RESUMO

Desmoplastic fibroma is a rare bone tumor with benign, locally aggressive, lytic behavior. Histologically, it is very similar to the more common and better known desmoid tumor. It can occur at any site in the skeleton but is most common in the mandible or in the long bones. We present two cases; the tumor was located in the proximal tip of the fibula in one case and in the proximal tip of the femur in the other. We describe the appearance of these lesions on CT and MRI. In both cases, the lesions were lytic, expansive, with a trabeculated appearance and internal septa, and without mineralized osseous matrix. At MRI, one of the tumors was very heterogeneous, with two differentiated areas on T2-weighted sequences: the upper zone was hypointense and the lower zone showed intermediate signal intensity. We also describe the behavior of the tumor after the administration of intravenous Gadolinium: the upper zone showed no significant enhancement and the lower showed significant enhancement in the early phase.

RESUMO

El fibroma desmoplásico es una neoplasia ósea poco frecuente, de comportamiento lítico, benigna y localmente agresiva. Histológicamente es muy similar al más frecuente y conocido tumor desmoide. Se puede presentar en cualquier punto del esqueleto, pero más frecuentemente en la mandíbula o en los huesos largos. Presentamos dos casos, localizados respectivamente en los extremos proximales del peroné y del fémur, y describimos su apariencia radiológica. Ambos casos se presentan como lesiones líticas, expansivas, de aspecto trabeculado y con septos internos, sin matriz ósea mineralizada. En el estudio mediante resonancia magnética (RM), uno de los tumores era muy heterogéneo,con dos áreas diferenciadas en la secuencia T2, la zona superior hipointensa y la inferior de intensidad intermedia. Describimos también su comportamiento tras la administración de gadolinio intravenoso: la zona superior sin un realce significativo y la inferior con un importante realce en fase precoz

RESUMO

La fibromatosis (tumor desmoide) es un tumor benigno de origen mesenquimal. La localización mamaria es excepcional y su etiología es desconocida. La fibromatosis mamaria parece, tanto clínica como radiológicamente, un carcinoma. El diagnóstico es hitológico y el tratamiento de elección es la exéresis quirúrgica. Se presenta uncaso en que el estudio histológico mostró un tumor desmoide extraabdominal, de localización mamaria, en una paciente de 23 años con antecedente de tumorectomía mamaria ipsolateral (AU)


Fibromatosis (desmoid tumors) is a rare benign mesenchytomatous proliferative process. The breast is an excepcional site. Etiology is still unknown. Fibromatosis of the breast mimics carcinoma clinically as well as radiographically. Diagnosis is made by histological studies. The treatment of choice is wide excision. We report the case of a 23-year-old woman with a previous ipsilateral breast tumorectomy (AU)

RESUMO

Aunque la mastectomía subcutánea es una técnica quirúrgica que pretende eliminar todo el tejido mamario macroscópico, conservando el complejo areola-pezón, suele quedar tejido glandular residual en la zona retroareolar, cola de mama y colgajos cutáneos. Por consiguiente, existe la posibilidad de desarrollar carcinoma mamario en este tejido residual. Describimos un caso de carcinoma mamario infiltrante en una paciente en la que previamente se le había realizado una mastectomía subcutánea por presentar un carcinoma intraductal extenso tipo comedo en el seno de un fibroadenoma, que sobrepasaba la cápsula de éste


Although subcutaneous mastectomy aims to remove the entire macroscopic breast gland while conserving the nipple-areola complex, some residual glandular tissue usually remains in the nipple, subareolar region, axillary tail and skin flaps. Consequently, there is a risk of developing breast cancer in the residual tissue. We describe the case of a patient with infiltrating breast cancer who had previously undergone subcutaneous mastectomy for an extensive comedo-type intraductal carcinoma within a fibroadenoma that spread through the fibroadenoma capsule
