RESUMO

Thrombotic events associated with SARS-CoV-2 at the vascular endothelium still remains unclear. The aim of the current study is to determine the relationship between cellular proteins on the (ocular) vascular endothelial surface and the immune thrombotic and/or endotheliopathy process elicited by SARS-CoV-2 using an in-silico modeling. The structural S (spike glycoprotein), N (nucleocapsid protein), M (membrane protein), and E (envelope protein) proteins, an accessory protein (ORF1ab) of SARS-CoV-2 and 158 cellular proteins associated with retinal vascular endothelial cell surface or structure were included in this study for comparison of three-dimensional (3D) structure and sequence. Sixty-nine of the retinal proteins were obtained from the Uniprot database. Remaining proteins not included in the database were included in the study after they were converted into 3D structures using the RaptorX web tool. Sequence and three-dimensional structure of SARS-COV-2 S, N, M, E, ORF1ab proteins and retinal vascular endothelial proteins were compared with mTM-align server. Proteins with significant similarity (score above 0.5) were validated with the TM-align web server. Immune and thrombosis-related protein-receptor interactions of similar proteins was checked with CABS-dock. We detected a high level of structural similarity between E protein and ACE, ACE2, LAT1, and TM9SF4 endothelial proteins. In addition, PECAM-1 was found to be structurally similar to ORF1ab and S protein. When we evaluated the likelihood/potential to stimulate an immune responses/a cytokine release, TLR-2 and TLR-3, which are highly susceptible to SARS-CoV2, showed a potential receptor-protein interaction with retinal vascular endothelial proteins. Our study demonstrates that SARS-CoV-2 proteins may have structural similarities with vascular endothelial proteins, and therefore, as immunological target sites, the counterpart proteins on the endothelial surface of many organs may also be secondarily affected by any immune response against SARS-CoV-2 structural proteins.

Objectives: The current study aimed to test the quality and educational utility of the videos of retinal detachment surgery available on YouTube. Methods: YouTube was searched using the keywords "retinal detachment, retinal detachment surgery" in October 2019. The steps of pars plana vitrectomy surgery were standardized according to the literature. The videos were evaluated in a blinded fashion by two retina specialists with the use of the Modified DISCERN, Health on the Net Foundation (HON)code, the Journal of American Medical Association (JAMA) checklist, and the Global Quality Score (GQS). Results: A total of 125 videos were reviewed. The total surgery score was 3.11 (2.16-3.67 IQR), and the most commonly shown surgical stages were photocoagulation and core vitrectomy (n = 99 [79.2%] and n = 97 [77.6%], respectively). In the multivariate linear regression model, the duration of video (minutes) (ß- Coefficient 0.033, p<0.001), the HONcode score (ß-Coefficient 0.310, p=0.005), and GQS (ß-Coefficient 0.768, p<0.001) were associated with better educational content, whereas JAMA and Modified DISCERN scores were not. Conclusion: So far, the educational value of YouTube videos has been unsatisfactory. Assessing the videos using tools such as HONcode, video duration and GQS may provide better educational perspective to ophthalmology trainees.

Purpose: Placoid-pigment-epitheliopathies like Acute-Posterior-Multifocal-Placoid-Pigment Epitheliopathy (APMPPE) and Relentless-Placoid-Choroidopathy (RPC) usually show bilateral involvement. This retrospective case series describes the clinical features with unilateral disease onset.Methods: 21 patients from 9 tertiary referring institutions were collected and evaluated.Results: Seventeen patients were included: 11 (65%) had unilateral placoid-pigment-epitheliopathy consistent with APMPPE, 6 (35%) with RPC (mean age: 28.82 ± 9.1 years). Mean follow-up time was 45 ± 48 months. Forty-one percent fellow-eyes had initial inflammatory signs including vitreous-cells, vascular-sheathing, and ONH swelling. Thirty-five percent developed chorioretinal lesions during follow-up after 19.5 ± 16.9 months. Initial inflammation in fellow-eye was not associated with fellow-eye involvement (p = .5). However, subfoveal choroidal-thickness (SCT) at baseline was significantly thicker in fellow-eyes who developed chorioretinal lesion compared to fellow-eyes who did not (400 ± 70 µm versus 283 ± 127 µm,p = .047).Conclusion: Unaffected fellow-eyes may have inflammatory signs at initial disease-onset and may develop lesions over time. They should be carefully monitored, particularly if they present with an increased SCT.

