RESUMO

OBJECTIVE: To evaluate the effectiveness of the Cytoflavin in patients with dyscirculatory encephalopathy (DE) who had a new coronavirus infection. MATERIAL AND METHODS: 82 patients were examined: 16 (19.5%) men and 66 (80.5%) women aged 58 to 80 years, mean age 69±6 years and 70±6 years, respectively. All patients had moderate vascular cognitive impairment (less than 26 points according to the MoCA test), a history of COVID-19 in the period from 3 to 12 months before the start of the study. Patients before COVID-19 were registered with a neurologist with chronic cerebrovascular diseases with non-demented cognitive impairment of vascular origin. Patients of the main group (MG) received the Cytoflavin from the 1st to the 25th day of observation, inclusive, 2 tablets 2 times a day against the background of standard basic therapy. Patients in the comparison group received only standard basic therapy. RESULTS: It was found that during therapy with Cytoflavin, patients noted a positive trend in the form of a decrease in the symptoms of cognitive impairment, improved orientation and working memory, concentration of attention and counting. Also, patients in MG noted a decrease in fatigue and depressive disorders, an increase in motivation and a positive attitude, the emergence of interest in life, an improvement in the emotional background of mood, an increase in physical activity and working capacity. Comparing the mechanisms of development of vascular dysfunction, a pathogenetic commonality between DE and the development of consequences in the form of cognitive impairment caused by COVID-19 was noted. CONCLUSION: Cytoflavin therapy at a dosage of 2 tablets 2 times a day for 25 days can be recommended as part of complex therapy for patients with DE and a COVID-19.

Currently, the ongoing pandemic of the novel coronavirus infection is still a major public health problem worldwide. The questions of immunosuppressive therapy of patients with psoriasis and psoriatic arthritis, the possibility of vaccination against the background of the use of genetically engineered drugs remain open. The article is of an overview nature and includes up-to-date information on the feasibility and safety of the use of genetically engineered biological drugs in patients with psoriasis in a pandemic (COVID-19). According to the international recommendations of the National Psoriasis Foundation COVID-19 Task Force expert group, treatment of psoriasis and/or PsA does not significantly alter the risk of SARS-CoV-2 infection and does not lead to worse prognosis of COVID-19. Therefore, patients not infected with SARS-CoV-2 should continue biological or other systemic therapy for psoriasis and/or PsA. According to a registry from 25 countries, hospitalization with SARS-CoV-2 infection was more common in patients receiving nonbiological systemic therapy than in patients receiving BAs. Thus, genetically engineered biological drugs do not appear to cause an increased risk of coronavirus infection and do not determine a more severe course. With regard to vaccination of patients with psoriasis, many issues require further study. According to international agreements, vaccination is not contraindicated in patients with psoriatic disease. However, there is still insufficient data on how the treatment of psoriatic disease affects vaccination. Avoiding COVID-19 or reducing the severity of infection following SARS-CoV-2 vaccination is thought to far outweigh any risk directly related to vaccination complications.

Involutional changes of the skin are associated with changes in its microcirculatory apparatus, among which spastic, atonic and mixed types are distinguished. The effect of low-intensity laser radiation in combination with autologous plasma with blood cells on the vascular component of the skin was studied. In the light of the restoration of the microcirculatory apparatus of the skin in different types, the clinical efficacy of the therapy was analyzed. The study of the effectiveness of low-intensity laser radiation in combination with autologous plasma with blood cells was conducted in 57 patients with clinical signs of involutive skin changes. Depending on the type of microcirculatory skin changes, all patients were divided into 3 comparable groups: the 1st - atonic type (n=24), the 2nd - spastic (n=12), the 3rd - mixed (n=21). The obtained data of a complex assessment of the dynamics of skin microcirculation, features of passive and active blood flow, indicate a significant pronounced positive effect of low-intensity laser radiation in combination with autologous plasma with blood cells in patients with spastic type of microcirculation.

