Objectives: This study compares the bacterial susceptibility to three different provisional restorations manufactured with different laboratory techniques, i.e., heat cure, CAD/CAM, and 3D printing. Materials and Methods: Totally, 45 health patients with age group of 30-60 years undergoing complete veneer crown treatment were selected for the study and divided into three groups with 15 samples in each as Group I: the crowns were made from polymethyl methacrylate polymer fabricated by the conventional heat cure method, Group 2: the crowns were made from polymethyl methacrylate fabricated by subtractive manufacturing CAD/CAM method, and Group 3: the crowns were made from oligomers fabricated by additive manufacturing 3D printing. The samples were examined for bacterial adherence at scanning electron microscope. The colony-forming units (CFUs) were calculated, and statistically analyzed. Results: It has been determined that the digitally fabricated provisional restorations prove to be superior in terms of surface topography than to the conventionally fabricated provisional restorations. Conclusion: The study concludes that 3D-printed provisional restorations are more precise with reduced bacterial susceptibility than milled ones.

INTRODUCTION: A woman's nutrition during pregnancy and nursing affects the mother and the growing child. Similarly, the first two years of a child's life are critical to their growth and development and are facilitated by optimum nutrition. Women's nutrition-related knowledge, attitudes, and practices influence household food and nutrition security. Mobile health (mHealth) is a potentially effective health intervention in pandemic situations when physical gatherings are restricted. OBJECTIVES: To examine the effectiveness of a mobile phone-based nutrition education intervention targeting pregnant and nursing mothers in six Sri Lankan divisional secretariat areas. METHOD: This intervention was evaluated using a before and after within-subjects design. The intervention included 19 messages over four weeks sent via mobile phone, covering nutrition themes such as pregnancy care, infant and young child-feeding, diet, family care for mother and child, and cash management. The intervention was evaluated based on a quantitative survey using a structured interviewer-administered questionnaire and qualitative interviews using a semi-structured questionnaire. The study population was pregnant and nursing mothers. The objective of the qualitative interviews was to identify how respondents used messages and how satisfied they were with the project. The outcome measures were awareness/knowledge, attitudes, social norms, self-efficacy, behaviour intentions, and practices of pregnant and nursing mothers. Trained enumerators collected data using a mobile phone. RESULTS: A total of 996 pregnant and nursing mothers participated in the pre-assessment survey, of which 720 completed the post-assessment. Most were nursing mothers (84.2% pre- and 78.9% post-assessment). Participants provided positive feedback on the intervention. Knowledge/awareness (t = -18.70, p < 0.01) and attitudes (t = -2.00, p < 0.05) increased when exposed to the intervention. Favourable improvements in the practices were also observed. Mothers' practices related to breastfeeding and 24-h dietary diversity showed a statistically significant improvement. However, social norms and behaviour intentions did not significantly improve. The qualitative component also revealed favourable responses. CONCLUSION AND RECOMMENDATIONS: The mobile intervention improved participants' knowledge, awareness, attitude, and practices, but not social norms or behaviour intentions. This approach is recommended to be used on a larger scale in community settings. In addition, mobile technology could drive intervention in pandemic-related situations.

Ternary nanohybrids based on mesoporous graphitic carbon nitride (g-C3N4) were synthesized and presented for developing stable and efficient Hydrogen (H2) production system. Based on photocatalytic activity, optimization was performed in three different stages to develop carbon nanotubes (CNTs) and WO3 loaded g-C3N4 (CWG-3). Initially, the effect of exfoliation was investigated, and a maximum specific surface area of 100.77 m2/g was achieved. 2D-2D interface between WO3 and g-C3N4 was targeted and achieved, to construct a highly efficient direct Z-scheme heterojunction. Optimized binary composite holds the enhanced activity of about 2.6 folds of H2 generation rates than the thermally exfoliated g-C3N4. Further, CNT loading towards binary composite in an optimized weight ratio enhances the activity by 6.86 folds than the pristine g-C3N4. Notably, optimized ternary nanohybrid generates 15,918 µmol h-1. g-1cat of molecular H2, under natural solar light irradiation with 5 vol% TEOA as a sacrificial agent. Constructive enhancements deliver remarkable H2 production and dye degradation activities. Results evident that, the same system can be useful for pilot-scale energy generation and other photocatalytic applications as well.