RESUMO

Ophthalmomyiasis is the infestation of the eye with fly larvae. Oestrus ovis is one of the most common causes of ophthalmomyiasis worldwide. Herein, we describe a case of external ophthalmomyiasis caused by Oestrus ovis in an 18-year-old female living in the city centre of Istanbul and who had no history of contact with any animal.

PURPOSE: To evaluate the changes in several ocular parameters, including choroidal thickness, during and after hemodialysis. METHODS: Twenty-seven eyes of 27 patients with chronic renal failure undergoing hemodialysis were included. The patients underwent an ophthalmic examination, including intraocular pressure, central corneal thickness, iridocorneal angle, subfoveal choroidal thickness, and blood pressure, just before a hemodialysis session, during the second hour of the session, and half an hour after the end of the session. Body weight was measured before and after the session, and ultrafiltration volume was noted after the session. Central corneal thickness, intraocular pressure, and subfoveal choroidal thickness were measured by optical coherence tomography. RESULTS: In comparison with baseline levels, mean intraocular pressure and central corneal thickness increased significantly during the second hour of hemodialysis (p=0.001 and p=0.011, respectively) and showed no significant changes after hemodialysis (p=0.844 and p=0.246, respectively). Mean iridocorneal angle did not significantly change during the second hour of hemodialysis (p=0.101) and after hemodialysis (p=0.589). Mean subfoveal choroidal thickness was significantly lower during the second hour of hemodialysis (p<0.001) and after hemodialysis (p<0.001). CONCLUSIONS: Mean intraocular pressure and central corneal thickness increased and subfoveal choroidal thickness decreased during the second half of the hemodialysis session. During the second half of the session, intraocular pressure and central corneal thickness had a trend toward reduction, while subfoveal choroidal thickness had a relatively steady course. Because of possible fluctuations in the values, it would be reasonable to evaluate the changes not only from before to after hemodialysis but also during hemodialysis when analyzing the ocular effects of dialysis.

RESUMO Objetivo: Avaliar as alterações em vários parâmetros oculares, incluindo a espessura da coroide, durante e após a hemodiálise. Métodos: Foram incluídos 27 olhos de 27 pacientes portadores de insuficiência renal crônica e submetidos a hemodiálise. Todos foram submetidos ao exame oftalmológico, incluindo pressão intraocular, medida da espessura corneana central, ângulo iridocorneal e espessura da coroide subfoveal, além da medida da pressão arterial. A espessura corneana central, o ângulo iridocorneal e a espessura da coroide subfoveal foram medidos através da tomografia de coerência óptica. Os exames foram realizados logo antes, durante (na segunda hora) e meia hora após uma única sessão de hemodiálise. O peso corporal foi medido antes e depois da sessão, e os volumes de ultra filtração foram anotados após a sessão de hemodiálise. Resultados: Em comparação com os níveis basais, a pressão intraocular média e a espessura corneana central aumentaram significativamente na segunda hora de hemodiálise (p=0,001 e p=0,011, respectivamente), mas não houve diferença após a hemodiálise (p=0,844 para pressão intraocular e p=0,246 para espessura corneana central). O ângulo iridocorneal mostrou ligeira diminuição na segunda hora de hemodiálise (p=0,101) e após a sessão de hemodiálise (p=0,589), porém esta redução não foi estatisticamente significativa. Já a espessura da coroide subfoveal mostrou uma redução significativa tanto na segunda hora de hemodiálise (p<0,001) quanto após a sessão de hemodiálise (p<0,001). Conclusões: A pressão intraocular e a espessura corneana central aumentaram, enquanto a espessura da coroide subfoveal diminuiu na primeira metade da sessão de hemodiálise. Na segunda metade da sessão, a pressão intraocular e a espessura corneana central mostraram uma tendência de redução, enquanto a espessura da coroide subfoveal permaneceu estável. Ao analisar os efeitos oculares da hemodiálise, seria interessante avaliar as mudanças não só antes e depois da sessão, como também durante a hemodiálise, devido a possíveis flutuações dos valores.