The adapted DIRAC experiment at the CERN PS accelerator observed for the first time long-lived hydrogenlike π^{+}π^{-} atoms, produced by protons hitting a beryllium target. A part of these atoms crossed the gap of 96 mm between the target and a 2.1 µm thick platinum foil, in which most of them dissociated. Analyzing the observed number of atomic pairs, n_{A}^{L}=436_{-61}^{+157}|_{tot}, the lifetime of the 2p state is found to be τ_{2p}=(0.45_{-0.30}^{+1.08}|_{tot})×10^{-11} s, not contradicting the corresponding QED 2p state lifetime τ_{2p}^{QED}=1.17×10^{-11} s. This lifetime value is three orders of magnitude larger than our previously measured value of the π^{+}π^{-} atom ground state lifetime τ=(3.15_{-0.26}^{+0.28}|_{tot})×10^{-15} s. Further studies of long-lived π^{+}π^{-} atoms will allow us to measure energy differences between p and s atomic states and so to discriminate between the isoscalar and isotensor ππ scattering lengths with the aim to check QCD predictions.

AIM: A comparative analysis of the psycho-emotional status and quality of life of patients with acne and post-acne. MATERIAL AND METHODS: There were 147 patients included in the study, 84 (57%) women and 63 (43%) men, aged from 14 to 48 years. The following parameters were studied: CADI score, APSEA score, DLQI score. Anxiety and depression were assessed with HAM-D and HAM-A. RESULTS: Higher scores on CADI, APSEA and DLQI and dissatisfaction with the results of therapy were noted in older patients. Significant changes in CADI, APSEA and DLQI scores in patients with post-acne symptoms were recorded later compared to patients with acne. Anxiety and depression symptoms measured with HAM-D and HAM-A reached the level of depressive disorder of moderate severity and moderate anxiety in patients over 20 years old. Mental disorders were qualified within the nosogenic anxiety-phobic reactions developing in connection with dermatosis manifestation. CONCLUSION: Acnes substantially better give in to correction than post-acne symptom complex. Regardless of age, post-acne symptoms considerably reduce quality of life of patients, sometimes causing more expressed changes (anxiety and depression) than dermatosis.

RESUMO

The observation of hydrogenlike πK atoms, consisting of π^{-}K^{+} or π^{+}K^{-} mesons, is presented. The atoms are produced by 24 GeV/c protons from the CERN PS accelerator, interacting with platinum or nickel foil targets. The breakup (ionization) of πK atoms in the same targets yields characteristic πK pairs, called "atomic pairs," with small relative momenta Q in the pair center-of-mass system. The upgraded DIRAC experiment observed 349±62 such atomic πK pairs, corresponding to a signal of 5.6 standard deviations. This is the first statistically significant observation of the strange dimesonic πK atom.

RESUMO

Observation covered 12 patients under various antitumor medications. Group 1 was formed of patients with developed palmoplantar syndrome varying in severity, who received complex treatment including IR-therapy and local antioxidant medication. Group 2 included patients without palmoplantar syndrome, who received preventive treatment with IR-therapy. All patients of group 1 demonstrated lower severity of palmoplantar syndrome manifestations. In group 2, 80% of the patients avoided palmoplantar syndrome development, and 20% of the patients had light course of the syndrome manifestations. Patients at high risk of palmoplantar syndrome under antitumor therapy are recommended to undergo IR-therapy and local antioxidant medication.

RESUMO

Trophic ulcers are complications of chronic venous insufficiency in 15-18% of cases and appear in 1-2% of able- bodied population. The frequency increases with age up to 4-5% among the patients aged over 65, and radical removalof the disease can be achieved only in one tenth of the patients. With that, even healed trophic ulcers recur frequently: after surgery in 4,8-31,6% of cases (according to various authors), after conservative treatment in 15-100% of cases. Main treatment modalities for trophic ulcers are medical and surgical. Recent researches necessitate including physical therapy methods into the treatment complex, as combined techniques that enable to improve results of various treatment variants.

RESUMO

Oncologic morbidity has a tendency to steady rise. Treatment of oncologic patients, aimed to remove or cease malignant cells growth and prevent metastases, has considerable side effects and is conducted on psychologic stress background. Rehabilitation measures hence get crucial importance. Rehabilitation of oncologic patients in complex treatment is a multi-stage continuous recovery process that includes various influences. Correct implementation of physical therapy factors in rehabilitation program causes no tumor growth and metastases, and sometimes inhibits tumor cells growth. On recommendations of sanatorium-and-spa treatment for oncologic patients, the complex promotes rapid recovery of disordered functions, activation of adaptive responses, psychologic and social adjustment.