The biochemical reduction of nitrite (NO2-) ions to ammonia (NH3) requires six electrons and is catalyzed by the cytochrome c NO2- reductase enzyme. This biological reaction inspired scientists to explore the reduction of nitrogen oxyanions, such as nitrate (NO3-) and NO2- in wastewater, to produce the more valuable NH3 product. It is widely known that copper (Cu)-based nanoparticles (NPs) are selective for the NO3- reduction reaction (NO3-RR), but the NO2-RR has not been well explored. Therefore, we attempted to address the electrocatalytic conversion of NO2- to NH3 using Cu@Cu2O core-shell NPs to simultaneously treat wastewater by removing NO2- and producing valuable NH3. The Cu@Cu2O core-shell NPs were constructed using the pulsed laser ablation of Cu sheet metal in water. The core-shell nanostructure of these particles was confirmed by various characterization techniques. Subsequently, the removal of NO2- and the ammonium (NH4+)-N yield rate were estimated using the Griess and indophenol blue methods, respectively. Impressively, the Cu@Cu2O core-shell NPs exhibited outstanding NO2-RR activity, demonstrating a maximum NO2- removal efficiency of approximately 94% and a high NH4+-N yield rate of approximately 0.03 mmol h-1.cm-2 at -1.6 V vs. a silver/silver chloride reference electrode under optimal conditions. The proposed NO2-RR mechanism revealed that the (111) facet of Cu favors the selective conversion of NO2- to NH3 via a six-electron transfer. This investigation may offer a new insight for the rational design and detailed mechanistic understanding of electrocatalyst architecture for the effective conversion of NO2- to NH4+.

Heat shock transcription factors (Hsfs) play an essential role as transcriptional regulatory proteins against heat stress by controlling the expression of heat-responsive genes. Common bean is a highly thermosensitive crop, and, therefore, its genome sequence information is segregated, characterized here in terms of heat shock transcription factors and its evolutionary significance. In this study, a complete comprehensive set of 29 non-redundant full-length Hsf genes were identified and characterized from Phaseolus vulgaris L. (PvHsf) genome sequence. Detailed gene information such as chromosomal localization, domain position, motif organization, and exon-intron identification were analyzed. All the 29 PvHsf genes were mapped on 8 out of 11 chromosomes, indicating the gene duplication occurred in the common bean genome. Motif analysis and exon-intron structure were conserved in each group, which showed that the cytoplasmic proteins highly influence the conserved structure of PvHsfs and heat-induced response. The HSF genes were grouped into three classes, i.e., A to C and 14 groups, based on structural features and phylogenetic relationships. Only one pair of paralog sequences suggests that it may be derived from the duplication event during evolution. A comparative genomics study indicated the influence of whole-genome duplication and purifying selection on the common bean genome during development. In silico expression analysis showed the active role of class A and B family during abiotic stress conditions and higher expression in floral organs. The qRT-PCR analysis revealed PvHSFA8 as the master regulator and PvHSFB1A and PvHSFB2A induction during heat exposure in French beans.

