RESUMO

The liposarcoma is a malignant tumor of mesodermic origin derived of the adipose tissue. Liposarcoma s types, according to his histological diagnosis, are: mixoide, pleomorphic, well differentiated and dedifferentiated. It can get to reach enormous proportions, mainly when it is located at abdominal level. His treatment is the radical surgery, it is possible, together with radiation therapy and/or chemotherapy. Four patients diagnosed of liposarcoma are shown up, a case of liposarcoma well differentiated, another case of liposarcoma pleomorphic and two cases about liposarcoma mixoide; with the characteristic that one of these two cases presented a local recidivation with a dediferenciation of itself. The evolution of the four cases, was in a different way. So, their prognosis is going to depend on their histopathologic variety and on a possible multicentricidad that is going to difficult the complete radical surgical exeresis.

RESUMO

El liposarcoma es un tumor maligno de origen mesodermico derivado del tejido adiposo. Los tipos de liposarcoma según su diagnóstico histológico son: mixoide, pleomórfico, bien diferenciado y desdiferenciado. Puede llegar a alcanzar enormes proporciones, sobre todo cuando se localiza a nivel abdominal. Su tratamiento es la cirugía radical, si es posible, junto con radioterapia y/o quimioterapia. Se presentan cuatro pacientes diagnosticados de liposarcoma, un caso de liposarcoma bien diferenciado, otro caso de liposarcoma pleomórfico y dos casos de liposarcomas mixoides; con la característica de que uno de estos presento una recidiva local con una desdiferenciación del mismo. Los cuatro evolucionaron de forma diferente. Así su pronóstico va a depender de su variedad histopatológica y de una posible multicentricidad que dificulta la exéresis quirúrgica radical completa

RESUMO

The tumour dissemination to penis from renal cell carcinoma is a rare occurrence; generally, it is seen in advanced stages of the disease, such that occurred in our patient that indicated a poor prognosis. A lesion node, high, painless, the size middle, located in the side right to penis, at long term free of symptoms associated was found. The histological examination of the specimen after cold biopsy demonstrated a metastasis from renal carcinoma.

La diseminación tumoral a pene por una neoplasia renal es un hecho poco habitual, ocurriendo generalmente en estadios muy avanzados, tal como sucedió en el enfermo comunicado, y lleva asociado un mal pronóstico. Se trata de una lesión nodular, elevada, indolora, de moderado tamaño, localizada en hemipene derecho, de larga evolución en ausencia de síntomas acompañantes; el estudio histológico después de biopsia fría demostró una metástasis por carcinoma renal. RESUMO

The syphilis is a venereal disease of sexual transmission that incidence is increasing in several groups populations. We report a case of a patient with secondary syphilis who consulted for an atypical solid mass to penile. The clinical evolution, histological examination and analytical tests confirmed the diagnosis of genital syphilis having a satisfactory response to specific antibiotic treatment. The epidemiological, clinical, diagnostic aspects and therapeutic of the genital ulcer disease are discussed.

Numerous studies of many tumor types have demonstrated that microvessel quantitation as a measure of angiogenesis is a powerful prognostic tool. Vascular enumeration has been claimed to be an independent prognosticator for several human tumors, including breast carcinoma, melanoma or bladder carcinoma; however, the studies of colorectal cancer have rendered variable results. To test the prognostic influence of this factor in our patients, we selected 39 patients with rectal carcinoma Dukes' stages A to C treated only with curative surgery, with no further adjuvant therapy. The minimal follow-up time was 5 years (60 months). After immunostaining with CD34, we performed a manual count of the vessels following Gasparini's criteria. In our series, vascular enumeration has been a prognosticator for OS (overall survival) but not for RFS (relapse-free-survival) at all Dukes' stages in the univariate analysis. This prognostic influence was lost in the multivariate analysis, in which only stage as well as vascular and neural invasion behaved as significant independent prognosticators. The presence of hypervascularization did not show any significant association with histologic grade, tumor staging, and vascular or neural invasion.

RESUMO

Occult prostatic carcinoma (carcinoma diagnosed because of the presence of metastasis) is commoner and commoner; the sites for metastatic disease are progressively more diverse. This is partially due to the longer life of the general population and to the better expectative of life for oncologic patients with modern treatments. We report the first case to our knowledge, of occult prostatic carcinoma diagnosed in a patient with a parenchymatous acute renal failure secondary to diffuse metastatic infiltration. We also review other atypical locations of metastasis from prostatic carcinoma reported in the literature and discuss the ways of spread in these cases.

Castleman's disease (CD) is a enigmatic lymphoid disease of unknown etiology which rarely manifest itself as an isolated pelvic mass. We report a case of pelvic Castleman's disease masquerading as a uterine myoma. The patient presented symptoms related to compression of adjacent structures, splenomegaly and abdominal lymphadenopathy, the laboratory data revealed positive Epstein-Barr virus serology, elevated beta 2-microglobulin level and presence of antinuclear antibodies. The intraabdominal involvement, histological patterns and clinical forms of this condition are reviewed. Likewise etiopathogenic, radiologic and therapeutic aspects related with this entity are discussed. We suggest that pelvic Castleman's disease should be included in the differential diagnosis of females presenting a pelvic mass containing calcifications.

RESUMO

Presentation of a 63-year old female patient with single metastasis in right suprarenal gland from a simultaneous contralateral renal adenocarcinoma. The approach used was left nephrectomy and right suprarenalectomy. Two years after surgery, the patient is alive and shows no evidence of relapse. The extreme rarity of this finding is emphasized, and clinical, diagnostic, therapeutic and prognostic aspects are commented after reviewing the relevant literature.

Adhesion of T cells to extracellular matrix (ECM) proteins through VLA integrin receptors is crucial for lymphocyte trafficking, tissue localization and inflammatory function. We have investigated the expression of different VLA integrins (VLA-1-5) on peripheral blood (PB) and synovial fluid (SF) T lymphocytes from patients with rheumatoid arthritis (RA). Their expression on different cell types from synovial membrane (SM) is also reported. The role of VLA-4 fibronectin (FN) receptors in the interaction of activated SF T cells from RA patients with a 38-kD fragment of FN has been previously demonstrated. Here we have focused functional studies on VLA-5 as an alternative FN receptor for RA T cells. A significant higher proportion of SF T cells were able to bind to an 80-kD fragment of FN, containing the Arg-Gly-Asp (RGD) cell binding site, compared with PB T cells. This attachment was almost completely inhibited by anti-VLA-5 MoAbs as well as by RGD peptides. This enhanced capability by SF T cells appears to be independent of the level of the surface expression of the receptor and correlates better with their activation state as determined by the expression of the activation molecule AIM (CD69). The evidence for the expression of VLA heterodimers on both SF and SM cells from RA patients suggests the possible implication of ECM proteins in mediating and perpetuating inflammation in vivo.
