RESUMO

The electrooculograms (EOGs) of 24 patients with central retinal vein obstruction (CRVO) were correlated with fundus appearance and with the electroretinogram (ERG) b/a wave ratio. Of the 24, 17 (70.8%) showed abnormal EOGs [light peak versus dark trough ratio (Lp/Dt) of less than 1.85]. The EOG Lp/Dt ratio was abnormal in 64.7%, although the ERG b/a ratio was within normal limits. Though the exact mechanism is still not entirely clear, the EOG became abnormal in most patients with CRVO and correlated well with the severity of the disease.

RESUMO

Fourteen cases of massive vitreous hemorrhage caused by a retinal tear, the cause having been confirmed during closed vitrectomy, were studied retrospectively. In the 3 cases with no retinal detachment and in the 6 cases with partial but not extensive retinal detachment at the time of surgery, the electroretinogram (ERG), visual evoked response (VER) and electrically evoked response (EER) were recordable preoperatively. After surgery the retina was attached in all 9 cases, but later redetached in one case. The visual acuity improved in 8 of the 9 cases. In the 5 cases with subtotal or total retinal detachment at the time of surgery, the ERG was nonrecordable preoperatively in all 5 cases, while the VER and EER were recordable in 4 cases. After surgery the retina was reattached in 3 of the 5 cases, but later redetached in one case. Visual acuity improved in only 2 of the 5 cases. When a retinal tear is suspected to be present, surgical intervention should not be delayed in cases of massive vitreous hemorrhage which does not clear after initial bilateral patching and bed rest. Ultrasonographic and electrophysiological tests help establish the diagnosis and prognosis.

RESUMO

We studied the central visual fields in 11 eyes with retinal branch vein occlusion that presented extensive areas of capillary nonperfusion by fluorescein angiography. Relative scotomata corresponding to the areas of capillary nonperfusion were seen in seven eyes where the venous occlusion was of recent onset (average, 11 months). In the remaining four eyes with long-standing venous occlusion (average, 24 months), capillary occlusion was associated with absolute scotomata.

RESUMO

We observed and photographed intraocular cysticercosis in a 50-year-old woman. The subretinal cysticercus in the macular area produced a macular break during its passage from the subretinal space into the vitreous cavity. The parasite was removed by closed vitrectomy, but the macular break was left untreated because there was no vitreous traction to the macula. The patient ultimately developed a subretinal scar in the macular area, and visual acuity improved from hand movements to counting fingers.
