INTRODUCTION: Hypotension is common during intermittent hemodialysis (IHD) and may be due to a decreased cardiac index (CI). However, no study has simultaneously and continuously measured CI and mean arterial pressure (MAP) to understand the prevalence, severity, and duration of CI decreases or relate them to MAP, blood volume (BV) and net ultrafiltration (NUF) rate. METHODS: In a prospective, pilot and feasibility investigation, we studied 10 chronic IHD patients. We used the ClearSight System™ to continuously monitor CI and MAP; the CRIT-LINE®IV monitor to detect BV changes and collected data on NUF rate. RESULTS: Device tolerance and compliance was 100%. All patients experienced at least ≥ 1 episode of severe CI decrease (> 25% from baseline), with a median duration of 24 minutes [IQR 6-87] and of 68 minutes [14-106] for moderate decreases (>15% but  25% from baseline). Eight patients experienced a low CI state (<2.2 L/min/m2). The lowest CI was 0.9 L/min/m2 with a concomitant MAP of 94 mmHg. When the fall in CI was severe, MAP increased in 58% of cases and remained stable in 28%. Overall, CI decreased by -0.55 L/min/m2 when BV decrease was moderate vs mild (p<0.001) and by -0.8 L/min/m2 when NUF rate was high vs low (p<0.001). CONCLUSION: Continuous CI monitoring is feasible in IHD and shows frequent moderate-severe CI decreases, sometimes to low CI state levels. Such decreases are typically associated with markers of decreased intravascular volume status but not with a decrease in MAP, implying marked vasoconstriction.

INTRODUCTION: Renal replacement therapy (RRT) is associated with hypotension. However, its impact on cardiac output (CO) is less understood. We aimed to describe current knowledge of CO monitoring and changes during RRT. METHODS: We searched MEDLINE, Embase, and Cochrane from January 1, 2000, to January 31, 2023, using Covidence for studies of intermittent hemodialysis (IHD) and continuous RRT (CRRT) with at least three CO measurements during treatment. Two independent reviewers screened citations, and a third resolved disagreements. The findings did not allow meta-analysis and are presented descriptively. RESULTS: We screened 3,285 articles and included 48 (37 during IHD, nine during CRRT, and two during both). Non-invasive devices (electrical conductivity techniques and finger cuff pulse contour) were the most common CO measurement techniques (21 studies). The median baseline cardiac index in IHD studies was 3 L/min/m2 (95% CI, 2.7-3.39). Among the 88 patient cohorts studied, a decrease in CO occurred in 63 (72%). In 16 cohorts, the decrease was severe (>25%). Changes in blood pressure (BP) were not concordant in extent or direction with changes in CO. The decrease in CO correlated weakly with ultrafiltration rate (r = -0.3, p = 0.05) and strongly with changes in systemic vascular resistance (SVR) (r = -0.6, p < 0.001). CONCLUSION: There are limited data on CO changes during RRT. However, a decrease in CO appeared common and was marked in 1 of 5 patient cohorts. Such decreases often occurred without BP changes and were associated with increased SVR.

Those who gamble compulsively, and those who shop or buy in a compulsive manner share a number of common characteristics, stemming from similar impulse-control issues. As such, it is predicted that a lexical analysis of personal narratives of compulsion would share similarities. Using secondary data from an online mental health forum, Psychforums, the research analyzed narratives of compulsive gambling (n = 199) and compulsive buying (n = 196) using the automated text analysis tool, LIWC. The results indicated that compulsive buying narratives rated significantly higher in clout and emotional tone and significantly lower in authenticity, with no significant differences noted in analytical thinking between the two compulsion narratives. Recommendations for future research include that demographic variables be incorporated and that narratives sourced from different online platforms should be contrasted.

Interfacial cues in the tumor microenvironment direct the activity and assembly of multiple cell types. Pancreatic cancer, along with breast and prostate cancers, is enriched with cancer-associated fibroblasts (CAFs) that activate to coordinate the deposition of the extracellular matrix, which can comprise over 90% of the tumor mass. While it is clear that matrix underlies the severity of the disease, the relationship between stromal-tumor cell assembly and cell-matrix dynamics remains elusive. Micropatterned hydrogels deconstruct the interplay between matrix stiffness and geometric confinement, guiding heterotypic cell populations and matrix assembly in pancreatic cancer. Interfacial cues at the perimeter of microislands guide CAF migration and direct cancer cell assembly. Computational modeling shows curvature-stress dependent cellular localization for cancer and CAFs in coculture. Regions of convex curvature enhance edge stress that activates a myofibroblast phenotype in the CAFs with migration and increased collagen I deposition, ultimately leading to a central "corralling" of cancer cells. Inhibiting mechanotransduction pathways decreases CAF activation and the associated corralling phenotype. Together, this work reveals how interfacial biophysical cues underpin aspects of stromal desmoplasia, a hallmark of disease severity and chemoresistance in the pancreatic, breast, and prostate cancers, thereby providing a tool to expand stroma-targeting therapeutic strategies.

BACKGROUND AND AIMS: Mothers who are primary carers of children with disabilities experience more mental health problems than mothers of typically developing children. We aimed to update and extend research into mothers' perceptions of grandparent support with care, and impacts for maternal psychological well-being, because although contemporary grandparents live longer they also work longer. METHODS: Surveys were completed by 72 mothers of clients (aged ≤8 years) of an Australian non-government disability agency. Mothers reported levels of practical and emotional support, face-to-face contact, and affectional solidarity with maternal and paternal grandparents. The Kessler Psychological Distress Scale (K10) was the dependent variable. RESULTS: Mothers indicated high to very high levels of psychological distress. As in previous comparable surveys, most support was offered by maternal grandmothers who had more contact and closest relationships with mothers. Maternal distress was related to maternal grandmothers' health and relationships with maternal grandfathers. Face-to-face contact and closeness of relationships were strongly linked to provision of support. CONCLUSIONS AND IMPLICATIONS: Grandparents are important supports for mothers of children with disabilities: mothers lacking support from their own parents are particularly vulnerable. Clinicians should actively facilitate grandparent involvement, and promote extended family relationships.
