RESUMO

Aim and objective: The present case report aims to describe the nonsurgical management of an anterior tooth with a blunderbuss canal and an open apex using mineral trioxide aggregate (MTA) under magnification. Background: When pulp is traumatized before root formation, it results in pulpal necrosis, due to which dentin and root formation are interrupted. As a result, the canal remains broad due to thin and fragile dentin walls leading to the open apex. Therefore, root canal treatment is a big challenge currently. In such cases, we prefer MTA apexification to form the hard tissue apical barrier, which is a foreseeable treatment and has been used as another advanced method than calcium hydroxide (CaOH2) apexification due to its superior properties. Case description: A novel apexification technique was used by the Departments of Pediatric Dentistry and Conservative Dentistry and Endodontics for MTA placement in the central incisor with respect to 11 of a 9-year-old female patient. MTA was used to form an apical barrier using the micro-apical placement (MAP) system under a dental operating microscope (DOM). Following MTA hard set confirmation, obturation with bioceramic sealer and gutta percha with warm vertical condensation was done, followed by post-endodontic composite restoration. Conclusion: This case describes the nonsurgical management of an open apex using MTA, MAP system, magnification, and bioceramics, which aided in the management of this endodontic enigma. AIM: The aim of the study was to evaluate and compare canal transportation (CT), centering ability (CA), and volumetric changes in severely curved canals prepared using ProTaper Gold (PTG), One Curve (OC), and Self-Adjusting File (SAF) systems via cone-beam computed tomography (CBCT). MATERIALS AND METHODS: Sixty mesiobuccal canals of maxillary molars were selected. Pre- and postinstrumentation CBCT scans were taken in the same position. CT and CA were calculated at 1 mm, 4 mm, and 7 mm from the apex; change in volume for whole canal was measured and analyzed statistically. RESULTS: SAF showed the least mean CT at all the levels with no statistically significant differences at 1 mm and 4 mm when compared to other groups while statistically significant differences were observed at 7 mm with PTG and OC. Regarding CA, SAF better maintained canal centricity than PTG and OC at all the levels assessed, though the differences were not statistically significant except at 7 mm where statistically significant difference was observed between SAF and OC. SAF removed less volume of dentin followed by PTG and the highest removal was shown by OC. CONCLUSION: SAF showed promising behavior with the least CT, most centered preparation, and minimal dentin removal.

AIM AND OBJECTIVES: This in vitro study was designed to comparatively evaluate the antimicrobial efficacy of Curcuma longa (turmeric - T1-10%, T2-20%); Tachyspermum ammi (ajwain - A1-10%, A2-20%); chlorhexidine (CHX) gluconate gel (hexigel - 1%); and calcium hydroxide (10%) as intracanal medicaments against Enterococcus faecalis. MATERIALS AND METHODS: Agar plates were prepared using brain-heart infusion (BHI) agar. Cultures of E. faecalis were grown in BHI broth at 37°C. Well diffusion method was used to derive results. Plates were inoculated for 72 h at 37°C and microbial zones of inhibition were recorded. Statistical analysis was performed with repeated measures analysis of variance. RESULTS: C. longa (T2-20%) and CHX gluconate gel (hexigel - CHX-1%) showed larger zones of microbial inhibition than C. longa (T1-10%) that were statistically significant (P < 0.05) and were highly significant when compared to T. ammi (ajwain - A1 and A2) and calcium hydroxide. CONCLUSION: C. longa can be used as intracanal medicament in endodontic failure cases.
