RESUMO

When a patient with a fractured anterior tooth visits the clinic, clinician has to restore the tooth esthetically and quickly. For esthetic resin restoration, clinician can use 'Natural Layering technique' and an index for palatal wall may be needed. In this case report, we introduce pre-restoration index technique on a Class IV defect, in which a temporary filling material is used for easy restoration. Chair-side index fabrication for Class IV restoration is convenient and makes a single-visit treatment possible.

RESUMO

PURPOSE: It is well known that papillary thyroid cancer (PTC) has a good prognosis and high incidence in young women. The objective of the study was to review our criteria to select surgical modality and surgical results and to confirm the feasibility of endoscopic thyroidectomy according to the preoperative risk stratification in the treatment of papillary thyroid cancer. METHODS: Between May 2009 and April 2012, 197 patients underwent either a conventional open or an endoscopic thyroidectomy with the preoperative consideration of risk group using clinical parameters of AMES system (patient age, size of tumor, extrathyroid extension, and presence of distant metastasis). A retrospective analysis of the pathologic data according to AMES system after surgery was also conducted. The endoscopic thyroidectomy was performed using the BABA (bilateral axillo-breast approach) method. RESULTS: Based on the method of operation, the low-risk group patients (n=197) were divided into two groups: endoscopic group (n=78) and open group (n=119). Procedure time, postoperative complication rates, and length of hospital stay were tracked, albeit these were not significantly different between the two groups. From the pathologic findings, it was reported that there were no significant differences in tumor size, extrathyroid extension, and metastasis and also in the Off T4 – thyroglobulin level between the two groups. CONCLUSION: Endoscopic thyroidectomy is a feasible method for the treatment of selected cases of thyroid cancer such as low-risk group according to the appropriate preoperative risk stratification.

RESUMO

PURPOSE: Konkuk University Hospital (KUH), which opened in September 2005, is currently categorized as a secondary hospital. Early on after its establishment, the surgical residents and nurses were relatively inexperienced in the treatment of stomach cancer. Therefore, the quality of surgery for stomach cancer at KUH may be different from that of the existing large-scale tertiary hospitals. The purpose of this study is first to investigate the clinicopathological characteristics of the gastric cancer patients at the KUH, and second to compare our morbidity & mortality rates with those of previous studies, and we also analyzed the risk factors of morbidity at the early stage of a training hospital. MATERIALS AND METHODS: This study retrospectively collected the clinicopathological characteristics and the post-operative morbidity rates and mortality rates with using the electronic medical records of all the patients who went under a gastric cancer operation at KUH from September 2005 to April 2008. RESULTS: The total number of gastric cancer patients who underwent operation was 201. The morbidity rate and death rate at KUH were 10.4% and 0.5%, respectively. The morbidity has increased with an older age. The other variables had no influence on morbidity. CONCLUSION: The morbidity rate, death rate and the clinicopathological characteristics of gastric cancer patients at KUH were similar to those of the previous reports. We found that age is the main factor affecting the morbidity rate after stomach cancer surgery. For further surgical qualification of stomach cancer surgery at KUH, it is necessary to collect the survival data of patients who undergo stomach cancer surgery.

RESUMO

The incidence of Tuberculosis among the patients with end-stage renal disease(ESRD) has increased up to 16 times of that in the general population. The impairment of the cellular immunity in the ESRD patients may have a role in the pathogenesis. Extrapulmonary tuberculous manifestations such as lymph node, peritoneum, and pleura involvement are more frequent in the ESRD patients than in the general population. However, there has been no case of upper gastrointestinal(UGI) bleeding as a result of a gastric tuberculosis in the ESRD patient on hemodialysis. Here we report an unusual case of a hemodialysis patient with UGI bleeding secondary to a tuberculous gastric ulcer. A 31-year-old female on hemodialysis was admitted with melena. Endoscopy revealed a benign gastric ulcer with a visible bleeding vessel at the base, located in the anterior wall of the antrum. An exploratory laparotomy showed multiple, round, small and yellow nodules on the visceral peritoneum as well as a 1-cm sized gastric ulcer. After gastric resection, a histological examination including peritoneal nodules demonstrated chronic granulomatous inflamation with caseous necrosis and giant cells. The patient has been on antituberculosis medication and followed up in the outpatient department without any event for 8 months.