Weight gain is an independent risk factor for decline in cardiometabolic and overall health-related quality of life in midlife women. The AIIMS-DST initiative aims to develop and validate stepwise recommendations specific for weight management in midlife women. The key clinical questions specific to weight management in midlife women were finalized with the help of a multidisciplinary team of experts in the guideline development group. Phase I including a systematic and/or narrative review, grading of evidence, and expert opinion was sought to develop clinical practice recommendations for each clinical question. Phase II focused on validation of clinical practice recommendations using the peer-review, Delphi method, and GRADE approach. The guidelines provide clinical practice points to address challenges encountered by midlife women in their attempts to manage obesity via lifestyle modification techniques. The initiation of discussion would help the health-care provider to identify the weight management needs of the women, educate women on different modalities of weight management, and empower them to incorporate corrective lifestyle behaviors. Before initiating the management, a comprehensive assessment of clinical and lifestyle-related parameters should be completed. A personalized behavioral lifestyle modification program addressing the midlife-specific barriers for optimal metabolic, musculoskeletal, and mental health should be planned. A consistent follow-up is required for maintenance of corrective eating and activity habits by addressing midlife-specific barriers for sustenance of healthy weight. These recommendations will be useful in opportunistic screening and management of obesity in midlife women across health-care settings.

OBJECTIVE: For a successful root canal therapy, it is necessary to locate all the canals debride and seal them with an inert filling material. The clinician must be aware of the internal morphology and variations in a permanent tooth. Mandibular first molars are widely studied to identify variations in the anatomy of the pulp space. In this study, the primary objective was to measure the distance between the mesiobuccal (MB) and mesiolingual (ML) canals in patients with and without a middle mesial canal (MMC) using cone-beam computed tomography (CBCT). The secondary objectives were to assess the tapering degree of the mesial root and to measure the dentinal thickness in relation to the danger-zone area in patients with and without an MMC. MATERIALS AND METHODS: A total of 200 CBCT scans were evaluated for the presence of an MMC. Two observers performed the observations, and the results obtained were subject to statistical analyses. RESULTS: The results revealed the prevalence of MMC was 5%. The average distance between the MB and ML canals was higher when there was an MMC [M(SD) = 3.61 (0.6) vs. 2.69 (0.66)]. However, there were no differences between the groups in the distance to the danger-zone area and the tapering degree of the mesial root. On CBCT images, the MMC was clearly visible 3 mm apical to the level of the cementoenamel junction; beyond 3 mm, the MMC could not be traced. CONCLUSIONS: Based upon the results of this study, the average distance between the MB and ML canals was higher when there was an MMC. A lesser degree of taper would be preferred to prepare the MMC than to prepare the other canals. It is recommended that careful examination of the canal 3 mm apical to the cementoenamel junction should be carried out when attempting to detect an MMC.

Vascular malformation of the gastrointestinal (GI) tract along with metachronous lesions elsewhere in the body is extremely rare. We report an unusual case of a female child, with a history of venous malformation excision over the right leg presenting with severe iron-deficiency anemia. Although venous malformations of the small bowel are a rare cause of occult GI bleed, in the presence of cutaneous vascular lesions, there should be a high index of suspicion of a vascular malformation, along with the other causes of GI bleed in children.

TiO2, ZnO, and SnO2 metal oxides were synthesized by the sol-gel method and heterojunctions were fabricated by combining TiO2 with either ZnO or SnO2 in a 1:1 ratio using mechanochemical ball milling process. The ball milling process promotes phase transition of TiO2 from anatase to rutile and yields ternary heterojunction of the type TiO2(A)/TiO2(R)/ZnO and TiO2(A)/TiO2(R)/SnO2 (A-anatase and R-rutile). These ternary heterojunctions were characterized by various analytical techniques and its photocatalytic efficiency is evaluated using 4-Chloro Phenol as a model compound under UV and solar light. The enhanced catalytic activity of TiO2(A)/TiO2(R)/ZnO heterojunction is attributed to the formation of Ti3+-Vo defect states which leads to the efficient charge carrier separation. During the ball milling process severe crystal deformation takes place in TiO2 and ZnO lattices by creating crystal lattice distortion which leads to the formation of defects due to valency mismatch between Ti4+ and Zn2+. A mechanistic pathway is proposed for the enhanced photocatalytic activity of the ternary heterojunctions.

