Introduction: Arthroscopic anterior cruciate ligament (ACL) reconstruction is a very commonly done procedure in recent times. There is a need for a long-term outcome study of ACL reconstruction with a comparison between different types of fixation techniques. The graft fixation methods vary from aperture fixation (interference screws) to suspensory fixation methods (endobutton). Failure of graft incorporation and the development of tunnel widening (TW) after ACL reconstruction have been frequently reported in the long term in present literature. TW especially complicates revision ACL surgery. This is a prospective non-randomized clinical study of arthroscopic ACL reconstruction comparing the functional results between aperture fixation and suspensory fixation. Materials and Methods: Two groups of 14 patients who underwent autogenous hamstring ACL reconstruction with a minimum of 2-year follow-up evaluation were included in the study. The first group underwent aperture fixation with bioabsorbable interference screw at tibial and femoral side. The second group underwent suspensory fixation with endobutton fixation on the femoral side and biointerference screw on the tibial side. Both group patients were examined clinically before surgery, and at 3 months, 6 months, 1 year, and at 2 years. They were compared for functional outcome with Tegner Lysholm knee score. Observation and Results: There was a significant improvement in functional outcome in both the groups for base and at 3 months, also for 3 months and 6 months but for 6 months-1 year. Group 2 is almost significant. This improvement in outcome in Group 2 is consistent from 1 year to 2 years. Conclusion: In our prospective study comparing the outcomes of functional outcomes of ACL reconstruction with aperture fixation versus suspensory fixation on the femur, which was evaluated using Tegner Lysholm knee score over a period of 2 years, suspensory fixation was found to be better. However, further studies involving a larger series of cases are required for a better evaluation of the outcome.

Bismuthinite (Bi2S3) nanostructures have garnered significant interest due to their appealing photoresponsivity which has positioned them as an attractive choice for energy conversion applications. However, to utilize their full potential, a simple and economically viable method of preparation is highly desirable. Herein, we present the synthesis and characterization including structural elucidation of a new air- and moisture-stable bismuth-pyrimidylthiolate complex. This complex serves as an efficient single-source molecular precursor for the facile preparation of phase-pure Bi2S3 nanostructures. Powder X-ray diffraction (PXRD), Raman spectroscopy, electron dispersive spectroscopy (EDS) and electron microscopy techniques were used to assess the crystal structure, phase purity, elemental composition and morphology of the as-prepared nanostructures. This study also revealed the profound effects of temperature and growth duration on the crystallinity, phase formation and morphology of nanostructures. The optical band gap of the nanostructures was tuned within the range of 1.9-2.3 eV, which is blue shifted with respect to the bulk bandgap and suitable for photovoltaic applications. Liquid junction photo-electrochemical cells fabricated from the as-prepared Bi2S3 nanostructure exhibit efficient photoresponsivity and good photo-stability, which project them as promising candidates for alternative low-cost photon absorber materials.

Stibnite Sb2S3 and tetrahedrite Cu12Sb4S13 nanostructures being economical, environmentally benign and having a high absorption coefficient are highly promising materials for energy conversion applications. However, producing these materials especially tetrahedrite in the phase pure form is a challenging task. In this report we present a structurally characterized single source molecular precursor [Sb(4,6-Me2pymS)3] for the facile synthesis of binary Sb2S3 as well as ternary Cu12Sb4S13 in oleylamine (OAm) at a relatively lower temperature. The as-prepared Sb2S3 and Cu12Sb4S13 nanostructures were thoroughly checked for their phase purity, elemental composition and morphology by powder X-ray diffraction (pXRD), electron dispersive spectroscopy (EDS) and electron microscopy techniques. pXRD and EDS studies confirm the formation of phase pure, crystalline orthorhombic Sb2S3 and cubic Cu12Sb4S13. The SEM, TEM and HRTEM images depict the formation of well-defined nanorods and nearly spherical nanocrystals for Sb2S3 and Cu12Sb4S13, respectively. The Sb2S3 nanorods and Cu12Sb4S13 nanocrystals exhibit an optical bandgap of ∼1.88 and 2.07 eV, respectively, which are slightly blue-shifted relative to their bulk bandgap, indicating the quantum confinement effect. Finally, efficient photoresponsivity and good photo-stability were achieved in the as-prepared Sb2S3 and Cu12Sb4S13 nanostructure-based prototype photo-electrochemical cell, which make them promising candidates for alternative low-cost photon absorber materials.

