RESUMO

A simple method of quantitative evaluation of modified human serum albumin (SA) was suggested for the use in clinical practice. It is based on the natural ability of modified SA to form complexes with endogenous urea. An SA content, which lost its secondary and tertiary structures, is indirectly predetermined by the quantity of urea bound with protein that is biochemically determined after hydrolysis of the studied sample of partially purified SA. The clinical value of the parameter that is being determined is under discussion.

RESUMO

A comparative investigation covered UV-fluorescence characteristics of partially purified preparations of serum albumin (SA) in 16 glomerulonephritis (GN) patients and 12 healthy subjects. Fluorescence spectra of SA in GN patients proved shifted to short-wave region, this evidencing differences in SA structure of the patients and healthy subjects. The fluorescence spectra were characterized by parameter A. A = I320/I365, where I320 and I365 are intensities at the spectra in the wave lengths 320 and 365 nm, respectively. In 16 GN patients with normal renal function an inverse relationship was derived between the A value and blood albumin levels. A value and daily protein loss were related directly. This may denote predominant passage of structurally deformed albumin through glomerular filter.

RESUMO

Morphological, immunological and biochemical aspects of pathogenesis and manifestation of the nephrotic syndrome in patients with chronic glomerulonephritis were considered on the basis of the authors' result and literature data. High clinical importance of the lymphocyte stimulation index for patients' follow-up was emphasized. 131I-albumin metabolism was studied in 105 patients phenotyping of nephrotic hyperlipoproteinemia was done in 28. The results obtained suggested the relationship of protein, lipid and carbohydrate metabolic derangements with severity of the nephrotic syndrome. Possible causation of the above phenomena was discussed.

RESUMO

Exogenous insulin sensitivity as well as the dynamics of blood serum levels of immunoreactive insulin and growth hormone (GH) after oral glucose loading were studied in patients with glomerulonephritis of different clinical manifestations and varying renal functions. A correlation between disturbances in carbohydrate metabolism on the one hand, and the degree of renal failure and protein depletion on the other, was established. Disturbances in carbohydrate metabolism were detected as early as in the preazotemic phase of the disease. The clinical significance of the disorders and methods for evaluation of insulin response to oral glucose are discussed.