RESUMO

Melanoma is a malignant neoplasm of the epidermal melanocytes. Awareness and early recognition of pigmented lesion inside oral cavity helps in initial diagnosis and further investigation and treatment. Oral malignant melanoma is a rare aggressive neoplasm commonly seen among middle age. The diagnosis of melanoma initiates from the pre-existing pigmented lesions. The poor prognosis of oral melanomas requires that pigmented lesions of undetermined origin be routinely biopsied. A case of malignant melanoma of hard palate with its clinical, radiological and histopathological presentation along with brief review is presented. Prognosis of these lesion is poor with survival rate of 5 years.

Background Central venous access devices (CVAD) are an essential part of safe practices in critical care, which enable effective venous access and help in avoiding repeated venipuncture. Discard method is widely practiced for blood sampling. A single occasion of blood sampling may cause minimal blood loss; however, the cumulative volume sequential sampling may become clinically significant. The study aims to reduce diagnostic blood loss, ensuring that the subsequent blood sample is not diluted or contaminated by residual intraluminal fluid. Patients and Methods Within-subjects comparative design was adopted for 64 adult patients in the medical intensive care unit of a tertiary hospital. Two blood samples, using 3 mL and 5 mL discarded volume methods, were collected from each patient. Six serum parameters were measured on each of the paired samples and compared. Statistical Analysis Used Paired t -test and Wilcoxon signed rank test were used for comparing the two methods. Bland-Altman plot analysis and intraclass correlation were used for clinically meaningful analysis. Results When tested for fixed bias, there is no statistically significant difference between the methods. Potassium and creatinine levels showed significant proportional bias. The agreement limits of sodium, potassium, creatinine, and direct bilirubin were outside the clinically accepted interval, but the proportion of samples outside these intervals was less than 10%. All serum parameters showed excellent reliability, except for sodium which demonstrated good reliability. Conclusions The practice of discarding 3 mL of blood for discard method is suggested, instead of the standard 5 mL to reduce iatrogenic blood loss. Thus, nurses in critical care are uniquely positioned to limit the diagnostic blood loss while obtaining blood samples.

The intensive research on the synthesis and characterization of gold (Au) nanostructures has been extensively documented over the last decades. These investigations allow the researchers to understand the relationships between the intrinsic properties of Au nanostructures such as particle size, shape, morphology, and composition to synthesize the Au nano/hybrid nanostructures with novel physicochemical properties. By tuning the properties above, these nanostructures are extensively employed to detect and remove trace amounts of toxic pollutants from the environment. This review attempts to document the achievements and current progress in Au-based nanostructures, general synthetic and fabrication strategies and their utilization in electrochemical sensing and environmental remediation applications. Additionally, the applications of Au nanostructures (e.g., as adsorbents, sensing platforms, catalysts, and electrodes) and advancements in the field of electrochemical sensing of different target analytes (e.g., proteins, nucleic acids, heavy metals, small molecules, and antigens) are summarized. The literature survey concludes the existing methods for the detection of toxic contaminants at various concentration levels. Finally, the existing challenges and future research directions on electrochemical sensing and degradation of toxic contaminants using Au nanostructures are defined.

Integration of semiconducting nanostructures with noble metal nanoparticles are turning highly desirable for cost efficient energy and environmental related applications. From this viewpoint, we report on a facile aqueous synthesis of polymer capped gold (Au) nanoparticles on free standing 2D layered structures of zinc oxide (ZnO) to result with ZnO/Au nanocomposites. Concentration of Au nanoparticles were observed to promote the preferential growth of ZnO along the (002) wurtzite plane. The ZnO/Au structures and their morphological dissemination was noted to be of few. This flake like structure was also noted to be greatly influenced by the concentration of Au in the colloidal blend. Optical band edge transformations noted in the absorption spectra across the lower wavelength region and the shift in surface plasmon resonance (SPR) towards the red region of the visible spectrum signify the improved absorptivity of the heterostructures along the visible spectrum. These heterostructures exhibited remarkable visible light driven photocatalytic activity (99% efficiency) on par with pristine ZnO. The findings also attest this new class of composite structures to open up new openings in diversified solar energy conversion related functions.

