RESUMO

Extracraneal meningiomas represent 2% of all meningiomas and can appear in different locations including paranasales sinuses. There are no statistics regarding ectopic malignant meningiomas, but they are considered extremely rare. Due to this, there is no management protocol. We present a patient with a malignant meningioma of the etmoidal sinus, his treatment and the evolution over a five year period.

Hemangionas of the ear have been rarely described. A few cases have been reported in the world literature involving the middle ear space or tympanic membrane, and a few more involving the inner ear. Cavernous hemangioma often is associated with other hemangiomas, and there is almost always a cutaneous segment that facilitates the diagnosis. The authors report a rare case (to their knowledge, the first to be described) of a subcutaneous cavernous hemangioma of the auricular concha. The diagnosis and treatment are discussed and the literature is reviewed.

RESUMO

OBJECTIVE: To determine whether there is an advantage in safety and outcome efficacy with the use of argon laser in revision stapes surgery as compared with conventional instruments. DATA SOURCES: A search of the published English-language literature, 1970-1995, was conducted using the following key words: revision, surgery, stapes, laser, stapedotomy, and argon laser. STUDY SELECTION: The following inclusion criteria were used to select articles for the meta-analysis: revision cases only, a comprehensive review of intraoperative pathological findings that led to the failure, and accurate documentation by the author, confirmed by our statisticians using a modified chi 2 test. Eleven studies without the use of the laser (n = 1,147 patients) and four studies with the use of the laser (n = 170 patients) including our own patients (n = 23) were entered into the model. DATA EXTRACTION: The data had to meet strict audiometric criteria, including preoperative and postoperative audiogram pure tone average air-bone gap; postoperative audiograms had to include five classifications, and these audiograms had to be obtained a minimum of 6 months after revision surgery. DATA SYNTHESIS: A log-linear model was developed for this meta-analysis study, with each study analyzed individually and collectively. CONCLUSION: Revision stapes surgery using the laser demonstrated statistically significant (p = 0.002) advantage in both safety and efficacy over revision procedures using conventional instruments.

RESUMO

This study was undertaken in order to establish the incidence of aerodigestive tract foreign body accidents among older children and adolescents, and to investigate the circumstances surrounding these events. A review of patients treated over a 5-year period identified 367 children from whom aerodigestive tract foreign bodies were removed. Seventeen percent of these patients were 5 years of age or older. Among these children, 88% aspirated nonfood items, half of which were school supplies; 78% of the group 5 years old and under aspirated food items. Among older children with esophageal foreign bodies, 31% had food impactions, compared with 7% in the younger group; 70% of these children had a history of some anatomic abnormality of the esophagus. These data suggest that older children and adolescents represent a distinct group of patients at risk for foreign body accidents. Pediatricians and parents of children in this age group should discourage the practice of using the oral cavity as a repository for school supplies, and should stress the need for adequate preparation and mastication of food, particularly among children with esophageal abnormalities.