PURPOSE: The purpose of the study was to calculate serum total oxidant status (TOS), total antioxidant status (TAS), and dynamic thiol-disulphide (T-D) homeostasis in patients with age-related macular degeneration (AMD), and compare the results with healthy individuals. METHODS: Thirty-three exudative AMD patients and 33 healthy controls were included in this case-control study. Participants' serum TAS and TOS levels were measured. In addition, total thiol (TT), native thiol (NT), and disulphide (DS) concentrations were assessed using a novel automated method of measurement. RESULTS: In comparison with the control group, serum TAS, TT, and NT levels were found to be significantly lower (p < 0.0001, p = 0.004, p = 0.003, respectively) and TOS levels were detected higher (p = 0.032) in AMD patients. Serum DS levels were elevated in the AMD patient group, but the difference was not statistically significant (p = 0.219). DS/TT and DS/NT ratios were significantly higher (p = 0.012, p = 0.013, respectively) in AMD patients. A positive correlation was found between TT and NT (p < 0.0001) in AMD group. CONCLUSIONS: Serum TOS levels are higher, TAS levels are lower, and the T-D balance is shifted to the DS bond side in AMD patients. These results suggest that increased oxidative stress and decreased antioxidant levels may play a role in AMD progression. Further studies are needed to confirm the pathophysiologic role of T-D homeostasis in AMD.

To evaluate the association between angiotensin I-converting enzyme insertion/deletion (ACE I/D) gene polymorphism and retinal vein occlusion (RVO). A total of 80 patients with retinal vein occlusion who was admitted to the Eye Department of Kartal Training and Research Hospital between 2008 and 2011, and 80 subjects were enrolled in this retrospective case-control study. Patients who experienced RVO within one week to six months of study enrolment were included, and those with coronary artery diseases, prior myocardial infarction history and coagulation disturbances were excluded from the study. The diagnosis was made by ophthalmoscopic fundus examination and fluorescein angiography. The ACE gene I/D polymorphism was determined by polymerase chain reaction, and the ACE gene was classified into three types: I/I, I/D and D/D. In multivariate logistic regression analysis, ACE D/D genotype (p = 0.035), diabetes-mellitus (p = 0.019) and hypertension (p = 0.001) were found to be independent predictive factors for RVO. The results of the present study reveal that ACE D/D polymorphism is an independent predictive factor for RVO. However, one cannot definitely conclude that ACE gene polymorphism is a risk factor for retinal vein occlusion.

PURPOSE: The current study aimed to investigate short- and long-term postoperative ocular refractive effects of medial rectus recession on the involved eye and to describe the detectable effects in detail by the results of vector analysis. METHODS: Data of patients in whom medial rectus recession was performed between 2010 and 2012 were retrospectively evaluated. The study included 52 eyes of 32 patients. Patients with a history of previous ocular surgery, congenital or progressive corneal disease, familial or acquired posterior segment disease, glaucoma, a history of ocular trauma, or any neurological or systemic disease were excluded. Best-corrected and uncorrected visual acuities and refraction examination findings of the patients were recorded before the surgery and at the postoperative first month and first year. A vector analysis program was used to analyze surgically induced astigmatism. RESULTS: Significant myopic shift and astigmatic change were determined in the spherical equivalent refraction in the early postoperative period (first month); however, these changes disappeared in the long term (at the postoperative first year). A positive correlation was not observed between the amount of surgical recession and the results of ophthalmic examination at the postoperative first month and first year. CONCLUSIONS: Only postoperative first month spherical equivalent showed a positive correlation with the amount of surgical recession; however, this correlation disappeared at the postoperative first year.