In this study, 2D/2D/2D heterostructured r-GO/LTH/ZnO/g-C3N4 nanohybrid were synthesized through hydrothermal method. The strong electrostatic interaction between the negatively charged g-C3N4 and r-GO nanosheets with positively charged layered triple hydroxide (LTH) nanosheets are effectively influences the successful formation of heterojunction. The LTH nanosheets are well spread on the g-C3N4 nanosheets combined with r-GO. In particular, the as prepared heterojunction shows a better photocatalytic degradation activity compared to pristine samples and the significant enhancement in the photocatalytic performance is mainly accredited to the large interfacial charge transition of photogenerated charge carriers under the visible light irradiation. Although the 2D/2D/2D heterojunction effectively hinders the charge carrier recombination resulting high photocatalytic activity with good stability. In addition, the r-GO supported LTH/ZnO/g-C3N4 heterojunction shows high photo-stability after sequential experimental runs with no obvious change in the dye degradation process. Consequently, the role of active species was investigated over the r-GO/LTH/ZnO/g-C3N4 heterojunction with the help of different scavengers.

BACKGROUND: Previous acute studies suggest the Glu298Asp polymorphism (rs1799983) may influence vascular reactivity in response to long-chain n-3 PUFA intake. However, the effects of this genotype on postprandial vascular function after meals rich in SFAs, n-6 PUFAs, and MUFAs are unclear. OBJECTIVES: This study determined the impact of the Glu298Asp polymorphism on changes in vascular function and cardiometabolic risk biomarkers in response to sequential meals of varying fat composition. METHODS: In a randomized, double-blind, crossover, acute study, 32 postmenopausal women (mean ± SD age: 58 ± 5 y; BMI: 25.9 ± 4.1 kg/m2) consumed mixed meals (breakfast: 0 min, 50 g fat; lunch: 330 min, 30 g fat) containing SFAs, n-6 PUFAs, or MUFAs on 3 occasions. Blood samples for cardiometabolic disease risk markers and real-time measures of vascular reactivity [including flow-mediated dilatation (FMD; primary outcome)] were collected/performed before and regularly for 480 min after breakfast. Participants were retrospectively genotyped for the Glu298Asp (rs1799983) polymorphism. Data were analyzed using linear mixed models. RESULTS: For the postprandial %FMD response, a test fat × genotype interaction was observed for the AUC (P = 0.019) but not incremental AUC (IAUC), with the AUC being ∼24% greater after MUFA- than after SFA- and n-6 PUFA-rich meals in the Glu298 homozygotes (P ≤ 0.026). Test fat × genotype interactions were also evident for postprandial insulin (P ≤ 0.005), with the MUFA-rich meals demonstrating significantly higher AUC (12.8%/14.9%), IAUC (14.6%/20.0%), and maximum concentration (20.0%/34.5%) than the SFA- and n-6 PUFA-rich meals, respectively, in Asp298 carriers (P < 0.05). Genotype did not influence other study outcome measures in response to the test fats. CONCLUSIONS: Our findings suggest the Glu298Asp polymorphism may represent a potential determinant of the inter-individual variability in postprandial responsiveness of %FMD and insulin to acute meal fat composition in postmenopausal women. Further studies are required to confirm these observations.This trial was registered at clinicaltrials.gov as NCT02144454.

