RESUMO
4212 patients with ulcer gastrointestinal bleedings were treated during 1990-2007 years. 364 patients had gastric ulcers, other patients had duodenal ulcers. 1819 patients (43.2%) were operated. 418 patients (23%) were operated due to ineffective endoscopic hemostasis and recurrent bleedings. 409 patients (22.5%) underwent urgent operations because of risk for recurrent bleeding. Elective operations were carried out to 1358 patients (74.7%). Postoperative lethality amounted 5.8% including 19.1% lethality after emergency operations, 4.2% lethality - after urgent operations and 0.8% lethality - after delayed operations. The general hospital lethality amounted 3.3%. Lethality can be decreased by extension of indications for urgent operations, preventing recurrent bleedings.

65 patients with hypertensive disease (HD) and without it were examined in the acute period of gastroduodenal ulcer hemorrhage (UH) by 24 hour ECG monitoring, measurement of arterial pressure (AP), and evaluation of the severity of psychovegetative syndrome. Control groups comprised 20 patients with ulcer disease (UD), 28 with HD, and 12 with UD + HD. Patients of the latter group, similar to normotensive ones, suffered suppressed function of the sinus node in addition to functional changes in myocardium including a change of ST-T segment. Circadian rhythm of AD was unrelated to the presence and absence of HD. In both cases, blood loss was associated with anxiety-depressive condition and well-apparent ergotropic reactions. Patients with UD and HD had more expressed emotional and personality disorders associated with impaired vegetative response. Dippers had marked sympathetic activity at rest and in vegetative reactivity tests whereas non-dippers showed ergotropic effects in similar tests.

The great majority of patients with duodenal ulcer (DU) given a single oral dose (50 mg) of dicetel (pinaverium bromide)--a selective blocker of calcium channels--showed optimization of vegetative reactivity and vegetative support of physical and mental activities. This contributes to relief of psychovegetative syndrome of the modulation of dysfunctions of proximal gastrointestinal tract (GIT). The addition of dicetel to combined treatment of PH shortened healing of DU, relieved pain, stopped gastric and intestinal dyspepsia, corrected motor-tonic defects of the proximal GIT caused by dysfunction of different links of the regulatory-adaptive system.

The study of the psychovegetative status in 106 duodenal ulcer patients discovered alterations in this status. Different treatments were employed, among them--acupuncture with low-frequency alternating magnetic field. The latter had a positive effect on emotional and personality spheres, vegetative regulation, shortened the relief of the basic symptoms, scarring of the ulcer defect.

Sixty-six patients with duodenal ulcer were found to have apparent shifts in psychovegetative correlations. The patients underwent puncture with alternating magnetic field of active biological points responsible for general adaptation (E 36, G 14, VB 20) and gastroduodenal function (E 20, T 9, T 8). The treatment improved emotional, personality and vegetative regulation. The symptoms declined and ulcer healed more rapidly.

86 duodenal ulcer patients were diagnosed to have serious psychovegetative disorders. Among other treatment, the patients were exposed to AMF puncture of biologically active points. The puncture produced optimization of the function of cerebral (suprasegmental) vegetative structures, promoted a regress of vegetative dystonia clinical symptoms and speeded up ulcer healing.