Fe3O4 magnetic nanoparticles, synthesized using co-precipitation method, were epoxy functionalized via plasma polymerization of 2,3-epoxypropylmethacrylate (EPMA) precursor. The EPMA-functionalized Fe3O4 nanoparticles (EPMA-f-MN) were employed as templates for facile, one-step covalent immobilization of laccase enzyme at room temperature. Samples were rigorously characterized by FTIR, TGA, SEM, TEM, XRD techniques, while Mössbauer spectroscopy (MöS) and vibrating sample magnetometry (VSM) confirmed the supermagnetic nature of Fe3O4 nanoparticles. Activities of free and immobilized laccase (ImLac) were assayed by spectrophotometrically monitoring the enzymatic reduction of substrate 2,2-azino-bis(3-ethylthiazoline-6-sulfonate) (ABTS) at 420 nm, corresponding to the λmax of ABTS.+. In addition to possessing higher thermal stability and a broader pH tolerance window compared to free laccase, the supermagnetic property of the Fe3O4 renders the ImLac system conveniently recoverable and recyclable. Practical applicability of ImLac towards catalytic degradation of industrial dyes was also ably demonstrated using Acid Blue 193 (AB 193) as a commercially used model textile dye, which belongs to the family of azo dyes. Over 95% degradation of the dye was achieved within a period of 4 hours. ImLac could be used for more than 10 dye degradation cycles with >90 % of retention in enzyme activity.

For any antimicrobial assay, a standard drug is used to compare the bactericidal efficiency of the bioactive compound under screening. The standard drugs have different targets that may be intracellular or membrane located. The location of the target is believed to be determining the bioactivity of the drug depending on the drug's access to its target. Therefore, different drugs must have a different magnitude in exhibiting the biological effect. However, in most of the published literature about the screening of bioactive compounds on antimicrobial activity, generally, the standard drug is randomly chosen while comparing against the bioactive compound of interest. Further, the antimicrobial activity is inferred by comparing the randomly chosen standard drugs without knowing the physicochemical parameters of the standard drug and the test molecule. It is just like an unfair comparison of the impact of a bullet with the impact of an explosive in a combat scene. Computer-based strategies for structure-based drug discovery presents a valuable alternative to the costly and time-consuming process of random screening. The docking studies provide better insights into the binding mechanism of substrate and inhibitor at the molecular level. The evaluation of such a comparison of bioactive compounds against randomly selected standard drugs through a customized virtual screening pipeline showed 57% false positives, 18% true positive, 17% true negative, 8% false-negative results. This study directs for mandatory cheminformatics-based assessment of the bioactive compounds before choosing the standard drug to compare with. BACKGROUND: The clinical feature and radiographic appearance of orthokeratinized odontogenic keratocyst (OKC) are not characteristic, which may lead to the misdiagnosis. The radiographic appearance of OKC may range from a small unilocular radiolucency to a large multilocular radiolucency, resembling other odontogenic cysts and tumors. AIM: The aim was to illustrate the characteristic feature of OKC presented on the digital panoramic radiograph and cone-beam computed tomography (CBCT), which may provide great value for the differential diagnosis and the treatment planning and also to compare the various radiographic features of OKC in CBCT and digital panoramic radiograph. MATERIALS AND METHODS: Digital panoramic and CBCT records of seven cases, with 11 lesions of OKC were analyzed retrospectively from the patients' database from our institute (2014 to 2016), which was histopathologically diagnosed as OKC. RESULTS: The mean age of the patients was 24.1 years, and the female-to-male ratio was 3: 4. Six of 11 lesions were localized within the mandible, and five lesions were in the maxilla. CONCLUSION: The presurgical assessment with radiological information is extremely important for treatment planning, and CBCT provides us with an accurate and faster three-dimensional representation of a lesion at a lower dose and cost, but the role of panoramic radiograph cannot be refuted.

The amelioration of antibacterial efficacy along with the reduced minimum inhibitory concentration (MIC) of sanguinarine (SGR) drug have been demonstrated through the uptake of SGR by p-sulfonatocalix[6]arene functionalized silver nanoparticles. The large upward pKa shift and enhanced stability of SGR resulting from the favorable supra-nanomolecular strategy are deciphered into an improved antibacterial drug against different pathogenic micro-organisms including multi drug resistant bacteria.

We report the development of pH-labile ascorbic acid-coated magnetic nanocarriers (AMNCs) for effective delivery of the anticancer drug doxorubicin hydrochloride (DOX) to tumor cells. The uniqueness of this drug delivery system lies in the covalent conjugation of DOX through carbamate and hydrazone bonds, resulting in a slow and sustained drug release profile at different environmental acidities. X-ray diffraction and transmission electron microscopy analyses reveal the formation of crystalline single-phase Fe3O4 nanoparticles with an average size of 10 nm. The changes in the interfacial characteristics of the nanocarriers and the presence of organic coatings are probed by infrared spectroscopy, dynamic light scattering, zeta potential, and thermogravimetric measurements. AMNCs show high colloidal stability in aqueous and cell culture media and possess good magnetic field responsivity and protein resistance characteristics. The drug-loaded nanocarriers exhibited sustained pH-triggered release of drug molecules in acidic mediums, substantial cellular internalization, and significant toxicity toward the proliferation of mouse skin fibrosarcoma (WEHI-164), human breast cancer (MCF-7), and human lung cancer (A549) cells. However, it showed significantly lower toxicity in human normal lung (WI26VA) cells. Overall, these results suggest a pH-sensitive drug release of nanoformulations, which showed selective toxicity to tumor than normal cells.