INTRODUCTION: Foot problems are most common among patients with diabetes, and they are an important cause of morbidity in patients with diabetes mellitus (DM). They can be prevented by following a simple foot care practice. The study objective was to stratify the level of risk for diabetic foot ulcer (DFU) among patients with diabetes by screening. MATERIALS AND METHODS: A descriptive cross-sectional study design was adopted in this study. The sample size was 196, and a standardized INLOW'S 60-s diabetic foot screening tool was used to assess the risk of diabetic foot. Data collection was done by face-to-face interview, and diabetic foot risk (DFR) was assessed by direct observation, inspection, and palpation methods. RESULTS: Among the 196 patients, majority required yearly foot screen in both feet. The mean and standard deviation of the level of DFR in the left foot (LF) and right foot (RF) was 4.31 ± 2.267 and 4.51 ± 2.391, respectively. There was a statistically significant association between the practice of treatment and level of foot screening recommendation in the LF with Chi-square value of χ2= 8.20 (df = 2) and RF with Chi-square value of χ2= 7.95 (df = 2) at P < 0.05 level. CONCLUSION: Health-care providers should be made aware of the regular practice of screening the foot of diabetic patients along with health education awareness programs. It may be helpful to identify the foot ulcer risk at an early stage. It will prevent further complications of DFU and recurrence of ulcer and will reduce the economic burden to patients and their family members and the health-care system.

Silver nanoparticles doped with FCNT-TiO2 heterogeneous catalyst was prepared via one-step chemical reduction process and their efficacy was tested for hydrogen production under solar simulator. Crystallinity, purity, optical properties, and morphologies of the catalysts were examined by X-Ray diffraction, Raman spectroscopy, UV-Visible diffuse reflectance spectra, and Transmission Electron Microscopy. The chemical states and interface interactions were studied by X-ray photoelectron spectroscopy and Fourier transform infrared spectroscopy. The optimized catalyst showed 19.2 mmol g-1 h-1 of hydrogen production, which is 28.5 and 7 times higher than the pristine TiO2 nanoparticles and FCNT-TiO2 nanocomposite, respectively. The optimized catalyst showed stability up to 50 h under the solar simulator irradiation. The natural solar light irradiated catalyst showed ~2.2 times higher hydrogen production rate than the solar simulator irradiation. A plausible reaction mechanism of Ag NPs/FCNT-TiO2 photocatalyst was elucidated by investigating the beneficial co-catalytic role of Ag NPs and FCNTs for enhanced hydrogen production.

BACKGROUND: Early identification of myocardial infarction (MI) is a determinant in the provision of appropriate treatment modalities. The focus of the present study is on the identification of gender-based differences in risk factors, clinical manifestations, and coronary angiography findings in patients presenting with MI. MATERIALS AND METHODS: A cross-sectional study was conducted among patients admitted with MI at a tertiary care center in South India during March 2016 to June 2017. Selected 120 male and 120 female consecutive patients admitted with acute MI, who had survived and been stabilized. Data was collected using a pre-tested structure data sheet. Appropriate parametric and nonparametric tests were used to analyze the data. RESULTS: Participants were homogenous as regards age (P < 0.107); majority of men and women were from the rural areas. About 32.5% of the men interpreted the pain as due to a cardiac problem or indigestion, whereas 60.8% of the women thought it was fatigue/muscle pain. The self-interpretation or perception of pain in both genders was statistically significant (P < 0.001). Compared to the men, the females increasingly presented with atypical symptoms (P = 0.005). Regarding ST-elevated MI, male preponderance was noted (P = 0.004)). Considering the anatomical location of MI, the presentation of Inferior Wall Myocardial Infarction (IWMI) was predominant in females compared to men (P = 0.003). The majority of men had increased presentation of single-vessel disease compared to women (P = 0.02), whereas normal coronaries and double-vessel disease were found statistically significantly higher in females (P = 0.03 and P = 0.008, respectively). CONCLUSION: Public education is needed on the atypical presentations which are common with women than in men. The public should, therefore, be informed of those symptoms and how to recognize them so that they may seek medical care promptly.