RESUMO

The combination of narrow-band range UV-A (311 nm) and middle/broad-band UV-B (320-400 nm) irradiation was shown to significantly reduce manifestations of objective and subjective symptoms in the patients presenting with lichen ruber planus. Moreover, it promoted elimination ofdisbalance between characteristics of congenital immunity and normalization of HBD-2 and TLR-2 gene expression.

RESUMO

The paper characterizes placental growth factors in the third trimester of pregnancy. The placental expression of vascular endothelial growth factor (VEGF) and transforming growth factor-beta2 (TGFbeta2) was studied in miscarriage after 34-37 weeks pregnancy and during term labor. Chronic placental insufficiency was diagnosed in miscarriage. The expression of VEGF and TGFbeta2 with trophoblast and decidual cells was decreased in miscarriage as compared with that during uncomplicated pregnancy. The growth factors under study are markers of chronic placental insufficiency in third-trimester miscarriage.

Effects of the principal mineral and mineralizing components of the saliva, nutrient organic acids (carbohydrate and some others substances' metabolites) on the activity of microflora of soft dental deposit were studied in vitro. The inhibitory effect was assessed visually from the appearance or absence of deposit coating, pH measurement, and by photo- and nephelometry. Concentrations of lactic acid and sucrose were measured. The inactivating effects of the reagents were assessed by double control: qualitative, by comparing the specimen with the optic density of the extract from aqueous suspension of the deposit, and time control, by comparing the specimen with the optic density of extracts from deposit suspensions directly after mixing with the medium. Acid medium was found to block the formation of soft dental deposit and inhibit the production of lactic acid by bacterial deposit. Hence, weakly acid gargle is recommended as an effective and physiological measure preventing cariogenic situation resultant from the intake of easily fermenting carbohydrates.

The course of saccharose adsorption on surface tissues of the oral cavity directly after carbohydrate loading was studied in vivo. The amount of saccharose adsorbed on tissue surface was assessed from its desorption from an isolated site of the solid phase of the oral cavity and was correlated to a unit of tissue surface. Cariogenic potential in the oral cavity was shown to be maintained to a great measure at the expense of soft tissues and, specifically, of the glossal mucosa which is characterized by a high adsorption capacity for saccharose and represents a sort of a saccharose "depot" in the oral cavity. Carbohydrate adsorption appears to be a physical process. Water gargle after sugar intake fails to solve the problem of prevention of a cariogenic situation. Weakly acid gargling appears to be more promising desorbents of saccharose from oral tissues.

Effects of the principle mineral and mineralizing components of the saliva, alimentary organic acids-products of carbohydrate metabolism, and of some other substances of various pH on sugar desorption from soft dental deposit were compared in vitro. The desorbing efficacy was assessed from the concentration of sucrose, derived from a sample of lyophilized dental deposit, in a suspension of the tested reagent. The major mineral components of the saliva, calcium and phosphate, were found to promote extraction of sucrose from the deposit in neutral and weakly alkaline media. Acid medium also favors sucrose desorption. Due to the physiological nature and efficacy of sucrose desorption, solutions of alimentary organic acids may be recommended for prevention of cariogenic situations caused by intake of easily fermented carbohydrates.

Sucrose content on surface of oral tissues and in the oral fluid following a carbohydrate loading was studied in vivo synchronously and over time. The content of sucrose adsorbed by oral tissues was assessed by its desorption with applications. Sucrose concentrations were the highest in the liquid closest to tissues in comparison with its levels in the saliva in general, this indicating a high cariogenic potential of the buccal mucosa due to intake of easily fermented carbohydrates. Prolonged utilisation of adsorbed sucrose to unavailable concentration provides a sufficiently long contact of mineralized tissues with acidogenic solutions, this promoting the development of a cariogenic situation. Search for means to prevent cariogenic situations should be aimed at search for effective desorbents of sucrose from the oral cavity just after its intake.