BACKGROUND: To evaluate the 3-year clinical outcomes after toric implantable collamer lens (ICL) implantation for the management of moderate to high myopic astigmatism. METHODS: Thirty-four eyes of 20 patients who underwent toric ICL implantation were reviewed. All eyes completed 3-year follow-up. Uncorrected (UDVA) and corrected (CDVA) distance LogMAR visual acuities, refraction, endothelial cell density (ECD), and surgical complications were evaluated. Vectorial analysis of astigmatic correction was also done. RESULTS: A significant improvement in UDVA, CDVA, manifest spherical and cylindrical refraction was observed at 1 week and remained stable after 3 years. Twenty-six eyes (76.5%) gained lines of CDVA, and two eyes (5.9%) showed a loss of 1 line of CDVA. The spherical equivalent (SE) was within ±0.50 D of emmetropia in 18 eyes (52.9%) and within ±1.00 D in 28 eyes (82.4%). Differences between target-induced astigmatism (TIA) and surgically-induced astigmatism (SIA) were statistically significant (p < 0.01), and a trend to undercorrection of the refractive astigmatism was present after 3 years. The magnitude of flattening effect (FE) was found to be significantly lower than the magnitude of TIA (p < 0.01). The magnitude of the torque vector was always positive, with a value below 0.50 D in all cases. No vision-threatening complications were observed during the follow-up. CONCLUSION: Toric ICL implantation is an effective and safe surgical option that provides a relatively predictable and stable refractive correction of myopic astigmatism. Further improvements are needed to minimize the degree of undercorrection.

PURPOSE: To compare astigmatic keratotomy (AK) outcomes in high astigmatism after deep anterior lamellar keratoplasty (DALK) and after penetrating keratoplasty (PK) in keratoconus patients. DESIGN: Prospective, comparative, interventional case series. METHODS: This study comprised 20 eyes that underwent DALK and 24 eyes that underwent PK. After suture removal, all eyes had more than 5 diopters (D) of astigmatism and underwent standard manual 1-pair, 90-degree, and 90% corneal thickness AK incisions. The main outcome measures included preoperative and postoperative manifest refraction, uncorrected visual acuity, best spectacle-corrected visual acuity, surgically induced astigmatism, Orbscan II (Bausch & Lomb) corneal topography results, keratometric astigmatism, and complications. RESULTS: All eyes completed 6 months of follow-up. The overcorrection rate was 35% and 41.6% in the DALK and PK groups, respectively (P=.75). At 6 months after AK, logarithm of the minimal angle of resolution uncorrected visual acuity improved from 0.88 ± 0.20 to 0.54 ± 0.26 and from 1.0 ± 0.34 to 0.53 ± 0.26 in the DALK and PK groups, respectively (P=.01 to P<.01). Best spectacle-corrected visual acuity improved from 0.16 ± 0.09 to 0.13 ± 0.08 and from 0.16 ± 0.12 to 0.11 ± 0.08 in the DALK and PK groups, respectively (P=.13 to P=.01). The mean refractive cylinder was decreased 2.74 ± 1.44 D in the DALK group and 3.18 ± 2.96 D in the PK group (P=.35). Surgically induced astigmatism was 6.10 ± 3.27 D in the DALK group and 7.15 ± 2.98 D in the PK group (P=.36). CONCLUSIONS: The manual AK for the treatment of postkeratoplasty astigmatism after DALK and PK in keratoconus patients is a safe and effective surgical procedure, allowing similar refractive cylinder reduction and improvement in uncorrected visual acuity and best spectacle-corrected visual acuity.