BACKGROUND: In current clinical practice, regardless of the clinical guidelines, BZDs and Z drugs are used beyond the period of indication, resulting in undesirable effects. This study aimed to assess feasibility of deprescribing amongst patients utilizing BZDs and Z drugs inappropriately for longer duration than the prescribed period. The study also analysed the Quality of Sleep (QoS) and Cost Savings incurred amongst deprescribed patients. METHODS: It was a prospective interventional study conducted in IP and OP settings of Psychiatry Department, Bangalore, India. Based on inclusion criteria, 109 patients were recruited for the study for a period of 7 months. Deprescribing was advised to inappropriate BZD and Z-drug users by clinical pharmacist after discussing with the prescribing psychiatrist. The patients were followed-up twice in a month after deprescribing. QoS was assessed by using Pittsburg Sleep Quality Index (PSQI) scale. The total medications cost incurred per patient/month before and after the intervention among both the groups was measured. RESULTS: Post-intervention, 40(30.69%) BZD users were deprescribed i.e, either dose tapered 6(5.5%), completely ceased 27(24.8%) or on si opus sit (SOS) BZDs prescription 7(6.4%). A majority of 44(40.36%) patients continued BZDs according to the algorithm. Clonazepam 35(87.5%) was the most deprescribed BZD. Deprescribing of BZDs showed an association with QoS of patients, p-value (<0.05). A statistically significant cost reduction was observed after deprescribing BZDs, (Z = 5.465, p=<0.001). DISCUSSION: Deprescribing BZDs was associated with decline in its usage; implementing deprescribing practice amongst the inappropriate BZD users is feasible, provides an improved QoS and an economic benefit.

Deinococcus radiodurans, an extremely radioresistant bacterium has a multipartite genome system and ploidy. Mechanisms underlying such types of bacterial genome maintenance and its role in extraordinary radioresistance are not known in this bacterium. Chromosome I (Chr I), chromosome II (Chr II) and megaplasmid (Mp) encode its own set of genome partitioning proteins. Here, we have characterized P-loop ATPases of Chr II (ParA2) and Mp (ParA3) and their roles in the maintenance of genome copies and extraordinary radioresistance. Purified ParA2 and ParA3 showed nearly similar polymerization kinetics and interaction patterns with DNA. Electron microscopic examination of purified proteins incubated with DNA showed polymerization on nicked circular dsDNA. ParA2 and ParA3 showed both homotypic and heterotypic interactions to each other, but not with ParA1 (ParA of Chr I). Similarly, ParA2 and ParA3 interacted with ParB2 and ParB3 but not with ParB1 in vivo ParB2 and ParB3 interaction with cis-elements located upstream to the corresponding parAB operon was found to be sequence-specific. Unlike single mutant of parA2 and parA3, their double mutant (ΔparA2ΔParA3) affected copy number of cognate genome elements and resistance to γ-radiation as well as hydrogen peroxide in this bacterium. These results suggested that ParA2 and ParA3 are DNA-binding ATPases producing higher order polymers on DNA and are functionally redundant in the maintenance of secondary genome elements in D. radiodurans The findings also suggest the involvement of secondary genome elements such as Chr II and Mp in the extraordinary radioresistance of D. radiodurans.

AIM OF THE STUDY: Bacteria that persist at the time of obturation increase the possibility of persistent apical periodontitis. An ideal sealer should be able to kill these remaining bacteria that are present on the dentinal walls as well as inside the dentinal tubules. This could be possible if a sealer has antimicrobial properties with optimum flow characteristics. Hence, the aim of this in vitro study was to assess the antimicrobial efficacy of epoxy resin-based sealer: AH Plus and Perma Evolution against Enterococcus faecalis on the 1st, 3rd, 5th, and 7th day and also to compare the flow characteristics of both epoxy resin-based sealers. MATERIALS AND METHODS: E. faecalis ATCC 35550 strain was used to assess the antibacterial efficacy of sealers by agar-diffusion test (ADT) and direct-contact test (DCT). Flow characteristics of sealers were measured according to the ADA specification no. 57. RESULTS: In ADT, Perma Evolution sealer showed larger zone of inhibition than AH plus on the 1st, 3rd, 5th, and 7th day, and in DCT, both sealers were equally effective in inhibiting E. faecalis growth on the 1st, 3rd, 5th, and 7th day. Flow test showed no significant difference between Perma Evolution and AH Plus sealer. CONCLUSION: Both the tested sealers were equally effective against E. faecalis up to 7 days of incubation period. Considering flow properties, both the tested sealers showed optimum flow as per the ADA specification no. 57.
