and in 30 percent of cases in: developing of spontaneous nystagmus; decreasing of an accuracy of fixed turns and tracking; facilitating of oculomotor responses during optokinetic and optovestibular stimulation. When used an additional support, on Day 0-1 after optokinetic and optovestibular stimulation on Days 2-3 of immersion the tested oculomotor responses were improved.

RESUMO

This paper presents the results of pre- and post-flight vestibular examinations using single and combined vestibular, optokinetic, proprioceptive, postural tests and cerebellar coordination tests. After extended flights most crewmembers showed spontaneous, position and positional nystagmus that points to vestibular dysfunction. The methods developed by the authors helped differentiate mechanisms of post-flight vestibular dysfunctions: in Group 1 subjects spontaneous nystagmus parameters varied during all tests; in Group 2 subjects they varied only in response to active tilt tests; in Group 3 subjects they varied only in response to vestibular, optovestibular and proprioceptive stimulation; and in Group 4 subjects they varied only in response to proprioceptive optovestibular stimulation and cerebellar coordination tests. Individual patterns of the above changes are associated with different pathways of adaptation of the vestibular function that depend on the involvement of various structures and compartments of the central nervous system.
