The data on the risk factors and etiology of ischemic stroke in 31 infants, aged under 3 years, are summarized. The results of genotyping of blood coagulation and folic acid gene polymorphisms in patients and 83 healthy people are presented. Significant differences were found for -455 G>A FGB (Ñ=0.03) and 807 C>T ITGA2 (Ñ=0,005) polymorphisms. Different gene-gene combinations that can cause hypercoagulation and arterial thrombosis in this age were identified. The most frequent combinations include polymorphisms of genes for FGB, fibrinolysis system and folate cycle enzymes (OR=3,79 and more, p<0.05). A clinical case of ischemic stroke in a girl, aged 10 months, after operated congenital heart malformation is presented.

New methods for the diagnosis of the endogenous intoxication syndrome (EIS) in critical states have been developed and tried in 585 infants. A complex of biochemical, hemorheological, biophysical, and optic polarization parameters characterizing the intensity of catabolic reactions and united under the term "endotoxicogram" is offered. Objective assessment of EIS is expressed by an integral index of intoxication which permits a universal assessment of the severity of patient's status, disease course, and efficacy of therapy and helps predict the disease outcome.

A study was made of platelet alpha 2- and beta 2-adrenoreceptors stimulated by adrenaline and alupent as well as of hemostatic system parameters the levels of ACTH, cortisol, glucagon and C-peptide in the blood of women with type I diabetes mellitus during pregnancy, surgical delivery, and the postoperative period. It is shown that the changes in the sensitivity of both subtypes of platelet adrenoreceptors are closely related to the activation of the stress-realizing systems and may serve as a test for estimating the intensity of neurohumoral imbalance both during pregnancy and surgical intervention. The sensitivity of platelet adrenoreceptors to agonists may be examined by a simple retention test allowing rapid information to be derived, which is of paramount importance under clinical conditions.

The pregnant suffering from nephropathies were found to have autonomic nervous control disorders closely related to the major platelet vascular and plasma hemostatic, hemodynamic, and microcirculatory parameters. A combined examination of the macro- and ++micro-hemodynamics and status of adrenoreactive structures assessed the adaptation of a pregnant to nephropathy and her reserve potentialities. Adrenoreactive structure derangement is a measure of the severity of nephropathy. Abnormalities along the whole vasculature are characteristic of dysadaptation.

Of 528 patients subjected to radical operations on the stomach for cancer, polyposis and ulcerous disease the dumping-syndrome was revealed in 74 patients. Following resection of the distal gastric portion the latter was observed in 49 cases, after resection of the cardia--in 18, and after gastrectomy--in 7. In most patients with the dumping-syndrome an impairment of basic hepatic functions and motor-evacuative function of the gastric stump was observed. The study of the work capacity in patients with the dumping-syndrome has deomonstrated its reduction in most patients (in 74% of cases). 55 of 74 investigated patients proved to be invalids of the second and third group. The capacity for work was mostly impaired in patients showing the dumping-syndrome of average and severe gravity.

In 282 patients, subjected to gastric resection for cancer, the working capacity was examined according to the data of the Medical Expertise within different terms after surgery. It was found that most of patients in primary examination were acknowledged as disabled workers of the first and second group (70.1%). A dynamic follow-up of patients in late postoperative periods revealed a tendency toward an improvement of their working capacity in a group of patients previously considered as totally disabled. However, in terms over 5 years following the operation more than half of patients proved to be disabled. A comparative estimation of the working capacity in these patients according to the Medical Expertise and personal authors' data has revealed a discrepancy between the group of disability and the work accomplished, i.e. a considerable number of patients, considered as group II, resumed their former activities (as prior to surgery) or did easier jobs. This discrepancy seems, to a certain extent, to be the result of underestimation of the patients' state by the Medical Expertise Commission and overrating the group of disability in them.
